Obesity has a negative effect on male reproductive function. It is associated with low testosterone levels and alteration in gonadotropin secretion. Male obesity has been linked to reduced male fertility. Data regarding the relation of obesity to sperm parameters are conflicting in terms of the nature and magnitude of the effect. New areas of interest are emerging that can help explain the variation in study results, such as genetic polymorphism and sleep apnea. Sleep disorders have been linked to altered testosterone production and hypogonadism in men. It was also correlated to erectile dysfunction. the relation of sleep disorders to male fertility and sperm parameters remains to be investigated. Men with hypogonadism and infertility should be screened for sleep apnea. treatment of obesity and sleep apnea improves testosterone levels and erectile function.

this paper presents a theoretical and experimental study on controller design for the AMBs in a small-scale flywheel energy storage system,where the main goals are to achieve low energy consumption and improved rotordynamic stability.A H-infinity optimal control synthesis procedure is defined for the permanent-magnet-biased AMB-rotor system with 4 degrees of *** the choice of design weighting functions,notch filter characteristics are incorporated within the controller to reduce AMB current components caused by rotor vibration at the synchronous frequency and higher *** tests are used to validate the controller design methodology and provide comparative results on performance and *** results show that the H-infinity controller is able to achieve stable rotor levitation and reduce AMB power consumption by more than 40%(from 4.80 to 2.64 Watts)compared with the conventional PD control ***,the H-infinity controller can prevent vibrational instability of the rotor nutation mode,which is prone to occur when operating with high rotational speeds.

Simultaneous vs staged treatment of urolithiasis in patients undergoing radical prostatectomy

AIM: to assess the outcomes of men treated for urolithiasis at the time of radical ***: From 1991 to 2010, 22 patients were retrospectively identified who were treated simultaneously(n = 10) at radical prostatectomy, or(n = 12) within 120 d prior to prostatectomy, for urolithiasis. Clinical characteristics were reviewed including: type of prostatectomy and stone surgery, location and amount of stone burden, perioperative change in hemoglobin and creatinine, stent frequency, total hospital d, stone-free rates, additional stone procedures and complications. Long-term functional outcomes including stress urinary incontinence and bladder neck contracture were reported. Differences between cohorts(simultaneous vs staged treatment) were assessed. RESULtS: Among men undergoing radical prostatectomy, primary stone procedures included 12 ureteroscopy, 6 shock wave lithotripsy, 2 open nephrolithotomyand 2 percutaneous nephrolithotomy. In staged shock wave lithotripsy there were 4 complications and 3 additional procedures vs 1(P = 0.5) and 0(P = 0.2) in the simultaneous cohort. Meanwhile in staged ureteroscopy there were 5 complications and 1 additional procedure vs 1(P = 0.2) and 1(P = 0.9) in the simultaneous cohort. Additional procedures for residual stones was greater among patients with asymptomatic upper tract calculi 3(60%) relative to patients with symptomatic stones 2(13%; P = 0.02). Likewise, patients with proximal or multiple calculi had a greater total hospital days 5.5 vs 4.1(P = 0.04), additional procedures 6 vs 0(P = 0.04) and lower stone-free rates 39% vs 89%(P = 0.02) relative to men with distal stones. Finally, there was no difference in the incidence of bladder neck contracture(P = 0.4) or stress urinary incontinence(P = 0.7) between ***: Ureteroscopic treatment of symptomatic distal urolithiasis at radical prostatectomy appears to be safe and efficacious with a low rate of adverse postoperative outcomes.

this paper investigates the free vibration and transient response of interconnected structures including double curved beams and intermediate straight beams,which are all constructed from functionally graded porous(FGP) materials. the strain potential and kinetic energies of each beam along with the work done by the external force are calculated. Additionally, a higher-order beam element is introduced to derive stiffness and mass matrices, along with the force vector. the curved and straight beams are discretized, and their assembled stiffness, mass matrices, and force vectors, are *** conditions at the joints are used to derive the total matrices of the entire structure. Subsequently, the natural frequencies and transient response of the system are determined. the accuracy of the mathematical model and the self-developed computer program is validated through the comparison of the obtained results with those of the existing literature and commercial software ANSYS, demonstrating excellent ***, a comprehensive study is conducted to investigate the effects of various parameters on the free vibration and transient response of the considered structure.

to quantify drug-drug-interactions (DDIs) encountered in patients prescribed hepatitis C virus (HCV) treatment, the interventions made, and the time spent in this *** standard of care, a clinical pharmacist screened for DDIs in patients prescribed direct acting antiviral (DAA) HCV treatment between November 2013 and July 2015 at the University of Colorado Hepatology Clinic. HCV regimens prescribed included ledipasvir/sofosbuvir (LDV/SOF), paritaprevir/ritonavir/ombitasvir/dasabuvir (OBV/PtV/r + DSV), simeprevir/sofosbuvir (SIM/SOF), and sofosbuvir/ribavirin (SOF/RBV). this retrospective analysis reviewed the work completed by the clinical pharmacist in order to measure the aims identified for the study. the number and type of DDIs identified were summarized with descriptive *** hundred and sixty four patients (83.4% Caucasian, 57% male, average 56.7 years old) were identified; 369 for LDV/SOF, 48 for OBV/PtV/r + DSV, 114 for SIM/SOF, and 133 for SOF/RBV. Fifty-one point five per cent of patients were cirrhotic. Overall, 5217 medications were reviewed (7.86 medications per patient) and 781 interactions identified (1.18 interactions per patient). the number of interactions were fewest for SOF/RBV (0.17 interactions per patient) and highest for OBV/PtV/r + DSV (2.48 interactions per patient). LDV/SOF and SIM/SOF had similar number of interactions (1.28 and 1.48 interactions per patient, respectively). Gastric acid modifiers and vitamin/herbal supplements commonly caused interactions with LDV/SOF. Hypertensive agents, analgesics, and psychiatric medications frequently caused interactions with OBV/PtV/r + DSV and SIM/SOF. to manage these interactions, the pharmacists most often recommended discontinuing the medication (28.9%), increasing monitoring for toxicities (24.1%), or separating administration times (18.2%). the pharmacist chart review for each patient usually took approximately 30 min, with additional time for more complex patients

Lymphangiomas are a group of benign malformations of the lymphatic system, and hemolymphangioma(or hemangiolymphangioma) is a rare congenital malformation of the lymphatic system with vascular involvement. these benign malformations are most commonly diagnosed at an early age, and may be present as a part of an associated syndrome. In this case report, we describe the first case of adolescent scoliosis associated with a large, paraspinous hemolymphangioma. A 15-year-old girl with an incidental finding of a paraspinous hemolymphangioma is presented along with her history, physical exam, radiographic findings, and operative course. the possible pathogenesis, treatment approach, and clinical dilemmas are also discussed. Given the well-known relationship between tumors and scoliosis, a benign paraspinous vascular and lymphatic tumor may be responsible for the presence of scoliosis in a small number of patients.

Appreciation of soft-tissue thickness(Stt)at surgical sites is an increasingly recognized aspect of arthroplasty procedures as it may potentially impacting postoperative *** research has focused on the predictive value of preoperative Stt measurements for complications following various forms of arthroplasty,particularly infections,across procedures such as total knee,hip,shoulder,and ankle *** studies have indicated that increased Stt is associated with a higher risk of complications,including infection and wound healing *** assessment of Stt before surgery could play a crucial role in identifying patients at a higher risk of complications and may be instru-mental in guiding preoperative planning to optimize outcomes in arthroplasty *** measurement techniques and further research are essential to enhance the reliability and clinical utility of Stt assessment for arthro-plasty surgery.

Ultrasonic interferometry was utilized in conjunction with synchrotron-based X-ray diffraction and X-radiographic imaging to determine the compressional and shear wave velocities and unlt-cdl volumes of pyrite （FeS2） at room temperature and pressures up to 9.6 GPa. Fitting all of the experimental volume and velocity data to third-order finite-strain equations yielded the adiabatic zero-pressure bulk and shear moduli and their first pressure derivatives： Ks0=138.9（7） GPa, Go=U2.3（3） GPa, （δKS0/δP）t=KS0＇=6.0（1）, （δG0/δP）t=G0＇=3.0（〈1）, where the numbers in parentheses represent the 1δ uncertainty in the last significant digit. these results are in good agreement with several previous static compression studies on this material but differ quite strongly from the results obtained via first principles calculations. this study presents the first direct measurement of the bulk shear properties of this material.
