Temporary mechanical circulatory support(tMCS)plays a pivotal role in managing severe or refractory cardiogenic shock(CS),furnishing essential hemodynamic support,and sustaining organ perfusion from hours to weeks,or to even *** the past decade,the utilization of tMCS has witnessed a signicant increase in American and European *** in Asia,the newly developed concepts and devices constituting tMCS have also made rapid ***,the absence of a consensus or standardized guidelines,especially for the Asian population considering inherited polymorphism,is *** address this knowledge gap,eight associations/societies and experts in thiseld came up with the present consensus after repeated discussions,more specically for the Asian *** comprehensively elucidates the commonly employed tMCS devices,criteria for patient selection,indications and contraindications,clinical management,and the optimal timing for implantation and *** goal of this statement is to promote the standardized application of tMCS and make reasonable decisions in clinical ***-length version of the consensus can be seen in Supplementary File,http://***/CM9/B883.

新型冠状病毒感染后间质性肺炎患者肺移植受体评估和术后管理中国专家共识

自2019年12月新型冠状病毒肺炎（coronavirus disease 2019,COVID-19）大流行开始，中国感染的人群已超过85%，新型冠状病毒（severe acute respiratory syndrome coronavirus 2,SARS-CoV-2）最主要累及呼吸系统，尤其是肺，重症感染患者死亡率高，6%～10%患者最终会发展为严重COVID-19相关的急性呼吸窘迫综合征（COVID-related acute respiratory distress syndrome,CARDS），需要机械通气和体外膜肺氧合支持，部分急性肺损伤幸存患者后续会出现持续的COVID-19后肺纤维化（post COVID-19 pulmonary fibrosis,PCPF）。充分治疗后的CARDS及严重的PCPF都是适宜的肺移植候选者，由于感染病程较为特殊，故需要采用不同于普通终末期肺病患者的筛查评估策略。COVID-19患者肺移植术后需要特殊的监测及治疗，包括规范进行SARS-CoV-2核酸检测、合理使用免疫抑制药物及规范使用抗病毒药物，这些均使肺移植术后管理面临较大的考验。本共识基于国内外大量关于SARS-CoV-2感染、治疗及肺移植手术的循证医学证据，并经过专家多次研讨达成一致意见，为SARSCoV-2感染后间质性肺炎患者的肺移植评估及术后管理提供参考。

Changes of phospholipase D activity of rat peritoneal mast cells in degranulation

IM: To study the changes of phospholipase D (PLD) activity of actively sensitized rat peritoneal mast cells (RPMC) in degranulation. METHODS: Degranulation of RPMC was determined by measurement of β-hexosaminidase release. PLD activity assay was carried out by measurement of PLD product, choline, with chemiluminescent oxidation of luminol. RESULTS: Actively sensitized RPMC challenged with ovalbumin (0.5-8 mg/L for 120 s, 4 mg/L for 15-120 s) resulted in significant activation of PLD accompanied with the increment of P-hexosaminidase release. PLD activity of sensitized RPMC was increased by more than 2-fold compared with that of unsensitized RPMC which contained low levels of PLD activity [(35±13) pmol choline/min in 1×106cells], but β-hexosaminidase releases of the sensitized cells were as low as spontaneous releases.

Since December 2019, the emergence of a new type of coronavirus (SARA-CoV-2) pneumonia (COVID-19) in Wuhan, Hubei Province, has become a national public health emergency. The disease diagnosis is currently confirmed to be based on nucleic acid test and high-resolution CT (High Resolution CT, HRCT). Lung ultrasound (LUS) has been used in clinical practice in the differential diagnosis of lung exudative inflammation lesions, and in the evaluation of thoracic effusion, pneumothorax and the evaluation of patients with dyspnea and acute respiratory failure, as well as in the treatment monitoring to guide the treatment more efficiently. The clinical LUS application has been formed a number of domestic and foreign expert consensus. As a fast, convenient, radiation-free and easy-to-implement lung disease visualization technology at the bedside, LUS plays a role in the treatment of critically ill patients of COVID-19 . In view of the problems of LUS performance in different lung diseases, LUS has not been widely used in front-line clinic and ICU and needs to be fully mastered by clinicians in routine practice. Starting from the increasing number of severe patients with COVID-19 pneumonia, the urgent need for immediate dynamic and effective image monitoring of lung lesions in severe patients bedside and the actual clinical needs of the accurate disease changes assessment and the evaluation of treatment timely, in line with the principle of ""problem-oriented, information technology support, model innovation"", the integration of various resources, and put forward a targeted short-term can achieve a wide range of efficient applications of LUS technology of the COVID-19 pneumonia, an enhenced remote diagnosis consultation platform should be established. To promote the clinical application of LUS in the routine diagnosis and treatment of COVID-19 pneumonia in various regions, it is recommended to use the 5G remote consultation platform to achieve cross-institutional, cross-regio

新型冠状病毒(新冠病毒)感染不仅给安全有效地开展临床诊治带来困难, 也使医疗工作者面临的感染风险增加。部分新冠病毒感染患者的肺部影像学表现可能与普通肺结节相似, 这给肺结节的诊断及治疗带来挑战。目前, 我国进入新冠病毒感染"乙类乙管"常态化防控阶段, 针对新冠病毒感染后肺结节患者的诊治、随访及围手术期管理尚缺乏相应的共识, 相关诊疗工作的开展主要依靠各中心的经验。本共识旨在针对新冠流行背景下的肺结节患者的诊疗, 总结国内外相关经验, 为广大胸外科医务工作者的科学、规范诊治提供依据。
