Latitudinal migration of sunspots based on the ESAI database

The latitudinal migration of sunspots toward the equator,which implies there is propagation of the toroidal magnetic flux wave at the base of the solar convection zone,is one of the crucial observational bases for the solar dynamo to generate a magnetic field by shearing of the pre-existing poloidal magnetic field through differential rotation.The Extended time series of Solar Activity Indices（ESAI）elongated the Greenwich observation record of sunspots by several decades in the past.In this study,ESAI＇s yearly mean latitude of sunspots in the northern and southern hemispheres during the years 1854 to 1985 is utilized to statistically test whether hemispherical latitudinal migration of sunspots in a solar cycle is linear or nonlinear.It is found that a quadratic function is statistically significantly better at describing hemispherical latitudinal migration of sunspots in a solar cycle than a linear function.In addition,the latitude migration velocity of sunspots in a solar cycle decreases as the cycle progresses,providing a particular constraint for solar dynamo models.Indeed,the butterfly wing pattern with a faster latitudinal migration rate should present stronger solar activity with a shorter cycle period,and it is located at higher latitudinal position,giving evidence to support the Babcock-Leighton dynamo mechanism.

Treatment of chronic proliferative cholangitis with c-myc shRNA

AIM: To investigate the feasibility and effectiveness of c-myc shRNA in inhibiting the hyperplastic behavior and lithogenic potentiality of chronic proliferative cholangitis (CPC), in order to prevent stone recurrence and biliary restenosis.METHODS: An animal model of CPC was established by giving intralumenally 0.5 mL of c-myc shRNA. Then, the effects of c-myc shRNA on hyperplastic behavior and lithogenic potentiality of CPC were evaluated by histological observation, immunohistochemistry, real-time PCR and Western blotting for c-myc, proliferating cell nuclear antigen (PCNA), procollagen TM mucin5AC, enzymatic histochemistry for β-glucuronidase, and biochemistry for hydroxyproline in the diseased bile duct.RESULTS: Treatment with c-myc shRNA efficiently suppressed the hyperplasia of biliary epithelium, submucosal gland, and collagen fiber by inhibiting mRNA and protein expression of c-myc. More importantly, it decreased the lithogenic potentiality of CPC by inhibiting the expression of mucin 5AC and the secretion of endogenous β-glucuronidase. further investigation indicated that c-myc shRNA-3 had a better inhibitory effect on CPC.CONCLUSION: Treatment with c-myc shRNA-3 can control CPC and reduce the lithogenic potentiality of CPC.

Extended lymphadenectomy in hilar cholangiocarcinoma: What it will bring?

Lymph node dissection is always a hot issue in radical resection of hilar cholangiocarcinoma(HCCA).There are still controversies regarding whether some lymph nodes should be dissected,of which the para-aortic lymph nodes are the most controversial.This review synthesized findings in the literature using the PubMed database of articles in the English language published between 1990 and 2019 on the effectiveness of extended lymphadenectomy including paraaortic lymph nodes dissection in radical resection of HCCA.Hepatobiliary surgeons have basically achieved a consensus that enough lymph nodes should be obtained to accurately stage HCCA.Only a very small number of studies have focused on the effectiveness of extended lymphadenectomy including para-aortic nodes dissection on HCCA.They reported that extended lymphadenectomy can bring some survival benefits for patients with potential para-aortic lymph node metastasis and more lymph nodes can be obtained to make the patient's tumor staging more accurate without increasing the related complications.Extended lymphadenectomy should not be adopted for HCCA patients with intraoperatively confirmed distant lymph node metastases.For these patients,radical resection combined with postoperative adjuvant chemotherapy seems to be a better choice.A prospective,multicenter,randomized,controlled clinical study of regional lymphotomy and extended lymphadenectomy in HCCA should be conducted to guide clinical practice.A standardized extended lymphadenectomy may help to more accurately stage HCCA.future studies are required to further assess whether extended lymphadenectomy can improve long-term survival in negative celiac,superior mesenteric,and para-aortic lymph node diseases.

Statistical investigation on the formation of sunspot light bridges

light bridges(LBs)are bright lanes that divide one sunspot umbra into two or more parts.Though frequently observed in sunspots,their formation mechanisms have rarely been studied and thus are not well understood.Here we present results from the first statistical investigation on the formation of LBs.Using observations with the Helioseismic and Magnetic Imager on board the Solar Dynamics Observatory,we identified 144 LBs within 71 active regions(ARs)over the whole year of 2014.The formation processes of these LBs can be categorized into three groups:penumbral intrusion(type-A),sunspot merging(typeB)and umbral-dot emergence(type-C).The numbers of events in these three categories are 74,57 and 13,respectively.The duration of the LB formation process is mostly less than 40 hours,with an average of~20 hours.Most LBs have a maximum length of less than 20.For type-A LBs,we found a positive correlation between the LB length and the duration of the LB formation process,suggesting a similar speed of penumbral intrusion in different sunspots.

Effects of epidermal growth factor receptor inhibitor on proliferative cholangitis in hepatolithiasis

BACKGROUND： There is currently no effective medication to prevent stone recurrence after choledochoscopic lithotomy or to treat proliferative cholangitis（PC）, which is the pathologic basis of hepatolithiasis. This study aimed to investigate whether gefitinib, an epidermal growth factor receptor（EGFR） inhibitor, inhibited cholangio hyperplasia and lithogenesis in PC.METHODS： After cholangioscopic lithotomy, indwelling catheters were placed in the diseased bile duct lumens in 94 patients with hepatolithiasis. Subsequently, 49 of the 94 patients were treated with 250 mg gefitinib solution via a catheter twice a week, and they were subjected to choledochoscopic biopsy at 6 and 12 weeks. The rest 45 hepatolithiasis patients without gefitinib treatment served as controls.RESULTS： The expressions of EGFR, PCNA and procollagen I were significantly reduced in the patients treated with gefitinib in 12 weeks compared with those in the control group. Patients in the gefitinib group had a much lower degree of hyperplasia of the biliary epithelium, submucosal glands and collagen fibers compared with those in the control group. Gefitinib treatment significantly decreased mucin 3 expression and β-glucuronidase activity.CONCLUSION： Postoperative gefitinib treatment could significantly inhibit PC-mediated hyperplasia and lithogenesis, which might provide a novel strategy for the prevention of biliary restenosis and stone recurrence in patients with hepatolithiasis.

Hepatic epithelioid hemangioendothelioma: Dilemma and challenges in the preoperative diagnosis

Hepatic epithelioid hemangioendothelioma(HEHE) is a rare category of vascular tumor with uncertain malignant potential. It commonly presents nonspecific and variable clinical manifestations, ranging from asymptomatic to hepatic failure. In addition, laboratory measurements and imaging features also lack specificity in the diagnosis of HEHE. The aim of the present study is to highlight the dilemma and challenges in the preoperative diagnosis of HEHE, and to enhance awareness of the range of hepatobiliary surgery available in patients with multiple hepatic nodular lesions on imaging. In these patients, HEHE should at least be considered in the differential diagnosis.

Repair of a common bile duct defect with a decellularized ureteral graft

AIM To evaluate the feasibility of repairing a common bile duct defect with a decellularized ureteral graft in a porcine model.METHODS Eighteen pigs were randomly divided into three groups. An approximately 1 cm segment of the common bile duct was excised from all the pigs. The defect was repaired using a 2 cm long decellularized ureteral graft over a T-tube(T-tube group, n = 6) or a silicone stent(stent group, n = 6). Six pigs underwent bile duct reconstruction with a graft alone(stentless group). The surviving animals were euthanized at 3 mo. Specimens of the common bile ducts were obtained for histological analysis.RESULTS The animals in the T-tube and stent groups survived until sacrifice. The blood test results were normal in both groups. The histology results showed a biliary epithelial layer covering the neo-bile duct. In contrast, all the animals in the stentless group died due to biliary peritonitis and cholangitis within two months post-surgery. Neither biliary epithelial cells nor accessory glands were observed at the graft sites in the stentless group.CONCLUSION Repair of a common bile duct defect with a decellularized ureteral graft appears to be feasible. A T-tube or intraluminal stent was necessary to reduce postoperative complications.
