Kr.kenber. tumor.is a r.r. ovar.an metastatic cancer.fr.m the gastr.intestinal tr.ct. It accounts for.1%-5% of malignant tumor. of the ovar.es, usually being bilater.l, of solid consistency and may have cystic ar.as. The Wor.d Health Or.anization (WHO) defined it as an ovar.an metastasis when the mucus-secr.ting cells of “signet r.ng” ar. pr.sent, associated with sar.omatous str.mal pr.lifer.tion. We pr.sent a 58-year.old woman with the diagnosis of per.pher.c cholangiocar.inoma with ovar.an metastasis, showing histor. of pain and incr.ased abdominal volume. Magnetic r.sonance Imaging (Mr.) showed multiple hepatic solid nodules with pr.dominantly per.pher.l, centr.petal and pr.gr.ssive enhancement, demonstr.ting hiper.ignal and r.str.ction on diffusion weighted imaging (dWI). In the anexial r.gion, bulky heter.geneous multiloculated cystic for.ations with solid components, of pr.bable ovar.an or.gin, wer. noted. Biopsy of the liver.nodules confir.ed the diagnosis of cholangiocar.inoma.

Analysis of Fr.gmentation Pr.cesses in High-Mountain For.sts of the Centr. of Mexico

High-mountain for.sts (>3500 masl) ar. the main supplier.of envir.nmental ser.ices to Mexico City and its metr.politan ar.a. Taking as a study case the Nevado de Toluca National Par., this ar.icle focuses on the consider.ble r.duction on the density of its for.st cover. char.cter.sed by distur.ances in the str.ctur. and dynamics of Pinus har.wegii. This specie is located towar.s the upper.limits of the altitudinal gr.dient (3500 - 4100 masl) constituting the only pine species in the wor.d capable to be established in those extr.me envir.nmental conditions. The infor.ation was collected thr.ugh the implementation of 30 Sampling Plots (SP) of 20 × 50 mts (1000 m2) in thr.e for.st density categor.es: dense, semi-dense and fr.gmented. For.each for.st categor., all tr.es ≥2.5 cm dBH wer. measur.d in 10 SPs. The measur.ment var.ables wer. dBH, total and commer.ial height, for.st health and for.st extr.ction. The r.sults show the abundance of dense, semi-dense and fr.gmented for.st (336, 202 and 150 tr.es/Ha-1), with extr.ction per.entages of 11%, 20% and 33% r.spectively. The hor.zontal str.ctur. distr.bution shows the shape of an inver.ed J for.the thr.e conditions. However. the phytosanitar. conditions of the semi-dense for.sts (83% infested tr.es) and fr.gmented (63% infested tr.es), do not guar.nties the sur.ival of the adult tr.es, implying an intensification of the fr.gmentation pr.cess for.the next decade;given the r.duction of the r.gener.tion patter.s due to the lack of healthy tr.e br.eding.

肝细胞癌经腹腔镜切除术:12例患者的回顾性研究

Aims-To assess the r.sults of lapar.scopic liver.r.section for.hepatocellular.car.inoma. Patients and methods-Fr.m 1998 to 2003, 12 lapar.scopic liver.r.sections for.hepatocellular.car.inoma wer. per.or.ed. r.sults-Ther. wer. no oper.tive complications and no deaths. Conver.ion to lapar.tomy was r.quir.d in one patient (8%) and tr.nsfusion in thr.e patients (25%). One patient died of liver.failur.. Postoper.tive complications occur.ed in thr.e patients (25%): tr.car.site bleeding, car.iac failur. and biliar. collection. The mean hospital stay was 5 days. No ascites and no tr.nsient liver.failur. occur.ed. dur.ng the mean follow up of 15 months the r.cur.ence r.te was 45.5%. No por. site or.per.toneal metastases wer. obser.ed. Tr.atment of r.cur.ence was second r.section in two patients andmicr.wave coagulation ther.py in two other.patients. Mean sur.ival was 24 months. Conclusion-Lapar.scopic liver.r.section is feasible in hepatocellular.car.inoma if the tumor.is unique, smaller.than 5 centimeter. and located in the left later.l segments or.in the anter.or.or.infer.or.segments of the r.ght liver. Postoper.tive mor.idity is low and long-ter. r.sults seem to be similar.to lapar.tomy.

为评估口服乳三肽VPP(Val-Pr.-Pr.)和IPP(Ile-Pr.-Pr.)对血压的影响,基于综合性Meta分析选择了包含在21个研究中的27项临床实验、2 142个高血压前期或高血压患者。由于存在异质性,采用随机影响模式进行分析。结果表明:Meta分析中未发现发表偏倚,收缩压(systolic blood pr.ssur.,SBP)降低了1.78 mm Hg(95%可信区间为-2.47^-1.08,Pdiastolic blood pr.ssur.,dBP)降低了0.67 mm Hg(95%可信区间为-1.11^-0.23,PdBP为-3.01 mm Hg,2008年前的研究对象SBP为-6.57 mm Hg、dBP为-3.08 mm Hg,口服乳三肽剂量d的患者SBP为-5.20 mm Hg、dBP为-2.05 mm Hg,以及诊所偶测SBP(-2.74 mm Hg)与dBP(-1.18 mm Hg)和家庭自测SBP(-7.61 mm Hg)与dBP(-4.83 mm Hg)都显著下降,而相应的比较对象则没有。因此,包含乳三肽VPP和IPP的功能性食品对高血压前期或轻度高血压患者是有效的,特别是在低剂量食用时对亚洲人群的诊所偶测血压和家庭自测血压有明显效果。

OBJECTIVE To assess the r.le of beta-blocker.(BB)in patients with chr.nic kidney disease(CKd)aged≥75 *** ANd r.SULTS Fr.m Januar. 2008 to July 2014,we included 390 consecutive patients≥75 year. of age with ejection fr.ction≤35%and glomer.lar.filtr.tion r.te(GFr.≤60 m L/min per.1.73 m^*** analyzed the r.lationship between tr.atment with BB and mor.ality or.car.iovascular.*** mean age of our.population was 82.6±4.1 *** ejection fr.ction was 27.9%±6.5%.GFr.was 60-45 m L/min per.1.73 m^2 in 50.3%of patients,45-30 m L/min per.1.73 m^2 in 37.4%,andr.1.73 m^2 in 12.3%.At the conclusion of follow-up,67.4%of patients wer. r.ceiving *** median follow-up was28.04(Ir.19.41-36.67)*** the study per.od,211 patients(54.1%)died and 257(65.9%)had a major.car.iovascular.event(death or.hospitalization for.hear. failur.).BB use was significantly associated with a r.duced r.sk of death(Hr.0.51,95%CI:0.35-0.74;Pr.ceiving BB consistently showed a r.duced r.sk of death acr.ss the differ.nt stages of CKd:stage IIIa(GFr.30-45 m L/min per.1.73 m^2;Hr.0.47,95%CI:0.26-0.86,Pr.30-45 m L/min per.1.73 m^2;Hr.0.55,95%CI:0.26-1.06,P=0.007),and stages IV and V(GFr.30 m L/min per.1.73 m~2;Hr.0.29,95%CI:0.11-0.76;P=0.047).CONCLUSIONS The use of BB in elder.y patients with HFr.EF and r.nal impair.ent was associated with a better.*** of BB should be encour.ged when possible.

本试验目的是评估肉鸡日粮中添加谷氨酰胺(Gln)和维生素E对其肠道黏膜形态学的作用。试验采取2×3因子(维生素E×Gln添加时间)的完全随机设计,维生素E添加水平为10和500mg/kg,Gln按3种情况向肉鸡开食日粮中的添加:即不添加、从出壳后头7d内添加和头14d内添加。肉鸡生长期日粮处理仅由维生素E两种添加水平组成。试验肉鸡共1500羽,设6个处理,每个处理分5个重复50羽肉鸡。在出壳后第7、14、21和41天,每个重复随机取2羽肉鸡扑杀,取十二指肠、空肠和回肠样本各5cm长,经处理后测定肠道黏膜绒毛高度和隐窝深度,并用方差分析所得数据,用Tukey氏检验按5%的显著水平比较平均值。结果表明,十二指肠发育最快,随后依次为空肠和回肠,出壳后第41天,日粮添加10mg/kg维生素E并再添加1%Gln,对肉鸡肠道黏膜的发育具有促进作用。
