BACKGroUND Pancreaticoduodenectomy(PD)is a technically complex operation,with a re-latively high risk for *** ability to rescue patients from post-PD complications is as a recognized quality *** protocols were instituted at our low volume facility in the year *** To document the rate of rescue from post-PD complications with tailored protocols in place as a measure of *** A retrospective audit was performed to collect data from patients who experienced major post-PD complications at a low volume pancreatic surgery unit in Trinidad and Tobago between january 1,2013 and june 30,***-dardized definitions from the International Study Group of Pancreatic Surgery were used to define post-PD complications,and the modified Clavien-Dindo classification was used to classify post-PD *** over the study period,113 patients at a mean age of 57.5 years(standard deviation[SD]±9.23;range:30-90;median:56)underwent PDs at this *** complications were recorded in 33(29.2%)patients at a mean age of 53.8 years(SD:±7.9).Twenty-nine(87.9%)patients who experienced major morbidity were salvaged after aggre-ssive treatment of their ***(3.5%)died from bleeding pseudoaneurysm(1),septic shock secondary to a bile leak(1),anastomotic leak(1),and myocardial infarction(1).There was a significantly greater salvage rate in patients with American Society of Anesthesiologists scores≤2(93.3%vs 25%;P=0.0024).CoNCLUSIoN This paper adds to the growing body of evidence that volume alone should not be used as a marker of quality for patients requiring *** low volumes at our facility,we demonstrated that 87.9%of patients were rescued from major *** attributed this to several factors including development of rescue protocols,the competence of the pancreatic surgery teams and continuous,and adaptive learning by the entire institution,cul-minating in the development of tailored peri-pancreatectomy protoc

A Facial Expression recognition Method Integrating Uncertainty Estimation and Active Learning

The effectiveness of facial expression recognition (FEr) algorithms hinges on the model’s quality and the availability of a substantial amount of labeled expression data. However, labeling large datasets demands significant human, time, and financial resources. Although active learning methods have mitigated the dependency on extensive labeled data, a cold-start problem persists in small to medium-sized expression recognition datasets. This issue arises because the initial labeled data often fails to represent the full spectrum of facial expression characteristics. This paper introduces an active learning approach that integrates uncertainty estimation, aiming to improve the precision of facial expression recognition regardless of dataset scale variations. The method is divided into two primary phases. First, the model undergoes self-supervised pre-training using contrastive learning and uncertainty estimation to bolster its feature extraction capabilities. Second, the model is fine-tuned using the prior knowledge obtained from the pre-training phase to significantly improve recognition accuracy. In the pre-training phase, the model employs contrastive learning to extract fundamental feature representations from the complete unlabeled dataset. These features are then weighted through a self-attention mechanism with rank regularization. Subsequently, data from the low-weighted set is relabeled to further refine the model’s feature extraction ability. The pre-trained model is then utilized in active learning to select and label information-rich samples more efficiently. Experimental results demonstrate that the proposed method significantly outperforms existing approaches, achieving an improvement in recognition accuracy of 5.09% and 3.82% over the best existing active learning methods, Margin, and Least Confidence methods, respectively, and a 1.61% improvement compared to the conventional segmented active learning method. © 2024 The Authors.

Factors resisting protein adsorption on hydrophilic/hydrophobic self-assembled monolayers terminated with hydrophilic hydroxyl groups

The hydroxyl-terminated self-assembled monolayer(oH-SAM),as a surface resistant to protein adsorption,exhibits substantial potential in applications such as ship navigation and medical implants,and the appropriate strategies for designing anti-fouling surfaces are ***,we employ molecular dynamics simulations and alchemical free energy calculations to systematically analyze the factors influencing resistance to protein adsorption on the SAMs terminated with single or double oH groups at three packing densities(∑=2.0 nm^(-2),4.5 nm^(-2),and 6.5 nm^(-2)),*** the first time,we observed that the compactness and order of interfacial water enhance its physical barrier effect,subsequently enhancing the resistance of SAM to protein ***,the spatial hindrance effect of SAM leads to the embedding of protein into SAM,resulting in a lack of resistance of SAM towards ***,the number of hydroxyl groups per unit area of double oH-terminated SAM at ∑=6.5 nm^(-2) is approximately 2 to 3 times that of single oH-terminated SAM at ∑=6.5 nm^(-2) and 4.5 nm^(-2),consequently yielding a weaker resistance of double oH-terminated SAM towards ***,due to the structure of SAM itself,i.e.,the formation of a nearly perfect ice-like hydrogen bond structure,the SAM exhibits the weakest resistance towards *** study will complement and improve the mechanism of oH-SAM resistance to protein adsorption,especially the traditional barrier effect of interfacial water.

Single-atom Pt on carbon nanotubes for selective electrocatalysis

Utilizing supported single atoms as catalysts presents an opportunity to reduce the usage of critical raw materials such as platinum,which are essential for electrochemical reactions such as hydrogen oxidation reaction(Hor).Herein,we describe the synthesis of a Pt single electrocatalyst inside single-walled carbon nanotubes(SWCNTs)via a redox *** via electron microscopy,X-ray photoelectron microscopy,and X-ray absorption spectroscopy show the single-atom nature of the *** electrochemical behavior of the sample to hydrogen and oxygen was investigated using the advanced floating electrode technique,which minimizes mass transport limitations and gives a thorough insight into the activity of the *** single-atom samples showed higher Hor activity than state-of-the-art 30%Pt/C while almost no oxygen reduction reaction activity in the proton exchange membrane fuel cell operating *** selective activity toward Hor arose as the main fingerprint of the catalyst confinement in the SWCNTs.

Toxicity of a seafood toxin， domoic acid， in the retina via modulation of the NrF2 and NF-κB pathways

Domoic acid (DA)， a biotoxin， is produced by several species of marine dinoflagellate and diatom during harmful algal bloom events. DA is a neurotoxin， in humans and non-human primates， oral exposure to DA results in gastrointestinal effects， while DA at higher doses leads to neurological symptoms， seizures and memory deficiency. Exposure of humans to DA occurs mainly through consumption of contaminated seafoods containing an accumulation of the toxin. Previously， it was unclear if DA can have toxic effects on the retina. We assessed the toxicity of DA in human retinal cells (ArPE-19) and in zebrafish embryos. DA significantly lowered ArPE-19 cell viability dose-dependently， and decreased anti-oxidative capacity， increased inflammation， and promoted cell death， possibly through modulating the NrF2 and NF-κB signalling pathways. Zebrafish embryos exposed to DA for four days from one day post fertilization (dpf) had an increase in mortality and a decrease in both hatching and heartbeat rate and exhibited morphological abnormalities. DA treatment also significantly downregulated expression of antioxidant genes and upregulated expression of inflammation mediators， as well as causing photoreceptor death in zebrafish embryos. The results demonstrate that consuming seafood containing DA will have potential toxic effects in human retinas.
