Sub-surface crack networks in areas of altered microstructure are a common cause for bearing *** to its appearance under light microscopy,the damage pattern is referred to as White Etching Cracks(WEC).The root causes leading to the formation of WEC are still under ***,it has already been shown that atomic hydrogen can have an accelerating effect on the formation and propagation of *** addition to hydrogen pre-charging,hydrogen can be released and absorbed during rolling/sliding due to the decomposing of the lubricant and *** current work focuses on the analysis of the hydrogen content of cylindrical roller thrust bearings after testing in a FE8 type test rig using two different *** the framework of this work,two different hydrogen analysis methods were used and assessed regarding their *** results show that the so-called Hydrogen Collecting Analysis(HCA)is more suitable to investigate the correlation between lubricant chemistry and hydrogen content in the test bearings than the Local Hydrogen Analysis(LHA).The measurements with the HCA show a continuously increasing freely movable and diffusible hydrogen content under tribological conditions,which leads to the formation of *** tests with an oil without hydrogen showed that the tendency of the system to fail as a result of WEC can be reduced by using a lubricant without hydride compounds.

Different patterns of intestinal response to injury after arterial,venous or arteriovenous occlusion in rats

AIM： To investigate the differences in injury patterns caused by arterial, venous or arteriovenous mesenteric occlusion. METHODS： Male Wistar rats were separated equally into four groups. Occlusion was performed by clamping the superior mesenteric artery （A）, the mesenteric vein （V） or both （AV） for 30 min, followed by 60 min of reperfusion. A control group received sham surgery only. Intestinal sections were examined for histological damage and serum tumor necrosis factor-α （TnF-α）, endothelin-1 （ET-1）, P-selectin, antithrombin Ⅲ （AT Ⅲ） and soluble intracellular adhesion molecule-1 （ICAM-1） concentrations were measured. RESULTS： All groups showed significant mucosal injury compared to controls. Furthermore, mucosal injury was significantly more severe in the V and AV groups compared to the A group （3.6 ± 0.55, 3.4 ± 0.55 and 2± 0.71, respectively, P = 0.01）. ICAM-1 was similarly elevated in all groups, with no significant differences between the groups. P-selectin levels were significantly elevated in the V and AV groups but not the A group （1.4 ± 0.5 ng/mL, 2.52 ± 0.9 ng/mL and 0.02± 0.01 ng/mL, respectively, P = 0.01） and ET-1 was significantly elevated in the A and V groups but not the AV group （0.32 ± 0.04 pg/mL, 0.36 ± 0.05 pg/mL and 0.29 ± 0.03 pg/mL, respectively, P = 0.01） compared to sham controls. ATⅢ levels were markedly depleted in the V and AV groups, but not in the A group （29.1 ± 5.2 pg/mL, 31.4 ±21.8 pg/mL and 55.8± 35.6 pg/mL, respectively, P = 0.01）, compared to controls. Serum TnF-α was significantly increased in all groups compared to sham controls （1.32 ± 0.87 ng/mL, 1.79 ± 0.20 ng/mL and 4.4 ± 0.69 ng/mL, for groups A, V and AV, respectively, P = 0.01）, with higher values in the AV group. Ketamine anesthesia reduces intestinal ischemia/reperfusion injury in rats

AIM: To investigate the effects of ketamine anesthesia on the motility alterations and tissue injury caused by ischemia/reperfusion in rats. METHODS: Thirty male Wistar rats weighing 200-250 g were used. Ischemia was induced by ob-structing blood flow in 25% of the total small intesti-nal length (ileum) with a vascular clamp for 45 min, after which either 60 min or 24 h of reperfusion was allowed. Rats were either anesthetized with pento-barbital sodium (50 mg/kg) or ketamine (100 mg/kg). Control groups received sham surgery. After 60 min of reperfusion, the intestine was examined for mor-phological alterations, and after 24 h intestinal basic electrical rhythm (BER) frequency was calculated, and intestinal transit determined in all groups. RESULTS: The intestinal mucosa in rats that were anesthetized with ketamine showed moderate altera-tions such as epithelial lifting, while ulceration and hemorrhage was observed in rats that received pento-barbital sodium after 60 min of reperfusion. Quantita-tive analysis of structural damage using the Chiu scaleshowed significantly less injury in rats that received ketamine than in rats that did not (2.35 ± 1.14 vs 4.58 ± 0.50, P nce traveled by a mark-er, expressed as percentage of total intestinal length, in rats that received pentobarbital sodium was 20% ± 2% in comparison with 25.9% ± 1.64% in rats that re-ceived ketamine (P = 0.017). BER was not statistically different between groups. COnCLUSIOn: Our results show that ketamine anesthesia is associated with diminished intestinal injury and abolishes the intestinal transit delay induced by ischemia/reperfusion.

ng>Background: ng>Pre-clinical and clinical studies have shown that inflammatory pain intensity is increased under diabetes condition. Open cholecystectomy (OC) is a surgical procedure with predictable postoperative pain. However, the comparison of postoperative pain due to open cholecystectomy in diabetic and non-diabetic patients remains unknown. The research question to answer was whether diabetic patients undergoing OC development greater intensity of postoperative pain than non-diabetic patients. ng>Methods: ng>The study was conducted between June 2016 and February 2018 at the Regional Hospital of High Specialty “Dr. Juan Graham Casasús” of Villahermosa, Tabasco, Mexico. Seventy patients in two groups of 35 patients each scheduled for OC under general anesthesia were studied. Pain was assessed using the 11-point numerical rating scale (nRS). The primary endpoint was to know nRS pain scores after awaking of general anesthesia. Secondary outcomes included the time of onset of pain and comparing nRS scores between diabetic and non-diabetic patients undergoing OC. ng>Results:ng> Diabetic patients reported significantly greater intensity pain than non-diabetic patients. The mean overall pain score in the diabetic and non-diabetic patients was 7.2 ± 0.3 and 5.3 ± 0.3 (P = 0.0002), respectively. Furthermore, 60% of diabetic patients had severe pain (nRS ≥ 8) compared to 20% of non-diabetics (P = 0.006). The time to onset postoperative pain was about 35 minutes in both groups (P = 0.876). ng>Conclusions:ng> Diabetic patients undergoing OC have greater intensity postoperative pain and also more frequency of patients with severe pain scores compared with non-diabetic patients. Therefore, analgesic treatment in those patients should consider this point in order to provide a satisfactory postoperative analgesia.

Effect of Aerobic Exercise on Protein Expression in Muscle of Obese Mexican Adolescents: A Proteomic and Bioinformatic Analysis

The beneficial effects of exercise have been recognized for many years yet the molecular mechanisms by which exercise show benefits on health are still elusive. A combination of experimental and bioinformatics approaches can be an invaluable tool to increase the cellular understanding of the molecular mechanisms that underlie the aerobic exercise in obese adolescents. Muscle skeletal proteins were separated by 2D-PAGE and changes in protein expression were revealed by ImageMaster 2D Platinum analysis software. Proteins with expression changes after aerobic exercise were identified by comparison with 2D maps from SWISS-2DPAGE and interactions of proteins were analyzed with STRInG and DAVID databases. After aerobic exercise, all participants decreased glucose, insulin, total cholesterol, body mass index and waist circumference. Proteomic differential analysis revealed 10 overexpressed proteins after aerobic training, which correspond to carbonic anhydrase III, beta enolase, creatine kinase, ATP synthase beta subunit, aldolase A, glyceraldehyde-3-phosphate dehydrogenase, triosephosphate isomerase, pyruvate kinase, lactate dehydrogenase and adenylate kinase. Exercise increased glycolysis and oxidative phosphorylation pathways, which is likely to be regulated by ubiquitin c protein. In conclusion, our analysis suggested that obese adolescents show changes in their body mass index, waist circumference and serum glucose after aerobic exercise through improved protein expression patterns that aid, especially, glucose metabolism and increase the oxidative phosphorylation. The most likely explanation for the observed pattern is that ubiquitin c protein keeps the glycolytic enzymes bound within skeletal muscle, obtaining a non-degradative role in cell signaling.

Penile prosthesis: Patient satisfaction, use and preference for malleable vs inflatable

AIM: To evaluate and compare long-term patient satisfaction and use after either malleable penile prosthesis(MPP) or inflatable penile prosthesis(IPP) implantation. METHODS: we present a retrospective unicenter study of 108 patients implanted with either 2 or 3-piece American Medical System^(TM)(AMS^(TM)) or Coloplast^(TM) inflatable penile prosthesis(AMS 700CX^(TM), AMS 700CXR^(TM), AMS Ambicor^(TM) or Coloplast TITAn^(TM)) or malleable(AMS Spectra^(TM) or Coloplast Genesis^(TM)) in our Centre between 1993 and 2011. We collected data from the medical record including follow-up, age and type of prosthesis. We used a four-question telephone surveydesigned ad hoc by urologist in our Department, with three multiple choice questions and a grading answer. After verbal consent was obtained, proposed questions concerned global satisfaction regarding to the procedure, quality of sexual intercourses graded from 0 to 10, frequency of sexual intercourse and about undergoing the same procedure again. SPSS^(TM) version 20.0 was used for the descriptive analysis of the data. RESULTS: Sixty seven(64%) patients underwent a MPP and 41(36%) an IPP. The mean age was 52.6 ± 3.6 years in the MPP group and 57.2 ± 2.8 years in the IPP group(P = 0.02). Total respond rate was 55.5%(60/108). Twenty six out of 33 MPP patients(78.9%) and 19 of the 27 IPP subjects(70.3%) were satisfied or very satisfied with the procedure. The quality of sexual intercourse was rated 7.13 ± 0.39 points in the MPP group and 6.16 ± 0.47 points in the IPP group. Frequency of sexual intercourse was 1 or more times per week in 15(46.9%) patients with MPP and in 12(46.1%) of the IPP patients. Twenty-eight(84.9%) patients who received a MPP would undergo the procedure for the same device again as well as 24(88.9%) of the IPP group. ng>Introduction:ng> Post-hospital syndrome (PHS) is defined as a period of vulnerability during the first 30 days after a patient is discharged from hospital, in which multiple factors come into play. Hyponatremia is the most frequent hydroelectrolytic disorder in hospitalized patients and may be related to the appearance of PHS. ng>Objective: ng>The objective is to estimate the prevalence of PHS that is assessed as the rate of readmissions in the first 30 days after discharge, in patients with hyponatremia. ng>Material and Methods:ng> It is a descriptive observational study of patients with hyponatremia who were discharged from 1 September 2010 to 2 February 2020 at the Internal Medicine Service of the Hospital University of San Juan (Alicante, Spain). ng>Results:ng> Of the 25 included patients, 5 (20%) were readmitted within a month of discharge, after a mean of 11.4 days (standard deviation [SD] 5.1). The overall mortality of the study was 20% (n = 5), with one case of death in the first 30 days post-hospitalization (4%). In 12 patients (48%) the origin of the hyponatremia was undetermined. The most frequently recorded etiology for the condition was pharmacological (n = 7, 28%), and there was pronounced variability in its clinical and laboratory study. The most widely used corrective measure was drug withdrawal, in 16 patients (64%). Water intake restriction was the most common treatment after discharge (5 patients, 20%), followed by urea (2 patients, 8%), while tolvaptan was not used. ng>Conclusion: ng>Hyponatremia may be the cause of PHS, which could increase the rate of early readmission. Hyponatremia is an underdiagnosed and undertreated entity, so it is necessary to apply an appropriate system to optimize its management and, in future studies, to assess its impact on PHS.
