Objective:To explore the mechanism of paeoniflorin(PF)on osteoarthritis(OA)synovial inflammation from network pharmacology to experimental pharmacology.Methods:Targets of OA were constructed by detecting the database of network database platforms(Therapeutic Target database,Drug Bank and Gene Cards),and the targets of PF were constructed by Pub Chem and Herbal Ingredients'Targets database.Gene Ontology(GO)and Kyoto Encyclopedia of Genes and Genomes(KEGG)analysis of these co-targeted genes were conducted via Database for Annotation,Visualization,and Integrated Discovery(DAVID)database,and protein-protein interaction(PPI)networks were conducted via the search tool for the retrieval of interacting genes(STRING)database.Cell counting kit-8(CCK-8)assay was performed to assess the potential toxicity of PF on human OA fibroblast-like synoviocytes(FLS),quantitative real-time polymerase chain reaction(q PCR),enzyme-linked immunosorbent assay(ELISA)and Western blot were used to verify the potential mechanism of PF in synovial inflammation.Results:Twenty-six co-targeted genes were identified.GO enrichment results showed that these co-targeted genes were most likely localized in the cytoplasm,and the biological processes mainly involved'cellular response to hypoxia''lipopolysaccharide(LPS)-mediated signaling pathway'and'positive regulation of gene expression'.KEGG pathway analysis indicated that these co-targeted genes may function through pathways associated with'hypoxia-inducible factor-1(HIF-1)signaling pathway'and'tumornecrosis factor(TNF)signaling pathway'.The PPI network showed that the top 3 hub genes were TP53,TNF,and CASP3.Molecular docking results showed that PF was well docking with TNF.CCK-8 showed no potential toxicity of 10,20 and 50μmol/L PF on human OA FLS.And PF significantly decreased the expression levels of interleukin-1β,interleukin-6,TNF-αmatrix metalloproteinase 13(MMP13),and a disintegrin and metalloproteinase with thrombospondin motifs 5(ADAMTS5)and TNF-αin LPS-induced OA FLS.Conclusion:PF exhibited

BACKGROUND Surgery for obese patients carries a higher risk of anesthesia complications compared with surgery for nonobese patients.Thus,a safe and effective anesthesia strategy is necessary to improve the medical experience of such patients and ensure their safety.AIM To compared the effectiveness and safety of remimazolam besylate versus dexmedetomidine(DEX)in gastrointestinal surgery in obese patients.METHODS The study cohort included 60 obese patients undergoing gastrointestinal surgery between July 2021 and April 2023,comprising 30 patients who received DEX intervention(control group)and 30 patients who received remimazolam besylate intervention(research group).Heart rate(HR),respiratory rate(RR),mean arterial pressure(MAP),blood oxygen saturation(SpO_(2)),safety(nausea and vomiting,bradycardia,hypotension,and apnea),anesthesia and examination indices[induction time,anesthesia recovery time,and postanesthesia care unit(PACU)discharge time],sedation effect(Ramsay Sedation Scale),and postoperative pain visual analog scale were comparatively analyzed before anesthesia(T0),during anesthesia(T1),and after anesthesia(T2).RESULTS At T1,the research group showed significantly smaller changes in HR,RR,MAP,and SpO_(2) than the control group,with a significantly lower adverse reaction rate and shorter induction,anesthesia recovery,and PACU discharge times.Additionally,the intra-and postoperative Ramsay Sedation Scale scores were statistically higher in the research group than in the control group.CONCLUSION Remimazolam besylate was significantly more effective than DEX in gastrointestinal surgery in obese patients and had a higher safety profile and value in clinical promotion.

Observing eddy dye patches induced by shear instabilities in the surf zone on a plane beach

The effects of surf zone eddy generated by alongshore currents on the deformation and transport of dye are still poorly understood,and related tracer release experiments are lacking.Therefore,a tracer release laboratory experiment was conducted under monochromatic,unidirectional incident waves with a large incident angle(30°)on a plane beach with a 1:100 slope in a large wave basin.A charge-coupled device suspended above the basin recorded the dye patch image.The evolution of eddy dye patch was observed and the transport and diffusion were analyzed based on the collected images.Subsequently,a linear instability numerical model was adopted to calculate the perturbation velocity field at the initial stage.The observation and image processing results show that surf zone eddy patches occurred and were separated from the original dye patches.Our numerical analysis results demonstrate that the structure of the perturbation velocity field is consistent with the experimental observations,and that the ejection of eddy patches shoreward or offshore may be ascribed to the double vortex.

Storm surge is often the marine disaster that poses the greatest threat to life and property in coastal areas.Accurate and timely issuance of storm surge warnings to take appropriate countermeasures is an important means to reduce storm surge-related losses.Storm surge numerical models are important for storm surge forecasting.To further improve the performance of the storm surge forecast models,we developed a numerical storm surge forecast model based on an unstructured spherical centroidal Voronoi tessellation(SCVT)grid.The model is based on shallow water equations in vector-invariant form,and is discretized by Arakawa C grid.The SCVT grid can not only better describe the coastline information but also avoid rigid transitions,and it has a better global consistency by generating high-resolution grids in the key areas through transition refinement.In addition,the simulation speed of the model is accelerated by using the openACC-based GPU acceleration technology to meet the timeliness requirements of operational ensemble forecast.It only takes 37 s to simulate a day in the coastal waters of China.The newly developed storm surge model was applied to simulate typhoon-induced storm surges in the coastal waters of China.The hindcast experiments on the selected representative typhoon-induced storm surge processes indicate that the model can reasonably simulate the distribution characteristics of storm surges.The simulated maximum storm surges and their occurrence times are consistent with the observed data at the representative tide gauge stations,and the mean absolute errors are 3.5 cm and 0.6 h respectively,showing high accuracy and application prospects.

Chinese medicine cinobufacini is an extract from the dried skin of Bufo bufo gargarizans Cantor,with active ingredients of bufadienolides and indole alkaloids.With further research and clinical applications,it is found that cinobufacini alone or in combination with other therapeutic methods can play an anti-tumor role by controlling proliferation of tumor cells,promoting apoptosis,inhibiting formation of tumor neovascularization,reversing multidrug resistance,and regulating immune response;it also has the functions of relieving cancer pain and regulating immune function.In this paper,the chemical composition,pharmacological effects,clinical applications,and adverse reactions of cinobufacini are summarized.However,the extraction of monomer components of cinobufacini,the relationship between different mechanisms,and the causes of adverse reactions need to be further studied.Also,high-quality clinical studies should be conducted.

Effects of individual heterogeneity on social contagions

Despite having significant effects on social contagions,individual heterogeneity has frequently been overlooked in earlier studies.To better understand the complexity of social contagions,a non-Markovian model incorporating heterogeneous social influence and adoption thresholds is introduced.For theoretical analysis,a generalized edge-based compartmental theory which considers the heterogeneities of social influence and adoption thresholds is developed.Focusing on the final adoption size,the critical propagation probability,and the phase transition type,social contagions for adoption thresholds that follow normal distributions with various standard deviations,follow various distributions,and correlate with degrees are investigated.When thresholds follow normal distributions,a larger standard deviation results in a larger final adoption size when the information propagation probability is relatively low.However,when the information propagation probability is relatively high,a larger standard deviation results in a smaller final adoption size.When thresholds follow various distributions,crossover phenomena in phase transition are observed when investigating the relationship of the final adoption size versus the average adoption threshold for some threshold distributions.When thresholds are correlated with degrees,similar crossover phenomena occur when investigating the relationship of the final adoption size versus the degree correlation index.Additionally,we find that increasing the heterogeneity of social influence suppresses the effects of adoption threshold heterogeneity on social contagions in three cases.Our theory predictions agree well with the simulation results.

Urban shrinkage has attracted the attention of many geographers and urban planners in recent years.However,there are fewer studies on vacant housing in shrinking cities.Therefore,this study combines multi-source remote sensing images and urban building data to assess the spatiotemporal variation patterns of housing vacancy in a typical shrinking city in China.The following points were obtained:(1)We developed an evaluation model to identify vacant residential buildings in shrinking cities by removing the contribution of nighttime lights from roads and non-residential buildings;(2)The residential building vacancy rate in fushun city significantly increased from 2013 to 2020,resulting in a significant high-value clustering effect.The impact of urban shrinkage on vacant residential buildings was higher than that on vacant non-residential buildings;(3)The World Pop population data demonstrated consistent spatial distribution and trend of population change in fushun with the residential building vacancy rate results,suggesting good reliability of the constructed evaluation model in this study.Identifying housing vacancies can help the local government to raise awareness of the housing vacancy problem in shrinking cities and to propose reasonable renewal strategies.

Carrimycin in the treatment of acute promyelocytic leukemia combined with pulmonary tuberculosis: A case report

BACKGROUND Pulmonary tuberculosis(PTB)is prevalent in immunocompromised populations,including patients with hematologic malignancies,human immunodeficiency virus infections,and chronic diseases.Effective treatment for acute promyelocytic leukemia(APL)combined with PTB is lacking.These patients show an extremely poor prognosis.Therefore,studies should establish efficient treatment options to improve patient survival and prognosis.CASE SUMMARY A 60-year-old male with pain in the right side of his chest and a fever for 4 d visited the outpatient department of our hospital.Peripheral blood smear revealed 54%blasts.Following bone marrow examinations,variant APL with TNRC18-RARA fusion gene was diagnosed.Chest computed tomography scan showed bilateral pneumonitis with bilateral pleural effusions,partial atelectasis in the lower lobes of both lungs,and the bronchoalveolar lavage fluid gene X-Pert test was positive,indicative of PTB.Carrimycin,ethambutol(EMB),and isoniazid(INH)were administered since he could not receive chemotherapy as the WBC count decreased continuously.After one week of treatment with carrimycin,the patient recovered from fever and received chemotherapy.Chemotherapy was very effective and his white blood cells counts got back to normal.After being given five months with rifampin,EMB and INH and chemotherapy,the patient showed complete remission from pneumonia and APL.CONCLUSION We report a case of PTB treated successfully with carrimycin with APL that requires chemotherapy.

A Video Captioning Method by Semantic Topic-Guided Generation

In the video captioning methods based on an encoder-decoder,limited visual features are extracted by an encoder,and a natural sentence of the video content is generated using a decoder.However,this kind ofmethod is dependent on a single video input source and few visual labels,and there is a problem with semantic alignment between video contents and generated natural sentences,which are not suitable for accurately comprehending and describing the video contents.To address this issue,this paper proposes a video captioning method by semantic topic-guided generation.First,a 3D convolutional neural network is utilized to extract the spatiotemporal features of videos during the encoding.Then,the semantic topics of video data are extracted using the visual labels retrieved from similar video data.In the decoding,a decoder is constructed by combining a novel Enhance-TopK sampling algorithm with a Generative Pre-trained Transformer-2 deep neural network,which decreases the influence of“deviation”in the semantic mapping process between videos and texts by jointly decoding a baseline and semantic topics of video contents.During this process,the designed Enhance-TopK sampling algorithm can alleviate a long-tail problem by dynamically adjusting the probability distribution of the predicted words.Finally,the experiments are conducted on two publicly used Microsoft Research Video Description andMicrosoft Research-Video to Text datasets.The experimental results demonstrate that the proposed method outperforms several state-of-art approaches.Specifically,the performance indicators Bilingual Evaluation Understudy,Metric for Evaluation of Translation with Explicit Ordering,Recall Oriented Understudy for Gisting Evaluation-longest common subsequence,and Consensus-based Image Description Evaluation of the proposed method are improved by 1.2%,0.1%,0.3%,and 2.4% on the Microsoft Research Video Description dataset,and 0.1%,1.0%,0.1%,and 2.8% on the Microsoft Research-Video to Text dataset,respectively,compared with the existing video c
