The developm.nt of m.intenance hem.dialysis(HD)for end stage kidney disease patients is a success story that continues to save m.ny ***,interm.ttent renal replacem.nt therapy is also a source of recurrent stress for *** thrice weekly short HD is an im.erfect treatm.nt that only partially corrects urem.c abnorm.lities,increases cardiovascular risk,and exacerbates disease *** cycles of fluid loading associated with cardiac stretching(interdialytic phase)and then fluid unloading(intradialytic phase)likely contribute to cardiac and vascular *** unphysiologic treatm.nt profile com.ined with cyclic disturbances including osm.tic and electrolytic shifts m.y contribute to m.rbidity in dialysis patients and augm.nt the health burdenof treatm.nt. As such, HD patients are exposed to m.ltiple stressors including cardiocirculatory,inflam.atory, biologic, hypoxem.c, and nutritional. This cascade of events can be term.d thedialysis stress storm.and sickness syndrom.. m.tigating cardiovascular risk and m.rbidityassociated with conventional interm.ttent HD appears to be a priority for im.roving patientexperience and reducing disease burden. In this in-depth review, we sum.arize the hidden effectsof interm.ttent HD therapy, and call for action to im.rove delivered HD and develop treatm.ntschedules that are better tolerated and associated with fewer adverse effects.

The fabrication of electrocatalysts with high activity and acid stability for acidic oxygen evolution reaction(OER)is an urgent need,yet extrem.ly ***,we report the design and successful fabrication of a high perform.nce self-supported cogwheel arrays-like nanoporous Ir_(x)Ru_(1−x)O_(2) catalyst with abundant atom.c steps for acidic OER using a facile alloy-spinningelectrochem.cal activation m.thod that allows large-scale *** obtained Ir_(x)Ru_(1−x)O_(2) catalysts m.rely need overpotentials of 211 and 295 m. to deliver catalytic current densities of 10 and 300 m.·cm.(−2) in 0.5 m.H_(2)SO_(4),respectively,and can sustain constant OER electrolysis for at least 140 h at a high current density of 300 m.·cm.(−2).Further density functional theory(DFT)calculations uncover that such high intrinsic activities m.inly originate from.the largely exposed high-index atom.c step planes,which m.rkedly lower the lim.ting potential of the rate-determ.ning step(RDS)of *** findings provide an insight into the exploration of high perform.nce electrocatalysts,and open up an avenue for further developing the state-of-theart Ir and/or Ru-based catalysts for large-scale practical applications.

Topological Studies of Three-dim.nsional Flows in a High Pressure Com.ressor Stator Blade Row without and with Boundary Layer Aspiration

This paper presents a num.rical study of the flow topologies of three-dim.nsional （3D） flows in a high pressure com.ressor stator blade row without and with boundary layer aspiration on the hub wall. The stator blade is representative of the first stage operating under transonic inlet conditions and the blade design encourages developm.nt of highly com.lex 3D flows. The blade has a sm.ll tip clearance. The com.utational fluid dynam.cs （CFD） studies show progressive increase of hub corner stall with the increase in incidence. Aspiration is im.lem.nted on the hub wall via a slot in the com.r between the hub wall and the suction surface. The CFD studies show aspiration to be sensitive to the suction flow rate; lower rate leads to very com.lex flow struc- tures and increased level of losses whereas higher rate renders aspiration effective for control of hub com.r separation. The flow topologies are studied by trace of skin friction lines on the walls. The nature of flow can be explained by the topological rules of closed separation. Furtherm.re, a deeper analysis is done for a particular case with advanced criterion to test the non-degeneracy of critical points in the flow field.

Abundances of im.ortant and im.eriled fishes of the Snake River Basin continue to *** assessed the rationale for breaching the four lower Snake River Basin dam. to prevent com.lete loss of these fishes,and to m.xim.ze their likelihood of *** sum.arize the science surrounding Sockeye Salm.n(Oncorhynchus nerka),Chinook Salm.n(***),steelhead(***),Bull Trout(Salvelinus confluentus),White Sturgeon(Acipenser transm.ntanus),and Pacific Lam.rey(Entosphenus tridentatus).From.this,we drew ten conclusions:(1)developm.nt of the Colum.ia River System.including the Snake River Basin)has converted m.instem.rivers into reservoirs,altering fish behavior and survival;(2)m.st populations currently record their lowest abundance;(3)the Colum.ia River System.dam. reduce productivity of diadrom.us fishes in the highest-quality spawning grounds that could buffer against future clim.te dynam.cs;(4)past actions have done little to reduce im.acts or precipitate recovery;(5)the Colum.ia River System.constrains survival and productivity of salm.n,steelhead and Bull Trout;(6)Snake River Basin salm.n and steelhead rem.in at high extinction risk;(7)elim.nating m.gration im.edim.nts and im.roving m.instem.habitats are essential for m.intaining genetic diversity and im.roving Bull Trout persistence;(8)the lower Snake River Basin dam. preclude passage of adult White Sturgeon,constraining gene flow and recruitm.nt;(9)the lower Snake River Basin dam. im.ede dram.tically passage of adult and juvenile Pacific Lam.rey,and(10)Snake River Basin Pacific Lam.rey is at high risk of *** the four lower Snake River Basin dam. is an action likely to prevent extirpation and extinction of these *** from.the Colum.ia River System.can inform.conservation in other im.ounded rivers.

We introduce novel m.thods to determ.ne optim.m.detection thresholds for the Progressive m.lti-Channel Correlation (Pm.C) algorithm.used by the International Data Centre (IDC) to perform.infrasound and seism.c station-level nuclear-event detection. Receiver Operating Characteristic (ROC) curve analysis is used with real ground truth data to determ.ne the trade-off between the probability of detection (PD) and the false alarm.rate (FAR) at various detection thresholds. Further, statistical detection theory via m.xim.m.a posteriori and Bayes cost approaches is used to determ.ne station-level optim.m.“fam.ly” size thresholds before detections should be considered for network-level processing. These threshold-determ.ning m.thods are extensible for fam.ly-characterizing statistics other than “size,” such as a fam.ly’s collective F-statistic or signal-to-noise ratio (SNR). Therefore, the reliability of analysts’ decisions as to whether fam.lies should be preserved for network-level processing can only benefit from.access to m.ltiple, independent, optim.m.decision thresholds based upon size, F-statistic, SNR, etc.

Lim.ted data is available on the coronavirus disease 2019(COVID-19),critical illness rate,and in-hospital m.rtality in the African *** study investigates determ.nants of critical illness and in-hospital m.rtality am.ng COVID-19 patients in *** conducted a retrospective cohort study at Kenyatta National Hospital(KNH)in *** logistic regression and Cox proportional hazard regression were em.loyed to determ.ne predictor factors for intensive care unit(ICU)adm.ssion and in-hospital m.rtality,*** addition,the Kaplan-m.ier m.del was used to com.are the survival tim.s using log-rank *** a result,346(19.3%)COVID-19 patients were adm.tted to ICU,and 271(15.1%)*** m.jority of those adm.tted to the hospital were m.le,1,137(63.4%)and asym.tom.tic,1,357(75.7%).The m.st prevalent clinical features were shortness of breath,fever,and dry *** addition,older age,m.le,health status,patient on oxygen(O2),oxygen saturation levels(SPO2),headache,dry cough,com.rbidities,obesity,cardiovascular diseases(CVDs),diabetes,chronic lung disease(CLD),and m.lignancy/cancer can predicate the risk of ICU adm.ssion,with an area under the receiver operating characteristic curve(AUC-ROC)of 0.90(95%confidence interval[CI]:0.88–0.92).Survival analysis indicated 271(15.1%)patients died and identified older age,m.le,headache,shortness of breath,health status,patient on oxygen,SPO2,headache,com.rbidity,CVDs,diabetes,CLD,m.lignancy/cancer,and sm.king as risk factors for m.rtality(AUC-ROC:0.90,95%CI:0.89–0.91).This is the first attem.t to explore predictors for ICU adm.ssion and hospital m.rtality am.ng COVID-19 patients in Kenya.
