Perfect lenses,superlenses and time-reversal mirrors can support and spatially separate evanescent waves,which is the basis for detecting subwavelength information in the far ***,the inherent limitations of these methods have prevented the development of systems to dynamically distinguish subdiffraction-limited *** the physical merits of spoof surface plasmon polaritons(SPPs),we demonstrate that subdiffraction-limited signals can be transmitted on planar integrated SPP channels with low loss,low channel interference,and high gain and can be radiated with a very low environmental ***,we show how deep subdiffraction-limited signals that are spatially coupled can be distinguished after line-of-sight wireless *** a visualized demonstration,we realize the high-quality wireless communication of two movies on subwavelength channels over the line of sight in real time using our plasmonic scheme,showing significant advantages over the conventional methods.

Nineteen multiparous barren Holstein cows were subjected to an induction of lactation protocol for 21 d administering estradiol cypionate （2 mg kg-1 of body weight （BW） d-1, on day 1 to 14）, progesterone （0.10 mg kg-1 of BW, on day 1 to 7）, lfumethasone （0.03 mg kg-1 of BW, on day 18 to 20） and recombinant bovine somatotropin （rbST;500 mg per cow, on day 1, 6, 16 and 21）. At the end of lactation and with a minimum of a 2-mon dry period, the same cows were again hormonally induced into lactation. Cows in both lactations were not artiifcially inseminated, they were milked 3 times daily and received rbST throughout lactation. Mean accumulated milk yield at 305 d in milk （DIM） did not differ between the ifrst and second induced lactations （（9 710 ＆#177;1 728） vs. （9 309＆#177;2 150） kg;mean＆#177;SD）. Total milk yield （（12 707＆#177;3 406） vs. （12 306＆#177;4 218） kg;mean＆#177;SD） and lactation length （（405＆#177;100） vs. （410＆#177;91） d;mean＆#177;SD） were not different between the ifrst and second induced lactations. In a second study, 15 empirical models including exponential, power law, yield-density, sigmoidal and miscellaneous models were compared for their suitability by modeling 12-mon （n=334）, 18-mon （n=164） and 29-mon （n=22） lactation cycles of Holsteins cows induced into lactation and treated with rbST throughout the lactation. Hoerl （Y=ab1/xxc）, Wood （Y=axb exp（cx）） and Dhanoa （Y=ax（bc）exp（cx）） models were equally suitable to describe 12-mon lactations. An exponential model with ifve parameters （Y=exp（a＋bx＋cd2＋e/x）） showed the best ift for milk yield for 18-mon lactations. The rational model （Y=a＋bx/1＋cx＋dx2） was found to produce the closest ift for 29-mon lactations. It was concluded that, with the protocol used in the present study, multiparous cows respond favorably to a second cycle of induced lactation, with milk yield similar to that experienced during the ifrst cycle. Tin(Sn)holds great promise as an anode material for next-generation lithium(Li)ion batteries but suffers from massive volume change and poor cycling *** clarify the dynamic chemical and microstructural evolution of Sn anode during lithiation and delithiation,synchrotron X-ray energydispersive diffraction and X-ray tomography are simultaneously employed during Li/Sn cell *** intermediate Li-Sn alloy phases during de/lithiation are identified,and their dynamic phase transformation is unraveled which is further correlated with the volume variation of the Sn at particle-and ***,we find that the Sn particle expansion/shrinkage induced particle displacement is anisotropic:the displacement perpendicular to the electrode surface(z-axis)is more pronounced compared to the directions(x-and y-axis)along the electrode *** anisotropic particle displacement leads to an anisotropic volume variation at the electrode level and eventually generates a net electrode expansion towards the separator after cycling,which could be one of the root causes of mechanical detachment and delamination of electrodes during long-term *** unraveled chemical evolution of Li-Sn and deep insights into the microstructural evolution of Sn anode provided here could guide future design and engineering of Sn and other alloy anodes for high energy density Li-and Na-ion batteries.

The aim of this study is to assess the results of episcleral brachytherapy as treatment of retinal vasoproliferative tumors(RVTs) in a referral Intraocular Tumors Unit(ITU). A retrospective review of all patients diagnosed with vasoproliferative tumors of the retina and treated with episcleral brachytherapy in the ITU, University Hospital of Valladolid between 2009 and 2015 was done. Five patients accomplished the inclusion criteria. All of them presented associated exudation and secondary retinal detachments(RD). Four patients had received prior treatments. Decreased tumor size and exudation regression was found in all cases after treatment. Visual acuity remained stable or increased in all patients. No recurrences have been found after twelve-months follow up. The results of the present study suggest that episcleral brachytherapy is an efficient and safe option in the management of vasoproliferative tumors, especially when large tumor or extensive subretinal fluid is present. In these cases episcleral brachytherapy could be considered as a first line treatment.

AIM:To determine if proton pump inhibitor use in cirrhotic patients with endoscopic findings of portal hypertension is associated with a lower frequency of gastrointestinal ***:Patients with cirrhosis and endoscopic findings related to portal hypertension,receiving or not receiving proton pump inhibitor (PPI) therapy,were included *** assigned patients to two groups:group 1 patients underwent PPI therapy and group 2 patients did not undergo PPI ***:One hundred and five patients with a median age of 58 (26-87) years were included,57 (54.3%) of which were *** varices were found in 82 (78%) patients,portal hypertensive gastropathy in 72 (68.6%) patients,and gastric varices in 15 (14.3%) *** therapy was used in 45.5% of patients (n=48).Seventeen (16.1%) patients presented with upper gastrointestinal bleeding;in 14/17 (82.3%) patients,bleeding was secondary to esophageal varices,and in 3/17 patients bleeding was attributed to portal hypertensive *** related to portal hypertension according to PPI therapy occurred in 18.7% (n=9) of group 1 and in 14% (n=8) of group 2 (odds ratio:0.83,95% confidence interval:0.5-1.3,P=0.51).CONCLUSION:Portal hypertension bleeding is not associated with PPI *** findings do not support the prescription of PPIs in patients with chronic liver disease with no currently accepted indication.

Purpose:The purpose of this study was to examine the reliability and the learning effect of an isokinetic trunk flexion-extension protocol designed to simultaneously assess trunk muscle strength and *** addition,the effect of the participants’sex on the reliability data was ***:Fifty-seven healthy and physically active young men(n=28)and women(n=29)performed the isokinetic protocol 5 times,separated by a week between each of the first 4 sessions and by a month between the last 2 *** protocol consisted of performing 4 trials of 15 maximum flexion-extension concentric exertions at 120°/s(range of trunk motion=50°).The absolute and relative peak torque and total work were calculated to assess trunk flexion and extension *** addition,endurance ratio,modified endurance ratio,fatigue final ratio,recovery ratio,and modified recovery ratio variables were used for the assessment of trunk muscle endurance in both ***:Regarding the absolute reliability,no relevant changes were found between paired-comparison sessions for most strength and endurance variables,except for total work and relative total work variables in the flexion movement in both *** addition,the typical error of the isokinetic variables was lower than 10%in both males and females,and minimum detectable changes ranged from 7%to 20%,with a tendency to be higher in females and in endurance *** strength variables showed high-to-excellent intraclass correlation coefficients(ICCs;>0.74);however,for the endurance variables only the endurance ratio and the modified endurance ratio obtained moderate-to-high ICC values(0.57

Miliary tuberculosis infection during hepatitis C treatment with sofosbuvir and ledipasvir plus ribavirin

Chronic hepatitis C virus(HCV) infection is one of the main causes of chronic liver disease worldwide. In the last 5 years, treatment for HCV infection has experienced a marked development. In 2014, the use of ledipasvir/sofosbuvir with or without concomitant weight-based ribavirin was approved with a very significant increase in the sustained virological response. However, new side effects have been associated. We report the first case of an HCV infected patient treated for 12 wk with the combination of sofosbuvir/ledipasvir plus ribavirin who developed a miliary tuberculosis(TB) infection while on therapy. The patient was a 65-year-old woman, who referred malaise, asthenia, hyporexia, 7 kg weight loss, productive cough, evening fever and night sweats, right after finishing the treatment. The chest computed tomography-scan revealed a superior mediastinal widening secondary to numerous lymphadenopathies with extensive necrosis and bilateral diffuse lung miliary pattern with little subsequent bilateral pleural effusion, highly suggestive of lymph node tuberculosis with lung miliary spread. A bronchoscopy was performed and bronchial suction showed more than 50 acid-alcohol resistant bacillus per line. A Mycobacterium tuberculosis DNA was detected in blood by polymerase chain reaction, which confirmed the diagnosis of miliary tuberculosis. Some cases of TB infection have been identified with α-interferon-based therapy and with the triple therapy of pegylated interferon, ribavirin and boceprevir or telaprevir. However, significant infection has not been reported with sofosbuvir/ledipasvir plus *** believe that the case is relevant to increase awareness of opportunistic infections and particularly TB infection. Although the international guidelines offer no recommendation regarding TB screening, we wonder whether it would be advisable to screen for opportunistic infections prior to the introduction of HCV therapy.

Background End-of-life care is not usually a priority in cardiology departments. We sought to evaluate the changes in end-of-life care after the introduction of a do-not-resuscitate （DNR） order protocol. Methods ＆ Results Retrospective analysis of all deaths in a cardiology department in two periods, before and after the introduction of the protocol. Comparison of demographic characteristics, use of DNR orders, and end-of-life care issues between both periods, according to the presence in the second period of the new DNR sheet （Group A）, a conven- tional DNR order （Group B） or the absence of any DNR order （Group C）. The number of deaths was similar in both periods （n = 198 vs. n = 197）. The rate of patients dying with a DNR order increased significantly （57.1% vs. 68.5%; P = 0.02）. Only 4% of patients in both periods were aware of the decision taken about cardiopulmonary resuscitation. Patients in Group A received the DNR order one day earlier, and 24.5% received it within the first 24 h of admission （vs. 2.6% in the first period; P 〈 0.001）. All patients in Group A with an implantable cardioverter defibrillator （ICD） had shock therapies deactivated （vs. 25.0% in the first period; P = 0.02）. Conclusions The introduction of a DNR order protocol may improve end-of-life care in cardiac patients by increasing the use and shortening the time of registration of DNR orders. It may also contribute to increase ICD deactivation in patients with these orders in place. However, the introduction of the sheet in late stages of the disease failed to improve patient participation.
