Microbiome in the setting of burn patients:implications for infections and clinical outcomes

Burn damage can lead to a state of immune dysregulation that facilitates the development of infections in *** most deleterious impact of this dysfunction is the loss of the skin’s natural protective ***,the risk of infection is exacerbated by protracted hospitalization,urinary catheters,endotracheal intubation,inhalation injury,arterial lines and central venous access,among other mainstays of burn ***,infections comprise the leading cause of mortality after major burn injuries,which highlights the improvements observed over the last 50 years in the care provided to burn *** need to implement the empirical selection of antibiotic therapy to treat multidrug-resistant bacteria may concomitantly lead to an overall pervasiveness of difficult-to-treat pathogens in burn centres,as well as the propagation of antimicrobial resistance and the ultimate dysregulation of a healthy *** preliminary studies are examining the variability and evolution of human and mice microbiota,both during the early and late phase burn injury,one must consider that abnormal microbiome conditions could influence the systemic inflammatory response.A better understanding of the changes in the postburn microbiome might be useful to interpret the provenance and subsequent development of infections,as well as to come up with inferences on the prognosis of burn *** review aims to summarise the current findings describing the microbiological changes in different organs and systems of burn patients and how these alterations affect the risks of infections,complications,and,ultimately,healing.

Interaction of major genes predisposing to hepatocellular carcinoma with genes encoding signal transduction pathways influences tumor phenotype and prognosis

Studies on rodents and humans demonstrate an inherited predisposition to hepatocellular carcinoma （HCC）. Analysis of the molecular alterations involved in the acquisition of a phenotype resistant or susceptible to hepatocarcinogenesis showed a deregulation of g1 and S phases in HCC of genetically susceptible F344 rats and a g1-S block in lesions of resistant Brown norway （BN） rats. Unrestrained extracellular signal-regulated kinase （ERK） activity linked to proteasomal degradation of dual-specificity phosphatase 1 （DUSP1）, a specific ERK inhibitor, by the CKS1-SKP2 ubiquitin ligase complex occurs in more aggressive HCC of F344 rats and humans. This mechanism is less active in HCC of BN rats and human HCC with better prognosis. Upregulation of iNos cross-talk with IKK/NF-KB and RAS/ERK pathways occurs in rodent liver lesions at higher levels in the most aggressive models represented by HCC of F344 rats and c-Myc-TgF-α transgenic mice. iNOS, IKK/NF-κB, and RAS/ERK upregulation is highest in human HCC with a poorer prognosis and positively correlates with tumor proliferation, genomic instability and microvascularization, and negatively with apoptosis. Thus, cell cycle regulation and the activity of signal transduction pathways seem to be modulated by HCC modifier genes, and differences in their efficiency influence the susceptibility to hepatocarcinogenesis and probably the prognosis of human HCC.

grain free diets for utility dogs during training work: Evaluation of the nutrient digestibility and faecal characteristics

Two different diets characterized by the absence of cereals or by the presence of conventional cereals were evaluated on the nutrient digestibility and faecal characteristics and faecal fermentative endproduct concentrations of 8 neutered adult Labrador retrievers housed at the Regional Centre Helen Keller(Messina,Italy)during the training work for the service guide for the ***(age = 17 ± 1 months,initial body weight [BW]= 26.3 ± 1 *** body condition score [BCS]= 4.5 ± 0.11)were divided into 2 homogeneous groups for sex(half males and half females).Dogs in the grain free(gF)group were fed a commercial diet characterized by the absence of grain cereals,and dogs in the control(CTR)group were fed a super-premium pet food characterized by conventional grains as the carbohydrate *** trial lasted 84 *** by a 7-d of adaption *** examination,*** faecal characteristics were *** statistical model included the effects of diet(gF ***),time(from d 0 to *** of the trial)and the interaction(diet x time).The high-protein,lowcarbohydrate dry diet(gF)offered higher apparent nutrient digestibility of protein(+10%;P =0.002)and fat(+7%;Pge intestinal fermentation of carbohydrate compared to the commercial high-carbohydrate dry diet,enabling dogs to use nutrients from the diet more efficiently and thus requiring less food(-13%)to satisfy their nutrient requirements,producing less excrement(-33%;P= 0.033).and reaching a higher final BW(+8%;Pgher final BCS(+15%;P= 0.003).Therefore,the gF diet appears the nutritional plan most suitable for these animals taking due account not only of the training work done by animals with their increased nutrient and energy needs,but also of the gastrointestinal disorders consequent to stress coming from work and life in kennels,which cause in the Labrador retrievers an unusual weight loss.

Lack of association between UgT1A7,UgT1A9,ARP,SPINK1 and CFTR gene polymorphisms and pancreatic cancer in Italian patients

AIM： To investigate simultaneously UgT1A7, UgT1A9, ARP, SPINK and CFTR genes to verify whether genetic polymorphisms predispose to the development of pancreatic cancer （PC）.METHODS： genomic DNA of 61 pancreatic cancer patients and 105 healthy controls （HC） were analyzed. UgT1A7 genotyping was determined by PCR-RFLP analysis. Specific PCR and sequencing were used to analyze genetic variants of UgT1A9, ARP, SPINK1 and CFTR genes.RESULTS： Four different alleles （^＊1：WT; ＂2： N129K and R131K; ＂3： N129K, R131K, and W208R; and ＂4： W208R） in UgT1A7 and three different alleles （^＊1：WT;^＊4： Y242X; and ^＊5： D256N） in UgT1A9 were detected. All UgT1A polymorphisms were observed at similar frequency in PC patients and HC. Seven different alleles in ARP were found in PC patients and HC at similar frequency. The SPINK1 mutations N34S and P55S occurred in five PC patients with a prevalence （4.1%） not significantly different from that observed （2.0%） in HC. The only CFTR AFS08 mutation was recognized in three PC patients with a prevalence （4.9%） similar to HC.CONCLUSION： UgT1A7, UgT1A9, ARP, SPINK1 and CFTR gene polymorphisms are not associated with PC in Italian patients.

Diffusion-weighted imaging and fluid-attenuated inversion recovery sequence in sporadic Creutzfeldt-Jakob disease:One-case report

The diagnosis of sporadic Creutzfeldt-Jakob disease （sCJD） is extremely difficult. Diffusion-weighted imaging has been shown to be the most sensitive technique for the detection of signal alterations in sCJD patients. The present study analyzed the diagnostic value of diffusion-weighted imaging and fluid-attenuated inversion recovery sequence in the early stage of sCJD in one female patient and correlated the clinical symptoms during disease course and magnetic resonance manifestations. Thalamic and basal ganglia hyperintensities were observed on magnetic resonance images in a very early stage, i.e., when the clinical typical manifestations of the disease were not present. With the progression of the disease, cortical and basal ganglia hyperintensities were observed on magnetic resonance images, showing an obvious cerebral atrophy. These findings suggest that diffusion-weighted imaging and fluid-attenuated inversion recovery sequence are helpful in diagnosing sCJD.

In this study, the effects of Radio Electric Asymmetric Conveyer(REAC), a non-invasive physical treatment, on neuroinflammatory responses in a mouse model of parkinsonism induced by intoxication with1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine(MPTP),were investigated in vivo. We found that the REAC tissue optimization treatment specific for neuro-regenerative purposes(REAC TO-RgN-N) attenuated the inflammatory picture evoked by MPTP-induced nigro-striatal damage inmice, decreasing the levels of pro-inflammatory molecules and increasing anti-inflammatory mediators. Besides, there was a significant reduction of both astrocyte and microglial activation in MPTP-treated mice exposed to REAC TORgN-N. These results indicated that REAC TO-RgN-N treatment modulates the pro-inflammatory responses and reduces neuronal damage in MPTP-induced parkinsonism.

Comparison of a monoclonal antigen stool test (Hp StAR) with the ^(13)C-urea breath test in monitoring Helicobacter pylori eradication therapy

AIM： To evaluate the agreement between a mAb-based stool test （HP STAR） and the urea breath test （UBT） in monitoring （H pylon） infection after eradication therapy. METHODS： Patients with discordant results on UBT and Hp STAR underwent endoscopy with biopsies for rapid urease test, culture, and histology to confirm H pylori status. RESULTS： Among 250 patients （50±14 years）, 240 （96.0%） had concordant UBT and Hp STAR tests with a significant correlation between DOB and A values （R = 0.87; P〈0.0001）. The remaining 10 （4.0%） patients had discordant tests （positive Hp STAR and negative UBT） with the Hp STAR inaccurate in five cases （false positive） and UBT inaccurate in the other five cases （false negative）. The “maximal expected” sensitivity, specificity, ＋PV, -PV, ＋LR, and -LR were 91%, 100%, 100%, 97.4%, ∞, and 8.2 respectively, for the UBT, and 100%, 97.4%, 91%, 100%, 38.8, and 0, respectively, for the Hp STAR. Overall accuracy for both tests was 98%. CONCLUSION： Both the UBT and the Hp StAR are equally accurate in monitoring H pylori infection. Nowadays, the choice of the “best” non-invasive H pylori test in the post-treatment setting should be done not only in terms of diagnostic accuracy but also in view of cost and local facilities.
