Objectives:The study aimed to the multicenter application of a nursing workload measurement scale in the internal medicine and surgery adults hospitalization ***:The study design was a multicenter,observational,and descriptive study.A multicenter application of the MIDEnF®nursing workload measurement scale was carried out,which consists of 21 items,and covers the four nursing functions(patient care items,teaching,manager,and researcher),in units of hospitalization of adults of internal medicine and surgery of four different *** item contains one or more of the nursing interventions of nursing Interventions Classification(nIC)and has an assigned time,after comparing the real time it takes to perform each intervention with the north American nursing Diagnosis Association(nAnDA)standardized *** study was carried out during nine months of the year 2020,measuring two days each month in the three work shifts(morning,evening,and night)to all patients admitted on the days of measurement in the indicated ***:The descriptive and inferential analysis of 11,756 completed scales,5,695 in general surgery and 6,061 in internal medicine,showed a greater care load for the two units during the morning shift(227,034 min in general surgery,261,835 min in internal medicine),especially in the items of“self-care”,“medication”,“common invasive procedures”,“fluid therapy”,and“patient and family support”,while the managerial function was similar during the three work shifts in the two units studied,getting values between 57,348 and 62,*** the analysis by shift and unit,statistical significance was obtained both for the total workload and the four nursing functions(Pnclusions:It is shown that the use of validated scales with the standardized language of nursing functions,adapted to the units,provides objective information to adjust the nursing staff to the real situation of care in any hospital and unit where it is applied,improving quality

AIM To demonstrate the non-inferiority(15% non-inferiority limit) of monotherapy with tenofovir disoproxil fumarate(TDF) vs the combination of lamivudine(LAM) plus adefovir dipivoxil(ADV) in the maintenance of virologic response in patients with chronic hepatitis B(CHB) and prior failure with *** This study was a Phase IV prospective, randomized, open, controlled study with 2 parallel groups(TDF and LAM+ADV) of adult patients with hepatitis B e antigen(HBe Ag)-negative CHB, prior failure with LAM, on treatment with LAM+ADV for at least 6 mo, without prior resistance to ADV and with an undetectable viral load at the start of the study, in 14 Spanish hospitals. The follow-up time for each patient was 48 wk after randomization, with quarterly visits in which the viral load, biochemical and serological parameters, adverse effects, adherence to treatment and consumption of hospital resources were *** Forty-six patients were evaluated [median age: 55.4 years(30.2-75.2); 84.8% male], including 22 patients with TDF and 24 with LAM+ADV. During study development, hepatitis B virus DnA(HBV-DnA) remained undetectable, all patients remained HBe Ag negative, and hepatitis B surface antigen(HBs Ag) positive. Alanine aminotransferase(ALT) values at the end of the study were similar in the 2 groups(25.1± 7.65, TDF vs 24.22 ± 8.38, LAM+ADV, P = 0.646). no significant changes were observed in creatinine or serum phosphorus values in either group. no significant differences between the 2 groups were noted in the identification of adverse effects(AEs)(53.8%, TDF vs 37.5%, LAM+ADV, P = 0.170), and none of the AEs which occurred were serious. Treatment adherence was 95.5% and 83.3% in the TDF and the LAM+ADV groups, respectively(P = 0.488). This paper,reports the performance of a wastewater treatment scale-pilot plant to treat 2 GPM(Gallons per Minute)discharges with 5,205 mg/L of pollutants expressed in COD(Chemical Oxygen Demand),from“Lomas de la Maestransa”a small community in Morelia City,Michoacan,*** scale-pilot plant is a train with(1)pretreatment with a triturated pump for floating solid,(2)primary treatment with“in line”coagulation,and rapid filtration to retain suspended colloids and dissolved solids higher of 5μm diameter,(3)double advanced oxidation as secondary treatment with ozone and heterogeneous photo catalysis to oxidize volatile solids,and(4)tertiary treatment with activated carbon to retain refractory *** performance was analyzed by a certified laboratory that belongs to Potable Water,Sewage and Sanitation Department from Morelia City *** show that treated water effluent complied with the Mexican Official Standard nOM-001-SEMARnAT-1996 for discharges into national waters,with exception of fecal coliforms,since the raw water contains an average of 64,228,351 MnP/100 mL of fecal coliforms,and in spite that we obtained a 99.998%efficiency,the maximum level allowable 2,000 MPn/100 mL standard,was exceeded by 400 MPn/100 *** this experience,the wastewater treatment plant is equipped with a residual chlorine tank to keep a 1.5 ppm chlorine residual concentration to keep the treated water *** project was possible because we had the support of the Morelia Sanitation Department.

Helicobacter pylori (H. pylori) affects nearly half of the world’s population and, thus, is one of the most frequent and persistent bacterial infections worldwide. H. pylori is associated with peptic ulcer disease, gastric ulcers, mucosa-associated lymphoid tissue lymphoma, and gastric cancer. Various diagnostic methods exist to detect infection, and the choice of one method or another depends on several factors, such as accessibility, advantages and disadvantages of each method, cost, and the age of patients. Once H. pylori infection is diagnosed, the clinician decides whether treatment is necessity, according to the patient’s clinical condition. Typically, eradication of H. pylori is recommended for treatment and prevention of the infection. Cure rates with the standard triple therapy are acceptable, and effective quadruple therapies, sequential therapies, and concomitant therapies have been introduced as key alternatives to treat H. pylori infection. In this work, we review the main diagnostic methods used to identify H. pylori infection and to confirm eradication of infection. In addition, key factors related to treatment are reviewed.

This review focuses on the laparoscopic approach to gastrointestinal emergencies and its more recent indications. Laparoscopic surgery has a specific place in elective procedures, but that does not apply in emergency situations. In specific emergencies, there is a huge range of indications and different techniques to apply, and not all of them are equally settle. We consider that the most controversial points in minimally invasive procedures are indications in emergency situations due to technical difficulties. Some pathologies, such as oesophageal emergencies, obstruction due to colon cancer, abdominal hernias or incarcerated postsurgical hernias, are nearly always resolved by conventional surgery, that is, an open approach due to limited intraabdominal cavity space or due to the vulnerability of the bowel. These technical problems have been solved in many diseases, such as for perforated peptic ulcer or acute appendectomy for which a laparoscopic approach has become a wellknown and globally supported procedure. On the other hand, endoscopic procedures have acquired further indications, relegating surgical solutions to a second place; this happens in cholangitis or pancreatic abscess drainage. This endoluminal approach avoids the need for laparoscopic development in these diseases. nevertheless, new instruments and new technologies could extend the laparoscopic approach to a broader array of potentials procedures. There remains, however, a long way to go.

MicroRnAs(miRnAs) are small non-coding RnAs that regulate multiple physiological and pathological functions through the modulation of gene expression at the post-transcriptional level. Accumulating evidence has established a role for miRnAs in the development and pathogenesis of liver disease. Specifically, a large number of studies have assessed the role of miRnAsin alcoholic liver disease(ALD) and non-alcoholic fatty liver disease(nAFLD), two diseases that share common underlying mechanisms and pathological characteristics. The purpose of the current review is to summarize and update the body of literature investigating the role of miRnAs in liver disease. In addition, the potential use of miRnAs as biomarkers and/or therapeutic targets is discussed. Among all miRnAs analyzed, miR-34 a, miR-122 and miR-155 are most involved in the pathogenesis of nAFLD. Of note, these three miRnAs have also been implicated in ALD, reinforcing a common disease mechanism between these two entities and the pleiotropic effects of specific miRnAs. Currently, no single miRnA or panel of miRnAs has been identified for the detection of, or staging of ALD or nAFLD. While promising results have been shown in murine models, no therapeutic based-miRnA agents have been developed for use in humans with liver disease.
