Engineered polycaprolactone–magnesium hybrid biodegradable porous scaffold for bone tissue engineering

In this paper, we describe the fabrication of a new biodegradable porous scaffold composed of polycaprolactone(PCl) and magnesium(Mg)micro-particles. The compressive modulus of PCl porous scaffold was increased to at least 150% by incorporating 29% Mg particles with the porosity of 74% using Micro-CT analysis. Surprisingly, the compressive modulus of this scaffold was further increased to at least 236% when the silane-coupled Mg particles were added. In terms of cell viability, the scaffold modified with Mg particles significantly convinced the attachment and growth of osteoblasts as compared with the pure PCl scaffold. In addition, the hybrid scaffold was able to attract the formation of apatite layer over its surface after 7 days of immersion in normal culture medium, whereas it was not observed on the pure PCl scaffold. This in vitro result indicated the enhanced bioactivity of the modified scaffold. Moreover, enhanced bone forming ability was also observed in the rat model after 3 months of implantation. Though bony in-growth was found in all the implanted scaffolds. High volume of new bone formation could be found in the Mg/PCl hybrid scaffolds when compared to the pure PCl scaffold. Both pure PCl and Mg/PCl hybrid scaffolds were degraded after 3 months. However, no tissue inflammation was observed. In conclusion, these promising results suggested that the incorporation of Mg micro-particles into PCl porous scaffold could significantly enhance its mechanical and biological properties. This modified porous bio-scaffold may potentially apply in the surgical management of large bone defect fixation.

目的　介绍一种采用锁定加压钢板螺钉 (l CP)固定人工关节周围骨折的方法。方法　为一例髋关节假体远端周围螺旋骨折 ,采用 12孔长锁定加压钢板于外侧固定。近折端 6孔 ,其中 5孔用锁定螺钉作单侧骨皮质固定 ,最末孔用 4 .5 m m螺钉固定 ;远折端 4孔 ,作双侧骨皮质固定 ,中间骨折区域 2孔空出 ,以免干扰骨折愈合。结果　术后 3天开始行髋膝关节功能锻炼 ,1周后下地站立锻炼 ,2周患肢部分负重训练。术后 X线片示骨折复位固定良好 ,假体柄周围无松动迹象。术后 3个月复查 X线片示骨折愈合无畸形 ,假体无松动 ;6个月随访 ,患肢无疼痛、肌萎缩 ,功能恢复正常。结论　 l CP用于处理关节假体周围骨折时方法简便 ,剥离范围小 ,对局部组织创伤小 ,有利于骨折的愈合。

Abnormal subchondral bone remodeling and its association with articular cartilage degradation in knees of type 2 diabetes patients

Type 2 diabetes （T2D） is associated with systemic abnormal bone remodeling and bone loss. Meanwhile, abnormal subchondral bone remodeling induces cartilage degradation, resulting in osteoarthritis （OA）. Accordingly, we investigated alterations in subchondral bone remodeling, microstructure and strength in knees from T2D patients and their association with cartilage degradation. Tibial plateaus were collected from knee OA patients undergoing total knee arthroplasty and divided into non-diabetic （n---70） and diabetes （n = 51） groups. Tibial plateaus were also collected from cadaver donors （n = 20） and used as controls. Subchondral bone microstructure was assessed using micro-computed tomography. Bone strength was evaluated by micro-finite-element analysis. Cartilage degradation was estimated using histology. The expression of tartrate-resistant acidic phosphatase （TRAP）, osterix, and osteocalcin were calculated using immunohistochemistry. Osteoarthritis Research Society International （OARSI） scores of lateral tibial plateau did not differ between non-diabetic and diabetes groups, while higher OARSI scores on medial side were detected in diabetes group. lower bone volume fraction and trabecular number and higher structure model index were found on both sides in diabetes group. These microstructural alterations translated into lower elastic modulus in diabetes group. Moreover, diabetes group had a larger number of TRAP~ osteoclasts and lower number of Osterix~ osteoprogenitors and Osteocalcin~ osteoblasts. T2D knees are characterized by abnormal subchondral bone remodeling and microstructural and mechanical impairments, which were associated with exacerbated cartilage degradation. In regions with intact cartilage the underlying bone still had abnormal remodeling in diabetes group, suggesting that abnormal bone remodeling may contribute to the early pathogenesis of T2D-associated knee OA.

Identification of optimal contemporary antiemetic prophylaxis for doxorubicin-cyclophosphamide chemotherapy in Chinese cancer patients: post-hoc analysis of 3 prospective studies

Clinical Presentations of Pediatric Horner Syndrome

Purpose: To understand the multiple signs of Horner syndrome and to recommend protocols for pediatricians to obtain an accurate diagnosis of Horner syndrome. Methods: The medical records of 17 pediatric patients with Horner syndrome, neonates to eighteen years of age, were collected and analyzed. Data recorded included age, presenting symptoms, other medical history, allergies, medications, pupil size, presence of anhidrosis, and presence of ptosis. From the available pupil sizes, average degree of anisocoria was calculated. Results: All 17 patients had other clinical findings of Horner syndrome in addition to anisocoria. On initial evaluation, 100% had ptosis and 25% had anhidrosis. Of the available pupil size data, the average level of anisocoria was 2.06 mm, with a standard deviation of 1.17 mm. Conclusion: Physicians are reminded to measure pupil size to determine the degree of anisocoria when present, as it may help distinguish benign conditions from underlying pathology. Educating pediatricians on measurement of anisocoria and additional signs of Horner syndrome will help with proper referral patterns.

桡骨远端骨折占肢体创伤的14%,常见于骨质疏松的老年人。桡骨远端关节内粉碎性骨折处理棘手,高龄骨质疏松患者的处理则更加困难。过去,高龄骨质疏松性桡骨远端骨折通常采用手法复位、石膏固定治疗,多数患者会出现明显的短缩和对线不良。为了得到更好的治疗效果,近来出现了许多新的治疗方法。利用微创技术局部注射磷酸钙骨水泥,如NorianSRS被证明可以加速患者的康复,尽管最终的影像学结果并没有明显改善。最近各种类型的带锁螺钉和内植物迅速普及开来。这些内植物带有直接锁定钢板的螺钉,固定角度的螺栓,可以为骨质疏松性骨折提供良好的固定。今后需要手术处理的骨质疏松性桡骨远端骨折会越来越多,我们已经到了需要反思我们治疗这种常见损伤策略的时候了。

Arthroscopic assessment of soft tissue injuries in comminuted intra-articular fractures of the distal radius

Objective To diagnose and evaluate soft tissue injuries in intra-articular fractures of the distal radius using *** Twenty young patients with displaced intra-articular fractures of distal radius were recruited in this prospective *** AO C2 and 17 C3 fractures were *** arthroscopic examination fractures of distal radius were treated by external fixation with limited internal fixation or open plate *** Triangular fibrocartilage complex(TFCC)injury was found in 18 patients(90%).Most were isolated Palmer type 1D injuries(67 %).Scapho-lunate ligament injury wa s found in two patients:one partial tear(grade Ⅱ)and one complete tear(grad e Ⅲ).Follow-ups ranged from 6 to 18 *** the last follow-up,wrist fun ction was excellent in one patient(6%),good in eight(44%)and fair in 11(50 %).Conclusion TFCC injuries are common in intra-articular fractures of the distal radius while injuries to scapho-lunate ligament are uncommon.

Profiles and removal efficiency of polycyclic aromatic hydrocarbons by two different types of sewage treatment plants in Hong kong

Sewage discharge could be a major source of polycyclic aromatic hydrocarbons （PAHs） in the coastal waters. Stonecutters Island and Shatin Sewage Treatment Works （SCISTW and STSTW） in Hong kong, adopted chemically enhanced primary treatment and biological treatment, respectively. This study aimed at （1） determining the removal efficiencies of PAHs, （2） comparing the capabilities in removing PAHs, and （3） characterizing the profile of each individual PAHs, in the two sewage treatment plants （STPs）. Quantification of 16 PAHs was conducted by a Gas Chromatography. The concentrations of total PAHs decreased gradually along the treatment processes （from 301 ± 255 and 307 ± 217 ns/l to 14.9 ± 12.1 and 63.3 ± 54.1 ng/l in STSTW and SCISTW, respectively）. It was noted that STSTW was more capable in removing total PAHs than SCISTW with average total removal effidency 94.4% ± 4.12% us. 79.2% ± 7.48% （p 〈 0.05）. The removal of PAHs was probably due to sorption in particular matter, confirmed by the higher distribution coefficient of individual and total PAHs in solid samples （dewatered sludge contained 92.5% and 74.7% of total PAHs in SCISTW and STSTW, respectively） than liquid samples （final efffluent-total contained 7.53% and 25.3% of total PAHs in STSTW and SCISTW, respectively）. Despite the impressive capability of STSTW and SCISTW in removing PAHs, there was still a considerable amount of total PAHs （1.85 and 39.3 ks/year, respectively for the two STPs） being discharged into Hone kon coastal waters, which would be an environmental concern.
