Clinical application of concentrated bone marrow aspirate in orthopaedics:A systematic review

AIM To examine the evidence behind the use of concentrated bone marrow aspirate(c BMA) in cartilage, bone, and tendon repair; establish proof of concept for the use of cB MA in these biologic environments; and provide the level and quality of evidence substantiating the use of cB MA in the clinical *** We conducted a systematic review according to PRISMA guidelines. EMBASE, MEDLINE, and Web of Knowledge databases were screened for the use of cB MA in the repair of cartilage, bone, and tendon repair. We extracted data on tissue type, cB MA preparation, cB MA concentration, study methods, outcomes, and level of evidence and reported the results in tables and *** A total of 36 studies met inclusion/exclusion criteria and were included in this review. Thirty-one of 36(86%) studies reported the method of centrifugation and preparation of cB MA with 15(42%) studies reporting either a cell concentration or an increase from baseline. Variation of c BMA application was seen amongst the studies evaluated. Twenty-one of 36(58%) were level of evidence Ⅳ, 12/36(33%) were level of evidence Ⅲ, and 3/36(8%) were level of evidence Ⅱ. Studies evaluated full thickness chondral lesions(7 studies), osteochondral lesions(10 studies), osteoarthritis(5 studies), nonunion or fracture(9 studies), or tendon injuries(5 studies). Significant clinical improvement with the presence of hyaline-like values and lower incidence of fibrocartilage on T2 mapping was found in patients receiving cB MA in the treatment of cartilaginous lesions. Bone consolidation and time to bone union was improved in patients receiving cB MA. Enhanced healingrates, improved quality of the repair surface on ultrasound and magnetic resonance imaging, and a decreased risk of re-rupture was demonstrated in patients receiving cB MA as an adjunctive treatment in tendon repair. Let v be a positive integer and let K be a set of positive integers. A （v, K, 1）-Mendelsohn design, which we denote briefly by （v, K, 1）-md, is a pair （X, B） where X is a v-set （of points） and B is a collection of cyclically ordered subsets of X （called blocks） with sizes in the set K such that every ordered pair of points of X are consecutive in exactly one block of B. If for all t =1, 2,..., r, every ordered pair of points of X are t-apart in exactly one block of B, then the （v, K, 1）-md is called an r-fold perfect design and denoted briefly by an r-fold perfect （v, K, 1）-md. If K = {k） and r = k - 1, then an r-fold perfect （v, （k）, 1）-md is essentially the more familiar （v, k, 1）-perfect Mendelsohn design, which is briefly denoted by （v, k, 1）-Pmd. In this paper, we investigate the existence of 4-fold perfect （v, （5, 8}, 1）-Mendelsohn designs.

This paper presents the improvement of the fuzzy inference model primarily developed for predicting rainfall with data from United States Department of Agriculture (USDA) Soil Climate Analysis Network (SCAN) Station at the Alabama Agricultural and Mechanical University (AAMU) Campus for the year 2004. The primary model was developed with Fuzzy variables selected based on the degree of association of different factors with various combinations causing rainfall. An increase in wind speed (WS) and a decrease in temperature (TP) when compared between the ith and (i-1)th day were found to have a positive relation with rainfall. Results of the model showed better performance after introducing the threshold values of 1) relative humidity (RH) of the ith day;2) humidity increase (HI) when compared between the ith and (i-1)th day;and 3) product (P) of increase in wind speed (WS) and decrease in temperature (TP) when compared between the ith and (i-1)th day. In case of the improved model, errors between actual and calculated amount of rainfall (RF) were 1.20%, 2.19%, and 9.60% when using USDA-SCAN data from AAMU campus for years 2003, 2004 and 2005, respectively. The improved model was tested at William A. Thomas Agricultural Research Station (WTARS) and Bragg farm in Alabama to check the applicability of the model. The errors between the actual and calculated amount of rainfall (RF) were 3.20%, 5.90%, and 1.66% using USDA-SCAN data from WATARS for years 2003, 2004, and 2005, respectively. Similarly, errors were 10.37%, 11.69%, and 25.52% when using SCAN data from Bragg farm for years 2004, 2005, and 2006, respectively. The primary model yielded the value of error equals 12.35% using USDA- SCAN data from AAMU campus for 2004. The present model performance was proven to be better than the primary model.

The study of speciation genetics is primarily concerned with identifying the genetic traits that allow divergent selec- tion to overcome the homogenizing effects of gene flow. Felsenstein reviewed this race between gene flow and selection, con- cluding that speciation with gene flow was unlikely under a ＂two-allele model＂（where two traits were necessary for reproductive isolation） but that divergence could occur quite easily under a ＂one-allele model.＂ Despite this finding, much of the sympatric speciation research involving phytophagous insects has relied on a two-trait model, where insects evolve both preferences for and increased performance on novel host plants. Philopatry （a tendency to remain where one was born） is known to occur in phyto- phagous insects and is a single trait isolation mechanism. However, it is traditionally invoked as simply augmenting reproductive isolation. Species in the Enchenopa binotata complex are believed to have speciated in sympatry. They exhibit host plant prefe- rences, host specific performance advantages and strong philopatry. We experimentally shifted E. binotata to evolutionarily novel host plants. Previous research has demonstrated that the experimental population of insects possesses genetic variation in prefe- rence and performance to the novel host. The degree of philopatry at mating and egg-laying was assayed for the first four years under full choice conditions. Host plant preference and performance was assayed after eight years. Philopatry was an immediate and strong isolating mechanism, while preference for and performance on the novel host lagged. We therefore suggest that philo- patry may be a more important mechanism in the early stages of a host shift than previously believed [Current Zoology 59 （5）： 658-666, 2013].

Predicting strength distributions of MEMS structures using flaw size and spatial density

The populations of flaws in individual layers of microelectromechanical systems(MEMS)structures are determined and verified using a combination of specialized specimen geometry,recent probabilistic analysis,and topographic *** distributions of notched and tensile bar specimens are analyzed assuming a single flaw population set by fabrication and common to both specimen *** the average spatial density of flaws and the flaw size distribution are determined and used to generate quantitative visualizations of *** probe-based topographic measurements are used to verify the flaw spacings determined from strength tests and support the idea that grain boundary grooves on sidewalls control MEMS *** findings here suggest that strength controlling features in MEMS devices increase in separation,i.e.,become less spatially dense,and decrease in size,i.e.,become less potent flaws,as processing proceeds up through the layer *** method demonstrated for flaw population determination is directly applicable to strength prediction for MEMS reliability and design.
