目的:探讨玻璃体腔内注射bevacizumab (Avastin)联合白内障手术预防脉络膜新生血管(choroidal neovascularization,CNV)的再次激活及年龄相关性黄斑变性(age-relatedmacular degeneration, AMD)。方法:对12例接受湿性年龄相关变性治疗的白内障患者给予透明角膜切口白内障超声乳化和人工晶状体植入手术,最后予以玻璃体腔内注射1.25mg bevacizumab (0.05mL)。主要评价指标:视力恢复、脉络膜新生血管的再次激活,及相干光断层成相术(OCT)对渗漏液体的评估。结果:患者最佳矫正视力(best-corrected visual acuity, BCVA)术后显著提高(脉络膜新生血管闭合后P<0.01,白内障发生后P=0.049,t检验)。白内障术后平均随访时间为11.8mo(标准差6.1,范围3 ～22mo),随访期内患者未出现脉络膜新生血管的再次激活及新的脉络膜新生血管病变。结论:在给予脉络膜新生血管及年龄相关性黄斑变性治疗的患者,白内障手术联合玻璃体腔内注射bevacizumab可预防脉络膜新生血管的再次激活。

目的:报道经玻璃体腔内注射高剂量曲安奈德(triamcinolone acetonide,TA)联合光动力学疗法(photodynamic therapy,PDT)治疗老年性黄斑变性( age related macular degeneration, AMD)的脉络膜新生血管(choroidal neovascularization,CNV)后发生的脉络膜毛细血管萎缩。方法:我们采用非随机回顾性干涉治疗病例。在阿利坎特学院眼科,连续观察51眼(实验组)玻璃体腔内的注射(19.4±2.1)mg/0.1mL TA联合PDT治疗AMD的全部中心凹下型CNV患者,经过2a的随访,检查黄斑部脉络膜毛细血管和视网膜色素上皮细胞(RPE)萎缩情况。同时,采用单独PDT治疗的连续30眼患者作为对照组,其年龄,性别和AMD的CNV类型及大小与实验组相匹配。结果:随访24mo后,在治疗区域21/47眼(45%,实验组)和7/30眼(23%,对照组)发展成黄斑部RPE和脉络膜毛细血管萎缩(P=0.04,卡方检验)。实验组平均最大萎缩区域的直径为(5044±1666)μm,而对照组为(4345±1550)μm。在实验组中,RPE萎缩患者的平均最佳矫正视力为(0.87±0.33),而非RPE萎缩患者的平均最佳矫正视力为(0.66±0.26) (P=0.11,秩和U检验)。结论:玻璃体腔内注射大剂量TA联合PDT治疗可能会增加RPE和脉络膜毛细血管萎缩的风险。

Tuberculosis (TB) incidence within Healthcare workers remains one of the lowest occupation specific rates in the United States. We report a case of a 33-year-old African American male presented with a right chest mass and without fever, chills or cough. Bacterial and Fungal cultures were sent during the initial visit and were negative throughout the entire management of his care. After several months of surgical interventions, the patient appeared positive for TB;however a staff member contracted the disease. The patient was managed successfully with the four-drug regimen of Isoniazid, Rifampin, Pyrazinamide and Ethambutol and recovered well from his surgeries.

Diabetes mellitus is a major health problem with increasing prevalence at a global *** discovery of insulin in the early 1900 s represented a major breakthrough in diabetes management,with further milestones being subsequently achieved with the identification of glucagon-like peptide-1(GLP-1)and the introduction of GLP-1 receptor agonists(GLP-1 RAs)in clinical ***,the subcutaneous delivery of biotherapeutics is a well-established route of administration generally preferred over the intravenous route due to better patient compliance and prolonged drug ***,current subcutaneous formulations of GLP-1 RAs present pharmacokinetic problems that lead to adverse reactions and treatment *** this review,we discuss the current challenges of subcutaneous administration of peptide-based therapeutics and provide an overview of the formulations available for the different routes of administration with improved bioavailability and reduced frequency of administration.

First Characterization of <i>Escherichia coli</i>Strains Isolated from Wildlife Griffon Vulture (<i>Gyps fulvus</i>) in the Southeast of Spain

The aim of the present study was to characterize a collection of Escherichia coli strains isolated from asymptomatic griffon vulture (Gyps fulvus) during a reintroduction program in the southeast of Spain, in order to establish if griffon vulture could play a role in the spread of resistant or potentially pathogenic E. coli strains. For this purpose, 14 E. coli strains obtained from 10 griffon vulture were studied to establish their serotypes, phylogroups, virulencegene profiles and antimicrobial resistances. High heterogeneity was observed within the 14 strains isolated which belonged to three phylogroups (A, B1 and D), 8 serogroups (O2, O21, O29, O60, 073, O78, O103 and O141) and 13 different serotypes. Out of 34 genes screened, we have detected eight virulence genes that are typical of extraintestinal pathogenic E. coli (ExPEC) (fimH, fimAvMT78, iroN, iucD, cvaC, iss, traT and tsh);however, none of the studied strains showed the ExPEC status. The 14 strains were also analyzed for the production of extended-spectrum beta-lactamases (ESBLs) and for antimicrobial resistances. None of the 14 strains were ESBL-producing E. coli, but high resistance-prevalences to ampicillin and cotrimoxazole were detected. To our knowledge, this is the first characterization of E. coli strains isolated from griffon vulture and although they did not show high virulencegene scores, they showed cotrimoxazole resistance.
