AIM: To investigate anti-hypersensitive effects of α2δ-1 ligands in non-inflammatory and inflammationassociated colonic hypersensitivity(cHS) mouse ***: To induce an inflammation-associated cHS, 1% dextran sulfate sodium(DSS) was administered to c57Bl/6J male mice, in drinking water, for 14 d. Regarding the non-inflammatory neonatal maternal separation(NMS)-induced cHS model, wild-type c57BI/6J pups were isolated from their mother from day 2 to day 14(P2 to P14), three hours per day(from 9:00 a.m. to 12:00 p.m.). colorectal distension was performed by inflating distension probe from 20 μL to 100 μL by 20 μL increment step every 10 s. After a first colorectal distension(cRD), drugs were administered subcutaneously, in a cumulative manner,(Gabapentin at 30 mg/kg and 100 mg/kg; Pregabalin at 10 mg/kg and 30 mg/kg; carbamazepine at 10 mg/kg and 30 mg/kg) and a second cRD was performed one hour after each ***: The visceromotor response(VMR) to cRD was increased by our NMS paradigm protocol in comparison to non-handled(NH) mice, considering the highest distension volumes(80 μL: 0.783 ± 0.056 mV /s vs 0.531 ± 0.034 m V/s, P ce, respectively). In the inflammationassociated cHS, DSS-treated mice showed a dramatic and significant increase in VMR at 60 and 80 μL distension volumes when compared to control mice(60 μL: 0.920 ± 0.079 m V/s vs 0.426 ± 0.100 m V/s P ce, respectively). carbamazepine failed to significantly reduce cHS in both models. Gabapentin significantly reduced cHS in the DSS-induced model for both subcutaneous injections at 30 or 100 mg/kg. Aims:Palbociclib has been approved in combination with endocrine therapy(ET)for hormone receptor-positive(HR+)/human epidermal growth factor receptor 2-negative(HER2-)advanced breast cancer(ABc),regardless of *** though ABc is one of the most prevalent cancers in older patients,very few patients≥65 years old were included in pivotal ***,the current study evaluated the safety and efficacy of palbociclib in“real-world”routine treatment of unselected older patients with HR+/***:Data were collected on patients>70 years old who were treated with palbociclib plus ET for HR+/HER2-ABc in our *** analyzed safety data(cTcAE v4.0 criteria)and outcomes,such as progression-free survival(PFS)and overall survival(OS),as well as any associations between main geriatric characteristics and our ***,we assessed safety at a national level by analyzing all palbociclib-related adverse events(AEs)reported in the French Pharmacovigilance Database(FPVD)during the same ***:From February 2016 to July 2019,52 patients were identified with a median age of 80.9 years,of whom 88%presented an *** most common grade 3-4 AE was neutropenia(64%).Median PFS and OS were nine months and not reached,*** FPVD reports 227 cases of palbociclib-related AEs,with older and younger patients sharing similar ***:Palbociclib is well tolerated in older patients with efficacy comparable to that in younger ***,the addition of palbociclib to ET should be evaluated individually in this older and frailer subgroup.

Natural history of uncomplicated sigmoid diverticulitis

While diverticular disease is extremely common, the natural history(NH) of its most frequent presentation(i.e., sigmoid diverticulitis) is poorly investigated. Relevant information is mostly restricted to populationbased or retrospective studies. This comprehensive review aimed to evaluate the NH of simple sigmoid diverticulitis. While there is a clear lack of uniformity in terminology, which results in difficulties interpreting and comparing findings between studies, this review demonstrates the benign nature of simple sigmoid diverticulitis. The overall recurrence rate is relatively low, ranging from 13% to 47%, depending on the definition used by the authors. Among different risk factors for recurrence, patients with c-reactive protein > 240 mg/L are three times more likely to recur. Other risk factors include: Young age, a history of several episodes of acute diverticulitis, medical vs surgical management, male patients, radiological signs of complicated first episode, higher comorbidity index, family history of diverticulitis, and length of involved colon > 5 cm. The risk of developing a complicated second episode(and its corollary to require an emergency operation) is less than 2%-5%. In fact, the old rationale for elective surgery as a preventive treatment, based mainly on concerns that recurrence would result in a progressively increased risk of sepsis or the need for a colostomy, is not upheld by the current evidence.

codiaeum variegatum has been widely investigated for its biological proprieties ranging from the antiamoebic potential to the phytochemical analysis. The aim of the present study was to evaluate the anti-inflammatory potential of c. variegatum leaf extracts and fractions. A primary macrophage culture activated by Saccharomyces cereviseae (Sc) was used to evaluate cell cytotoxicity and anti-inflammatory potential of the plant extracts and fractions. Macrophages were treated with different concentrations (0.1;1;10 and 100 μg/mL) of the extracts/fractions for the inhibition of 5-lipoxygenase activity, nitric oxide (NO) and Tumor Necrosis Factor Alpha (TNF-α) production. No significant difference was observed on cell viability in the presence of extracts and fractions at tested concentration during the incubation period. Extracts and fractions of c. variegatum inhibited the 5-lipoxygenase activity, NO and TNF-α production by viable primary mouse macrophages in a concentration-dependent manner. The fractionation process increased anti-inflammatory activity. Among fractions, HEF2, HEF3, HEF5, EEF1, EEF3 and EEF5 exhibited the best anti-inflammatory potential. c. variegatum extracts and fractions exhibited a greater anti-inflammatory potential throughout the inhibition of pro-inflammatory mediators such as NO, 5-Lox and TNF-α.

目的:探讨乌鳖口服液对自身免疫性卵巢早衰小鼠的影响。方法:通过皮下多点注射小鼠透明带3建立自身免疫性卵巢早衰小鼠模型。然后连续4周给予乌鳖口服液进行治疗,每天剂量为(16.5 g/kg)。通过酶联免疫法检测小鼠抗透明带抗体,用免疫组化法测定脾脏T细胞cD4+、cD8+的含量。同时用免疫组化法测定卵巢Fas、FasL蛋白的表达。卵巢和脾脏做病理组织学的分析。结果:乌鳖口服液急性毒性实验显示乌鳖口服液经口给药给予小鼠未能测出LD50,小鼠经口最大给药量实验结果表明:20只小鼠给药后无明显毒性反应,2周内无死亡。因此,乌鳖口服液是安全的药物。模型组小鼠抗透明带抗体的浓度(8.74±0.99U/ml)和cD4+/cD8+比值(1.74±0.31)增加,同时,模型组小鼠卵巢组织的Fas蛋白表达(0.54±0.29)减少,Fas-L蛋白表达(2.25±0.0)增加。与模型组比较,乌鳖口服液组小鼠抗透明带抗体的浓度(7.34±1.21U/ml)明显低于模型组(pcD4+/cD8+比值(1.07±0.14)明显低于模型组(pcD4+/cD8+比值和Fas/FasL系统的作用。

背景手术前应用糖皮质激素可以减少手术后恶心，同时也能提高镇痛效果并减少阿片类药物的用量。方法选择50例连续择期脊麻辅助丙泊酚镇静下行初次单侧全髋关节置换术（THA）的患者，手术开始前以随机、双盲、安慰剂对照的方式静脉输注40mg地塞米松或生理盐水。手术后48小时持续使用吗啡静脉自控镇痛，并每6小时口服布洛芬400mg和对乙酰氨基酚650mg。每4小时记录1次静息疼痛评分（0～10数字评分，NRS）、不良反应和患者自控镇痛吗啡的累积使用量，共48小时。在手术后第24小时记录活动时的NRS评分。在手术后第48小时测量25例患者亚组的c-反应蛋白水平。结果地塞米松组手术中对丙泊酚镇静的需求量明显比对照组增加（234．6±160．1VS138．8±122．7mg，P=0．02）。与对照组相比，地塞米松组手术后的运动痛明显减轻（NRS评分：2．7，95％cI：2．2～3．1和6．8，95％cI：6．4～7．2；P〈0．0001）。地塞米松对任何时点的静息疼痛或者累积的吗啡使用量没有显著影响。地塞米松可以显著减少手术后48小时时c一反应蛋白的水平（52．4mg／L，95％cI：28．2。76．6vs 194．2mg／ml，95％cI：168．9—219．4；P〈0．0001）。对照组有7例患者因恶心需要处理，而地塞米松组仅有1例患者需要处理（P=0．05）。结论全髋关节置换术术前静脉给予单次剂量的地塞米松40mg能持续抑制炎性反应，减轻术后24小时的运动痛。
