BACKGROUND The Karnofsky Performance Status(KPS)scale has been widely validated for clinical practice for over 60 *** To examine the extent to which poor pre-transplant functional status,assessed using the KPS scale,is associated with increased risk of mortality and/or graft failure at 1-year *** This study included 38278 United States adults who underwent first,non-urgent,liver-only transplantation from 2005 to 2014(Scientific Registry of Transplant Recipients).Functional impairment/disability was categorized as severe,moderate,or none/*** were conducted using multivariableadjusted Cox survival regression *** The median age was 56 years,31%were women,median pre-transplant Model for End-Stage for Liver Disease score was *** impairment was present in 70%;one-quarter of the sample was severely *** controlling for key recipient and donor factors,moderately and severely disabled patients had a 1-year mortality rate of 1.32[confidence interval(CI):1.21-1.44]and 1.73(95%CI:1.56-1.91)compared to patients with no impairment,*** with moderate and severe disability also had a multivariable-adjusted 1-year graft failure rate of 1.13(CI:1.02-1.24)and 1.16(CI:1.02-1.31),*** Pre-transplant functional status is a useful prognostic indicator for 1-year posttransplant patient and graft survival.

背景：有关药物和介入治疗对ST段抬高型和非ST段抬高型急性冠脉综合征（non—ST-segment elevation acute coronary syndrome，NSTE ACS）患者的作用，随机试验已经提供了足够的证据，但是是否已经转化为相应的临床变化还不清楚。目的：确定ST段抬高型心肌梗死（ST-segment elevation myocardial infarction，STEMI）和NSTE ACS患者的院内治疗改变是否伴随临床结果的改善。设计、地点及患者：全球急性冠脉事件注册研究（Global Registry of Acute Coronary Events，GRACE）是一项多国家队列研究。自1999年7月1日至2006年12月31日，该研究共入选并随访了来自14个国家、113家医院的44372例ACS患者。主要观测指标：当前循证药物和介入治疗应用的趋势及患者预后（死亡、充血性心力衰竭、肺水肿、心源性休克、卒中和心肌梗死）。结果：随着研究时间的进展，相关药物的应用增加（β-受体阻滞剂、他汀类药物、血管紧张素转换酶抑制剂、行或未行经皮冠状动脉介入治疗[percutaneous coronary intervention，PCI]患者应用噻氯毗啶、糖蛋白Ⅱb／Ⅲa抑制剂和低分子肝素；P〈0．001）。STEMI患者药物再灌注治疗下降了22个百分点（95％可信区间[confidence interval，CI]，-27～-17），而直接PCI增加了37个百分点（95％CI，33—41）。在NSTEMI患者中，PCI的比例显著升高了18个百分点（95％CI，15～20）。充血性心力衰竭和肺水肿在STEMI患者中降低了9个百分点（95％CI，-12～-6），在NSTEACS患者中降低了6．9个百分点（95％CI，-8．4～-4．7）。STEMI患者的院内死亡率降低了18个百分点（95％CI，-5．3～-1．9），心源性休克降低了24个百分点（95％CI，-4．3～-0．5）。校正了危险评分后，NSTEACS患者的院内死亡率降低了0．7个百分点（95％CI，-1．7～0．3）；STEMI患者6个月随访时卒中发生率降低了0．8个百分点（95％CI，-1．7～0．1），心肌梗死发生率降低了2．8个百分点（95％CI，-6．4～0．9）。NSTEACS患者6个月死亡率降低了1．6个百分点（95％CI，-3．0～-0．1），卒中发生率降低了0．7个百分点（95％CI，-1．4～0．1）。结论：在本项多国家观察性研究中，ACS患者治疗的改善与6个月新发心力衰竭、死亡率、卒中和心肌梗死发生率显著降低相关。

Over-ground walking in Parkinson’s disease: A pilot study utilizing a portable metabolic analyzer

Alterations in gait biomechanics are common during early stages of Parkinson’s disease (PD), potentially elevating energy requirements of walking and leading to impaired economy of gait. Although gait economy is traditionally assessed during treadmill walking with simultaneous ox-ygen consumption (VO2) monitoring, treadmill gait mechanics, particularly in PD, may be different from over-ground walking mechanics, possibly providing a distorted picture of true gait economy. Currently, no studies have directly examined the energy cost of over-ground walking in PD patients. The purpose of this study was to test the feasibility of measuring energy expenditure during over-ground walking in mild to moderate PD using portable gas exchange monitoring technology. Additionally, we sought to determine whether energy expenditure, as assessed through VO2 measures, related to disease severity for PD. Seventeen PD patients underwent separate 6-minute walk (6MW) tests both with and without the COSMED K4b2 portable oxygen monitoring system. Gait economy was calculated as measured VO2 during 6MW divided by the predicted VO2 for non-PD age-matched subjects, according to a standard estimation equation utilizing ground speed. Distance covered during the 6MW with the portable system (420 ± 12 meters) was highly correlated (r = 0.96, p 2 peak for normal floor walking, and show impaired gait economy relative to prediction equations. Interestingly, the degree of elevated energy expenditure during gait did not relate to disease severity.

2011年MW9.0的日本东北地区近海大地震在日本海沟附近产生了最大超过50m的同震滑动,这可能导致该区域进入了应力近乎完全卸载的状态。为检验上述假设,我们利用综合大洋钻探计划在震后1年左右时间得到的钻孔资料,以及对震前应力状态的推断结果,确定了陆楔前缘的原地应力状态。根据孔壁崩落法对水平应力方向与应力大小的估算结果,以及在破裂向海沟轴方向传播过程中同震位移增大的现象,可推断原地水平应力在地震过程中变小了。这种应力变化显示出板块界面的前缘发生了相对自由的滑动,该结论与地震引起了断层弱化和发生了近乎全部的应力降等观测结果是吻合的。
