In vitro intracellular IFNγ, IL-17 and IL-10 producing T cells correlates with the occurrence of post-transplant opportunistic infection in liver and kidney recipients

AIM To validate intracellular cytokine production functional assay as means of cell-mediated immunity monitoring of post-transplant patients with opportunistic infection(OI).METHODS Intracellular cytokine-producing CD4^+ and CD8^+ T-cell monitoring was carried out in 30 liver transplant(LTr) and 31 kidney transplant(KTr) recipients from 2010 to 2012. Patients were assessed in our Department of Immunology at the Clinical University ‘Hospital Virgen de la Arrixaca-IMIB' in Murcia, Spain for one year following transplantation. FACS Canto Ⅱ flow cytometer was employed to quantify the intracellular production of IL-17, IFNγ and IL-10 cytokines on stimulated CD4^+CD69^+ and CD8^+CD69^+ T cells and BD FACS DIVA v.6 software was used to analysed the data. Statistical analysis was carried out using SPSS *** LTr with OI had significantly lower % of CD8^+CD69^+IFNγ^+T cells at 60(7.95 ± 0.77 vs 26.25 ± 2.09, P < 0.001), 90(7.47 ± 1.05 vs 30.34 ± 3.52, P < 0.001) and 180(15.31 ± 3.24 vs 24.59 ± 3.28, P = 0.01) d posttransplantation. Higher % of CD4^+CD69^+IL-10^+ as well as CD4^+CD69^+IL-17^+ T cells were yet reported at 30(14.06 ± 1.65 vs 6.09 ± 0.53, P = 0.0007 and 4.23 ± 0.56 vs 0.81 ± 0.14, P = 0.005; respectively), 60(11.46 ± 1.42 vs 4.54 ± 0.91, P = 0.001 and 4.21 ± 0.59 vs 1.43 ± 0.42, P = 0.03; respectively) and 90 d(16.85 ± 1.60 vs 4.07 ± 0.63, P < 0.001 and 3.97 ± 0.43 vs 0.96 ± 0.17, P = 0.001). Yet, KTr with OI had significantly lower percentage of CD4^+CD69^+IFNγ^+ at 30(11.80 ± 1.59 vs 20.64 ± 3.26, P = 0.035), 60(11.19 ± 1.35 vs 15.85 ± 1.58, P = 0.02), 90(11.37 ± 1.42 vs 22.99 ± 4.12, P = 0.028) and 180(13.63 ± 2.21 vs 21.93 ± 3.88, P = 0.008) d post-transplantation as opposed to CD4^+CD69^+IL-10^+ and CD8^+CD69^+IL-10^+ T cells which percentages were higher at 30(25.21 ± 2.74 vs 8.54 ± 1.64, P < 0.001 and 22.37 ± 1.35 vs 17.18 ± 3.54, P = 0.032; respectively), 90(16.85 ± 1.60 vs 4.07 ± 0.63, P < 0.001 and 23.06 ± 2.89 vs 10.19 ± 1.98, P = 0.002) and 18

Update on the treatment of type 2 diabetes mellitus

To achieve good metabolic control in diabetes and keep long term, a combination of changes in lifestyle and pharmacological treatment is necessary. Achieving near-normal glycated hemoglobin significantly, decreases risk of macrovascular and microvascular complications. At present there are different treatments, both oral and injectable, available for the treatment of type 2 diabetes mellitus(T2DM). Treatment algorithms designed to reduce the development or progression of the complications of diabetes emphasizes the need for good glycaemic control. The aim of this review is to perform an update on the benefits and limitations of different drugs, both current and future, for the treatment of T2 DM. Initial intervention should focus on lifestyle changes. Moreover, changes in lifestyle have proven to be beneficial, but for many patients is a complication keep long term. Physicians should be familiar with the different types of existing drugs for the treatment of diabetes and select the most effective, safe and better tolerated by patients. Metformin remains the first choice of treatment for most patients. Other alternative or second-line treatment options should be individualized depending on the characteristics of each patient. This article reviews the treatments available for patients with T2 DM, with an emphasis on agents introduced within the last decade.

The Transcervical Extended Access, a Feasible Approach for the Surgical Treatment of Benign Tumors of the Posterior Mediastinum?

Transcervical approach for tumors of the posterior mediastinum is traditionally thought not to be indicated. Hereby we report on a case of a patient with a huge neurogenic tumor of the posterior mediastinum which was successfully excised through a transcervical extended approach and, additionally, the variety of surgical approaches used to remove tumors of the posterior mediastinum is discussed. The new refined techniques of transcervical extended mediastinal operations, which are recently gaining popularity among surgeons, allowed for a safe dissection of the tumor, thus patient could benefit from a short painless postoperative course. The authors suggest that surgeons trained in these particular techniques should consider the choice of the transcervical extended access in selected cases of benign tumors of the posterior mediastinum.

目的:报道经玻璃体腔内注射高剂量曲安奈德(triamcinolone acetonide,TA)联合光动力学疗法(photodynamic therapy,PDT)治疗老年性黄斑变性( age related macular degeneration, AMD)的脉络膜新生血管(choroidal neovascularization,CNV)后发生的脉络膜毛细血管萎缩。方法:我们采用非随机回顾性干涉治疗病例。在阿利坎特学院眼科,连续观察51眼(实验组)玻璃体腔内的注射(19.4±2.1)mg/0.1mL TA联合PDT治疗AMD的全部中心凹下型CNV患者,经过2a的随访,检查黄斑部脉络膜毛细血管和视网膜色素上皮细胞(RPE)萎缩情况。同时,采用单独PDT治疗的连续30眼患者作为对照组,其年龄,性别和AMD的CNV类型及大小与实验组相匹配。结果:随访24mo后,在治疗区域21/47眼(45%,实验组)和7/30眼(23%,对照组)发展成黄斑部RPE和脉络膜毛细血管萎缩(P=0.04,卡方检验)。实验组平均最大萎缩区域的直径为(5044±1666)μm,而对照组为(4345±1550)μm。在实验组中,RPE萎缩患者的平均最佳矫正视力为(0.87±0.33),而非RPE萎缩患者的平均最佳矫正视力为(0.66±0.26) (P=0.11,秩和U检验)。结论:玻璃体腔内注射大剂量TA联合PDT治疗可能会增加RPE和脉络膜毛细血管萎缩的风险。

Efficacy and Tolerability of Thermocoagulation Treatment of High-Grade Cervical Intraepithelial Neoplasia

Purpose: Cervical cancer is still one of the main causes of cancer and mortality in women, especially in low- and middle-income countries, although it is a completely preventable disease through the detection and treatment of pre-cancer lesions. We evaluated the efficacy and tolerability of thermocoagulation treatment of high-grade cervical intraepithelial neoplasia (CIN 2 and 3). Methods: We evaluated 115 women with high-grade cervical intraepithelial neoplasia, 54 with CIN 2 and 61 with CIN 3, confirmed by biopsy and without previous treatment, from January 2016 to December 2018, undergoing thermocoagulation treatment at the Lower Genital Tract Pathology and Colposcopy Service of the Hospital de Clinicas de Porto Alegre, Porto Alegre, Brazil. Results: The mean age was 33.11 years (SD = 9.83) for CIN 2 and 35.28 years (SD = 7.97) for CIN 3 patients. Treatment efficacy was 90.8% in CIN 2 and 94.9% in CIN 3 cases. Pain was the main symptom reported at the time of treatment, occurring more frequently in the CIN 3 group (49.1% versus 27.8% in the CIN 2 group). As the more important long-term complication, there were 3 cases of pelvic inflammatory disease in CIN 2 (5.6%) and 3 in CIN 3 group (5.0%). We present numerical studies on the switching characteristics of a fiber Bragg grating (FBG) with modulation in the third order nonlinear index of refraction along it’s length. The FBG is operating in a continuous wave regime (CW). This study was done taking into account the possible asymmetry brought by the non harmonic modulation of the nonlinearity, leading to different reflection and transmission characteristics, that depend on the direction of propagation along the modulated nonlinear FBG. This phenomenon may be useful for applications like an optical isolator. It was found that for a set of values of the modulation parameter, the FBG can exhibit multistable states. The numerical studies were obtained starting from the coupled-mode equations solved from the coupled-mode theory and simulated using the fourth-order Runge-Kutta method.

Effect of Thickness on Magnetic Dipolar and Exchange Interactions in SmCo/FeCo/SmCo Thin Films

SmCo/FeCo/SmCo trilayer was deposited with two different thickness configurations for soft phase (FeCo);50 nm/10 nm/50 nm and 50 nm/25 nm/50 nm were deposited on Si (111) substrate and Ta (50 nm) seed layer by RF magnetron sputtering in a pressure, p, of 30 - 35 m Torr. After deposition the films were annealed under Ar atmosphere at temperature T equal to 923 and 973 for different times followed by quenching in water. X-ray diffraction patterns were obtained to identified phase presents and calculate average crystallite size. To study the effect of configuration thickness in soft phases, DC magnetic measurements were carried out;the measurements were done in the temperature interval of 300 - 50 K. Hysteresis loops collected at low temperatures exposed an increment in coercivity with the decrease of T and at same time, presented a “knee” in the second quadrant of the demagnetization curve, which suggests that the inter-layer exchange coupling becomes less effective, being more evident for sample with 50 nm/25 nm/50 nm thickness. Moreover, δM (H) plots were calculated from magnetic measurements at three different temperatures, T, equal to 300, 150 and 50 K, which corroborates that the dipolar interactions became stronger when thickness of soft phases increases. Finally, the thickness effect is attributed to the SmCo5 phase magnetocrystalline anisotropy constant, which is responsible for the exchange coupling length.

Prey availability, prey selection, and trophic niche width in the lizard Psammodromus algirus along an elevational gradient

Mountains imply enormous environmental variation, with alpine habitats entailing harsh environ- ments, especially for ectotherms such as lizards. This environmental variability also may imply variation in prey availability. However, little is known about how lizard trophic ecology varies with elevation. In this study, we analyze diet, prey availability, prey selection, and trophic niche width in the lacertid lizard Psammodromus algirus along a 2,200-m elevational gradient in the Sierra Nevada （SE Spain）. The analysis of fecal samples has shown that Orthoptera, Formicidae, Hemiptera, Coleoptera, and Araneae are the main prey, although, according to their abundance in pitfall traps, Formicidae and Coleoptera are rejected by the lizard whereas Orthoptera, Hemiptera, and Araneae are preferred. Prey abundance and diversity increase with elevation and diet subtly varies along with the elevational gradient. The consumption of Coleoptera increases with elevation probably as a consequence of the lizard foraging more in open areas while basking. The electivity for Araneae increases with elevation. Araneae are rejected in the lowlands--where they are rela- tively abundant--whereas, at high elevation, this lizard positively selects them, despite they being less abundant. The lizard trophic niche width expands with elevation due to concomitant greater prey diversity and hence this lizard feeds on more prey types in highlands. Although no sex difference in diet has been found, the trophic niche is broader in females than males. As a whole, alpine lizards show a trophic niche simitar to that found at lower elevat（ons, suggesting that P. algirus is well adapted to the harsh environment found in alpine areas.

The aim of the current study was to determine the nature of the seasonal variability of the Suspended Particulate Matter(SPM) fluxes from the drainage basin to the estuary in a macrotidal region(Northeastern Brazil),and the estuarine response to a seawater intrusion regarding sediment deposition,which will support the understanding of the global transport of materials at the continent-ocean *** structure data was acquired using a Conductivity,Temperature,and Depth(CTD) probe with a sampling frequency of 4 *** particulate material was measured by gravimetric measurements applied to exact filtered volume *** outflows were measured through the use of an Acoustic Doppler Current Profiler(ADCP) with frequency of 1.5 *** horizontal thermal and saline gradients varied from warmer and less saline waters(2014) to cooler and saline waters(2015).The gradient behavior when linked to volume transport and SPM flows,suggests a minimization of the fluvial flows in 2015,easing the advance of coastal water(CW) towards the inner estuary,leading to an inversion of the baroclinic pressure *** bottom saline front,generated by the entrance of coastal water masses,caused an increase in SPM concentrations due to increased fluid density,resuspension of previously deposited sediment,and erosion of *** concentrations of SPM indicate higher volume transport suggesting a hydraulic barrier due to the change/inversion of the baroclinic pressure gradient,resulting in water and material *** deposition was observed during neap tide,while during spring tide the material is resuspended,increasing the concentration,generating cycles of deposition and erosion during the neap-spring *** sediment in suspension that reach the estuary,even with low fluvial volume,stay in this environment forming new islands because of *** deposition rates or sediment cycling,if generated by the hydraulic barrier,may indicate that the flo

Social responsibility requires a set of duties and obligations, whether individuals or firms in relation to society and to the communities in which the organization is operating. The tradition of social intervention in Portugal dates from the fifteenth century and the founding of Mercy. However, environmental issues have never been felt in a sensitive position in responsible business, given the weakness of industrial development which lasted until the 60＇s. In Brazil, the corporate social responsibility is still particularly a big challenge, since the actions of social responsibility have not contributed effectively to improve the living conditions of society and the transformation of existing social reality. The purpose of this study is to emphasize the perspective of environmental sustainability that underpins the more general concept of social responsibility and, through the case study Caixa Geral de Dep6sitos, assess the strengths and weaknesses of the development of measures of social responsibility in this area in Portugal, and present the case of the company Vale, carrying out business in Brazil
