A risk score model for predicting cardiac rupture after acute myocardial infarction

Background:Cardiac rupture (CR) is a major lethal complication of acute myocardial infarction (AMI).However,no valid risk score model was found to predict CR after AMI in previous researches.This study aimed to establish a simple model to assess risk of CR after AMI,which could be easily used in a clinical environment.Methods:This was a retrospective case-control study that included 53 consecutive patients with CR after AMI during a period from January 1,2010 to December 31,2017.The controls included 524 patients who were selected randomly from 7932 AMI patients without CR at a 1:10 ratio.Risk factors for CR were identified using univariate analysis and multivariate logistic regression.Risk score model was developed based on multiple regression coefficients.Performance of risk model was evaluated using receiveroperating characteristic (ROC) curves and internal validity was explored using bootstrap analysis.Results:Among all 7985 AMI patients,53 (0.67%) had CR (free wall rupture,n=39;ventricular septal rupture,n=14).Hospital mortalities were 92.5% and 4.01% in patients with and without CR (P<0.001).Independent variables associated with CR included:older age,female gender,higher heart rate at admission,body mass index (BMI)<25 kg/m^2,lower left ventricular ejection fraction (LVEF) and no primary percutaneous coronary intervention (pPCI) treatment.In ROC analysis,our CR risk assess model demonstrated a very good discriminate power (area under the curve [AUC]= 0.895,95% confidence interval:0.845–0.944,optimism-corrected AUC= 0.821,P<0.001).Conclusion:This study developed a novel risk score model to help predict CR after AMI,which had high accuracy and was very simple to use.

Effect of Methylprednisolone on Mortality and Clinical Courses in Patients with Severe CoVID-19: A Propensity Score Matching Analysis

Background Whether methylprednisolone therapy can reduce the mortality rate of patients with severe coronavirus disease 2019(COVID-19)remains controversial,and its effects on the length of hospital stay and virus shedding time are also unknown.This retrospective study investigates the previous issues to provide more evidence for methylprednisolone treatment in severe COVID-19.Methods This retrospective study included 563 of 4827 patients with confirmed COVID-19 admitted to Wuhan Huoshenshan Hospital or Wuhan Guanggu Hospital between February 3,2020 and March 30,2020 who met the screening criteria.The participants’epidemiological and demographic data,comorbidities,laboratory test results,treatments,outcomes,and vital clinical time points were extracted from electronic medical records.The primary outcome was in-hospital death,and the secondary outcomes were 2 clinical courses:length from admission to viral clearance and discharge.Univariate and multivariate logistic or linear regression analyses were used to assess the role of methylprednisolone in different outcomes.Propensity score matching was performed to control for confounding factors.Results Of the 563 patients who met the screening criteria and were included in the subsequent analysis,138 were included in the methylprednisolone group and 425 in the nonmethylprednisolone group.The in-hospital death rate between the methylprednisolone and nonmethylprednisolone groups showed a significant difference(23.91%vs.1.65%,PFurther multivariate logistic regression analysis found comorbidities(OR,3.327;95%CI,1.702 to 6.501;P<0.001),lower lymphocyte count(OR,0.076;95%CI,0.012 to 0.461;P=0.005),higher lactate dehydrogenase(LDH)levels(OR,1.008;95%CI,1.003 to

The mechanism of hydrogen abstraction by high valence transition metal oxo compounds

We present here a systematic theoretical study to explore the underlying mechanisms of the H abstraction reaction from methane. Various abstracting agents have been modeled, using oxygen radicals and a set of high valence metal oxo compounds. Our calculations demonstrate that although H abstraction from CH3-H by metal oxoes can be satisfactorily fitted into the Polanyi correlation on the basis of oxygen radicals, the mechanisms behind are significantly different. The frontier orbital analyses show that there are three electrons and three active orbitals (3e, 3o) involved in H abstraction by oxygen radicals; whereas an additional orbital of pi(M-O)* is involved in H abstraction by M = O, resulting in a (4e, 4o) interaction. In terms of valence bond state correlation diagram, we find that H abstraction by a metal oxo may benefit from the contribution of ionic resonance structures, which could compensate the penalty of opening the M-O pbond. We believe that these findings can help to design more effective catalysts for the activation of light alkanes. Human gastric cancer is a highly lethal disease,but the underlying multiomic mole cular signatures remain largely uncle ar.Here,we performed multi-regional sampling,parallel single-cell multiomics sequencing and integrated analyses of human gastric cancer.We identified common transcrip tomic alterations of gastric cancer cells,such as aberrant down-regulation of genes associated with normal stomach Function and up-regulation of KRT7,PI3,S100A4,etc.Surprisingly,aberrant and prevalent up-regulation of genes highly expressed in normal colorectal epithelial cells were also identified in cancer cells,which maybe partially regulated by promoter chromatin accessibility and DNA methylation levels.We reve aled the single-cell DNA methylome landscape of gastric cancer,and identified candidate DNA methylation biomarkers,such as hypermethylated promoters of TMEM240 and HAGLROS,and hypomethylated promoters of TRPM2-AS and HRH1.Additionally,the relationships between genetic lineages,D NA methylation and transcriptomic clusters were systematically revealed at single-cell level.We showed that DNA methylation heterogeneities were mainly among different genetic lineages of cancer cells.Moreover,we found that D NA methylation levels of cancer cells with poorer differentiation states tend to be higher than those of cancer cells with better differentiation states in the primary tumor within the same patient,although still lower than in normal gastric epithelial cells.Cancer cells with poorer differentiation states also prevalently down-regulated MUC1 expression and immune-related pathways,and had poor infiltration of CD 8+T cells.Our study dissected the mole cular signatures of intratumoral heterogeneities and differentiation states of human gastric cancer using integrative single-cell multiomics analyses.

Truck and Trailer Routing Problem Solving by a Backtracking Search Algorithm

Truck and trailer routing problem(TTRP)is one of the most frequently encountered problem in city distribution,particularly in populated and intensive downtown.This paper addresses this problem and designs a novel backtracking search algorithm(BSA)based meta-heuristics to solve it.The initial population is created by T-sweep heuristic and then based on the framework of backtracking search algorithm,four types of route improvement strategies are used as building blocks to improve the solutions of BSA in the process of mutation and crossover.The computational experiments and results indicate that the proposed BSA algorithm can provide an effective approach to generate high-quality solutions within the satisfactory computational time.

Genetic diversity and reproductive success of a wild population of Chinese sturgeon(Acipenser sinensis)from the Yangtze River inferred from juveniles born in 2014

The Chinese sturgeon(Acipenser sinensis Gray,1835)is a large anadromous fish species,which is under considerable threat due to dramatic declines in population numbers.In the current study,population genetic diversity and individual reproductive success were assessed using nuclear microsatellite markers(simple sequence repeat,SSR)and complete mitochondrial(mt DNA)genome analysis of juveniles born in 2014.Results showed the existence of size polymorphism in the mt DNA genome of Chinese sturgeon,which was caused by a repeat motif.Population genetic diversity was high based on both SSR(Ho:0.728±0.211;He:0.779±0.122)and mt DNA genome analyses(H:0.876±0.0035;Pi:0.0011±0.0010).A positive inbreeding coefficient(FIS:0.066±0.143)was also found,indicating the occurrence of inbreeding.Reconstruction of sibling groups identified 11 mothers and 11 fathers involved in reproduction of Chinese sturgeons in 2014.Variance in individual reproductive success was not significant,with reproductive success of parent fish instead shown to be relatively even(P=0.997>0.05),thus suggesting the absence of sweepstakes reproductive success(SRS).These results indicate that,in regard to conservation,loss of genetic diversity due to the effects of SRS is not of particular concern.However,we must focus on having an adequate number of adults and suitable environmental conditions to ensure that fish can reproduce.

Effect of methylprednisolone therapy on hospital stay and viral clearance in patients with moderate COVID-19

Background:The benefits and harms of methylprednisolone treatment in patients with moderate coronavirus disease 2019(COVID-19)remain controversial.In this study,we investigated the effect of methylprednisolone on mortality rate,viral clearance,and hospitalization stay in patients with moderate COVID-19.Methods:This retrospective study included 4827 patients admitted to Wuhan Huoshenshan and Wuhan Guanggu hospitals from February to March 2020 diagnosed with COVID-19 pneumonia.The participants’epidemiological and demographic data,comorbidities,laboratory test results,treatments,outcomes,and vital clinical time points were extracted from electronic medical records.The primary outcome was in-hospital death;secondary outcomes were time from admission to viral clearance and hospital stay.Univariate and multivariate logistic or linear regression analysis were used to assess the roles of methylprednisolone in different outcomes.The propensity score matching(PSM)method was used to control for confounding factors.Results:A total of 1320 patients were included in this study,of whom 100 received methylprednisolone.Overall,in-hospital mortality was 0.91%(12/1320);the 12 patients who died were all in the methylprednisolone group,though multivariate logistic regression analysis showed methylprednisolone treatment was not a risk factor for in-hospital death in moderate patients before or after adjustment for confounders by PSM.Methylprednisolone treatment was correlated with longer length from admission to viral clearance time and hospital stay before and after adjustment for confounders.Conclusions:Methylprednisolone therapy was not associated with increased in-hospital mortality but with delayed viral clearance and extended hospital stay in moderate COVID-19 patients.

Heterostructuring an equiatomic CoNiFe medium-entropy alloy for enhanced yield strength and ductility synergy

High-/medium-entropy alloys(H/MEAs)of face-centered-cubic-structured single phase usually suffer from a glaring drawback of low yield strength.Even worse,the trade-off emerges frustratingly between strength and ductility as strength increases.Here,the lamellar heterostructure(HS)is designed in an equiatomic ternary CoNiFe ME A by means of cold rolling followed by an incomplete recrystallization annealing.The lamellar HS consists of the soft recrystallized grains as well as severely deformed structures which are partly reserved.By comparison to the coarse-grained counterpart,the lamellar HS,shows a well enhanced yield strength-ductility synergy,together with an increased yield strength.This is ascribed to the hetero-deformation-induced(HDI)stress in HS during tensile deformation.Accordingly,the HDI strain hardening is induced,serving as an important addition to the conventional forest hardening.The HDI hardening is evidenced experimentally to account for a large proportion of global strain hardening.Furthermore,a Fully recrystallized microstructure is obtained to show a simultaneous increase in both yield strength and ductility.The microstructures are evaluated in detail prior to and after tensile deformation by using the electron backscattered diffraction and transmission electron microscope observations.The mechanism for HDI strain hardening in various microstructures is analyzed to correlate to the evolution of microstructures in terms of the kernel average misorientation values,Schmid factor,and dislocation behaviors in response to plastic deformation.

Direct ink writing of TiC-316L metal matrix composites with an epoxy resin-absolute ethanol system

TiC-316 L metal matrix composites were successFully printed with an epoxy resin-absolute ethanol system by direct ink writing(DIW) process in this study.Microstructure and fracture morphology of the samples were observed by a scanning electron microscope(SEM).Also,the relative density,hardness and transverse rupture strength(TRS) of the sintered samples were tested.The results show that the samples prepared with a new epoxy resin-absolute ethanol system by the DIW process have high solid content of printing slurry,good surface roughness,high relative density and high strength.The solid content of the slurry suitable for DIW was 60 vol%,and the internal diameter of the nozzle was set to 0.4 mm,the printing speed was set to 30 mm·s^(-1),and the layer height was set to 0.36 mm.For the sample with 35 wt% TiC-316 L,the relative density,hardness and TRS of the sintered sample can reach 99.3%,HRA 79.5 and 1438 MPa,respectively.

Nonlinearity analysis of piezoelectric micromachined ultrasonic transducers based on couple stress theory

This paper studies the static deformation behavior of a piezoelectric micromachined ultrasonic transducer （PMUT） actuated by a strong external electric field. The transducer membrane consists of a piezoelectric layer, a passive layer and two electrode layers. The nonlinearities of the piezoelectric layer caused by electrostriction under a strong electric field are analyzed. Because the thickness of the transducer membrane is on the microscale, the size dependence of the deformation behavior is evaluated using the couple stress theory. The results show that the optimal ratio of the top electrode diameter and the membrane diameter is around 0.674. It is also found that this optimal value does not depend on any other parameters if the thicknesses of the two electrodes are negligible compared with those of the piezo- electric and passive layers. In addition, the nonlinearities of the piezoelectric layer will become stronger along with the increase of the electric field, which means that softening of the membrane stiffness occurs when a strong external electric field is applied. Meanwhile, the optimal thickness ratio for the passive layer and the piezoelectric layer is not equal to 1.0 which is usually adopted by previous researchers. Because there exists size dependence of membrane deforma-tion, the optimal value of this thickness ratio needs to be greater than 1.0 on the microscale.
