In Italy,a large stock of public housing was built during the 1970s and 1980s with industrialized/prefabricated *** buildings have envelopes characterized by the presence of many thermal bridges and low transmittance *** addition,they feature inefficient single heating systems in residential units and no cooling/ventilation *** a result,these buildings require urgent energy retrofitting actions,and it is therefore necessary to define procedures that will guarantee effective *** possible interventions must be compatible with building construction techniques as well as be minimally invasive and *** are only a limited number of technical solutions,considering that residents should not have to move out during the *** most Italian climatic zones,current interventions are usually linked to external insulation and window replacement,leading to an improvement in energy performance and comfort only during *** comfort conditions tend to worsen in summer months because seasonal temperatures tend to increase by a few ***,solutions should be proposed that will improve both summer and winter *** work proposes an energy recovery procedure applied to a representative building from the abovementioned period located in the Florence area and constructed with an industrialized system named the“tunnel system”(great panels structure).The procedure used in this study provides for the redevelopment of the envelope and the application of a simple mechanical ventilation system to achieve substantial energy savings and improved indoor comfort conditions.

Orthotopic liver transplantation(OLT)represents a major treatment for end-stage chronic liver disease,as well as selected cases of hepatocellular carcinoma and acute liver *** ever-increasing development of imaging modalities significantly contributed,over the last decades,to the management of recipients both in the pre-operative and post-operative period,thus impacting on graft and patients *** properly used,imaging modalities such as ultrasound,multidetector computed tomography,magnetic resonance imaging(MRI)and procedures of direct cholangiography are capable to provide rapid and reliable recognition and treatment of vascular and biliary complications occurring after *** defined is the role for imaging in assessing primary graft dysfunction(including rejection)or chronic allograft disease after OLT,e.g.,hepatitis C virus(HCV)*** paper:(1)describes specific characteristic of the above imaging modalities and the rationale for their use in clinical practice;(2)illustrates main imaging findings related to post-OLTcomplications in adult patients;and(3)reviews future perspectives emerging in the surveillance of recipients with HCV recurrence,with special emphasis on MRI.

Symptomatic hepato-diaphragmatic interposition of a bowel loop or Chilaiditi's syndrome is a peculiar anatomical condition most often found by chance. Its described symptoms range from intermittent, mild abdominal pain and dyspepsia to acute intestinal obstruction. We report a case of hepato-diaphragmatic migration of the hepatic flexure of the colon associated to an unusual, heretofore unreported, angina-like pain exclusively evoked by the left lateral decubitus. To maximize the chance of observing anatomical changes in different postures, computed tomography of the chest and abdomen was performed after air insufflation into the colon. While frank herniation into the chest was excluded, the scan showed that the hepatic flexure-with the interposition of the diaphragm-came in contact with the right side of the heart in the left lateral, but not in the supine, decubitus. This finding was reproduced by echocardiography which also showed virtually unaltered hemodynamics after the change of posture. ECG, left and right ventricular global and regional function as well as cardiac injury markers also remained unchanged during the maneuver, indicating that the pain evoked by the latter was unlikely due to myocardial ischemia. This case suggests that Chilaiditi's syndrome should be included among the possible, although rare,causes of unexplained angina-like symptoms.

Acinar cell carcinoma(ACC)is a rare malignant epithelial neoplasm accounting for 1%-2%of all pancreatic exocrine neoplasm,affecting more frequently man with an age between 50 and 70 *** patients present with nonspecific symptoms,which may give rise to difficulties in clinical diagnosis[1].ACC can manifest with diarrhea,weight loss,abdominal pain and,in up to 10%-15%,with lipase hypersecretion syndrome,characterized by elevated lipase production,diffuse subcutaneous fat necrosis and polyarthralgia[1,2].Biliary obstruction and jaundice are infre-quent clinical manifestations,unlike ductal adenocarcinoma,due to pushing rather than infiltrating growth of ACC.

Despite advances in patient and graft management,biliary complications(BC)still represent a challenge both in the early and delayed period after orthotopic liver transplantation(OLT).Because of unspecific clinical presentation,imaging is often mandatory in order to diagnose *** imaging modalities,magnetic resonance cholangiography(MRC)has gained widespread acceptance as a tool to represent the reconstructed biliary tree noninvasively,using both the conventional technique(based on heavily T2-weighted sequences)and contrast-enhanced MRC(based on the acquisition of T1-weighted sequences after the administration of hepatobiliary contrast agents).On this basis,MRC is generally indicated to:(1)avoid unnecessary procedures of direct cholangiography in patients with a negative examination and/or identify alternative complications;and(2)provide a road map for interventional procedures or *** illustrated in the review,MRC is accurate in the diagnosis of different types of biliarycomplications,including anastomotic strictures,nonanastomotic strictures,leakage and stones.

目的偶然发现椎体骨折是评价骨骼健康情况的关键因素。该研究目的是回顾性评价乳腺MRI的定位像作为显示骨质疏松性椎体骨折的潜在价值。方法 3位肌骨放射学专家采用半定量方法回顾性分析了856例乳腺MRI的矢状定位像以发现椎体骨折。同时研究病人的病史和基本信息。回顾正式的乳腺MRI与既往的影像学检查,以确定病人病史中是否有椎体骨折或其他相关骨病的情况。结果 856例病人[21～89岁,平均(54.7±12.2)岁]中,780例(91.1%)由于肿瘤原因而进行MRI检查,76例(8.9%)由于非肿瘤因素而参与本研究;57例的MRI定位像不能达到诊断要求而被排除。剩余799例中,71例(8.9%)MRI定位像中发现椎体骨折。其中63例(88.7%)的椎体骨折在影像学报告和既往病史中均未提到(P<0.001)。任何乳腺报告中都没有提到椎体骨折。4例病人的MRI定位像中发现了椎体转移瘤。结论系统评价MRI定位像能够获得额外的临床信息,以防止漏诊椎体骨折。在其他影像学检查时,MRI定位像可用于椎体骨折的筛查。

AIM To investigate the role of minimally invasive surgery for gastric cancer and determine surgical, clinical, and oncological *** This is a propensity score-matched case-control study, comparing three treatment arms: robotic gastrectomy(RG), laparoscopic gastrectomy(LG), open gastrectomy(OG). Data collection started after sharing a specific study protocol. Data were recorded through a tailored and protected web-based system. Primary outcomes: harvested lymph nodes, estimated blood loss, hospital stay, complications rate. Among the secondary outcomes, there are: operative time, R0 resections, POD of mobilization, POD of starting liquid diet and soft solid diet. The analysis includes the evaluation of type and grade of postoperative complications. Detailed information of anastomotic leakages is also *** The present analysis was carried out of 1026 gastrectomies. To guarantee homogenous distribution of cases, patients in the RG, LG and OG groups were 1:1:2 matched using a propensity score analysis with a caliper = 0.2. The successful matching resulted in a total sample of 604 patients(RG = 151; LG = 151; OG = 302). The three groups showed no differences in all baseline patients characteristics, type of surgery(P = 0.42) and stage of the disease(P = 0.16). Intraoperative blood loss was significantly lower in the LG(95.93 ± 119.22) and RG(117.91 ± 68.11) groups compared to the OG(127.26 ± 79.50, P = 0.002). The mean number of retrieved lymph nodes was similar between the RG(27.78 ± 11.45), LG(24.58 ± 13.56) and OG(25.82 ± 12.07) approach. A benefit in favor of the minimally invasive approaches was found in the length of hospital stay(P < 0.0001). A similar complications rate was found(P = 0.13).
