BACKGROUND There is still considerable heterogeneity regarding which features of cryptoglandular anal fistula on magnetic resonance imaging(MRI)and endoanal ultrasound(EAUS)are relevant to surgical *** a con-sequence,the quality and completeness of the report are highly dependent on the training and experience of the *** To develop a structured MRI and EAUS template(SMART)reporting the minimum dataset of information for the treatment of anal *** This modified Delphi survey based on the RAND-UCLA appropriateness for consensus-building was conducted between May and August *** hundred and fifty-one articles selected from a systematic review of the lite-rature formed the database to generate the evidence-based statements for the Delphi *** questions were anonymously voted by an interdisciplinary multidisciplinary group for a maximum of three iterative *** degree of agreement was scored on a numeric 0–10 *** consensus was defined as a score≥8 for≥80%of the *** Eleven scientific societies(3 radiological and 8 surgical)endorsed the *** three rounds of voting,the experts(69 colorectal surgeons,23 radiologists,2 anatomists,and 1 gastroenterologist)achieved consensus for 12 of 14 statements(85.7%).Based on the results of the Delphi process,the six following features of anal fistulas were included in the SMART:Primary tract,secondary extension,internal opening,presence of collection,coexisting le-sions,and sphincters *** A structured template,SMART,was developed to standardize imaging reporting of fistula-in-ano in a simple,systematic,time-efficient way,providing the minimum dataset of information and visual diagram useful to refer-ring physicians.

Magnetic resonance imaging:A new tool for diagnosis of acute ischemic colitis?

AIM： To define the evolution of ischemic lesions with 7T magnetic resonance imaging （7T-MRI） in an animal model of acute colonic ischemia. METHODS： Adult Sprague-Dawley rats were divided into two groups. Group I underwent inferior mesenteric artery （IMA） ligation followed by macroscopic observa- tions and histological analysis. In group H, 7T-MRI was performed before and after IMA ligation and followed by histological analysis. RESULTS： Morphological alterations started to develop 1 h after IMA ligation, when pale areas became evident in the splenic flexure mesentery and progressively wors-ened up to 8 h thereafter, when the mesentery was less pale, and the splenic flexure loop appeared very dark. The 7T-MRI results reflected these alterations, showing a hyperintense signal in both the intraperitoneal space and the colonic loop wall 1 h after IMA ligation; the lat- ter progressively increased to demonstrate a reduction in the colonic loop lumen at 6 h. Eight hours after IMA ligation, MRI showed a persistent colonic mural hyper- intensity associated with a reduction in peritoneal free fluid. The 7T-MRI findings were correlated with histolog- ical alterations, varying from an attenuated epithelium with glandular apex lesions at 1 h to coagulative necro- sis and loss of the surface epithelium detected 8 h after IMA ligation. CONCLUSION： MRI may be used as a substitute for invasive procedures in diagnosing and grading acute ischemic colitis, allowing for the early identification of pathological findings.

Investigation of compensatory postures with videofluoromanometry in dysphagia patients

AIM: To investigate the effectiveness of head compensatory postures to ensure safe oropharyngeal transit. METHODS: A total of 321 dysphagia patients were enrolled and assessed with videofluoromanometry (VFM). The dysphagia patients were classified as follows: safe transit; penetration without aspiration; aspiration before, during or after swallowing; multiple aspirations and no transit. The patients with aspiration or no transit were tested with VFM to determine whether compensatory postures could correct their swallowing disorder. RESULTS: VFM revealed penetration without aspiration in 71 patients (22.1%); aspiration before swallowing in 17 patients (5.3%); aspiration during swallowing in 32 patients (10%); aspiration after swallowing in 21 patients (6.5%); multiple aspirations in six patients (1.9%); no transit in five patients (1.6%); and safe transit in 169 patients (52.6%). Compensatory postures guaranteed a safe transit in 66/75 (88%) patients with aspiration or no transit. A chin-down posture achieved a safe swallow in 42/75 (56%) patients, a head-turned posture in 19/75 (25.3%) and a hyperextended head posture in 5/75 (6.7%). The compensatory postures were not effective in 9/75 (12%) cases. CONCLUSION: VFM allows the speech-language therapist to choose the most effective compensatory posture without a trial-and-error process and check the effectiveness of the posture.

The precision livestock farming(PLF)has the objective to maximize each animal's performance while reducing the environmental impact and maintaining the quality and safety of meat *** the PLF techniques,the personalised management of each individual animal based on sensors systems,represents a viable *** is worth noting that the implementation of an effective PLF approach can be still expensive,especially for small and medium-sized farms;for this reason,to guarantee the sustainability of a customized livestock management system and encourage its use,plug and play and cost-effective systems are *** this context,we present a novel low-cost method for identifying beef cattle and recognizing their basic activities by a single surveillance *** leveraging the current state-of-the-art methods for real-time object detection,(i.e.,YOLOv3)cattle's face areas,we propose a novel mechanism able to detect the ear tag as well as the water ingestion state when the cattle is close to the *** cow IDs are read by an Optical Character Recognition(OCR)algorithm for which,an ad hoc error correction algorithm is here presented to avoid numbers misreading and correctly match the IDs to only actually present *** to the detection of the tag position,the OCR algorithm can be applied only to a specific region of interest reducing the computational cost and the time *** times for the areas are outputted as cattle activity recognition *** results demonstrate the effectiveness of our proposed method,showing a mAP@0.50 of 89%.

Genome-resolved metagenomics of Venice Lagoon surface sediment bacteria reveals high biosynthetic potential and metabolic plasticity as successful strategies in an impacted environment

Bacteria living in sediments play essential roles in marine ecosystems and deeper insights into the ecology and biogeochemistry of these largely unexplored organisms can be obtained from‘omics’***,we characterized metagenome-assembled-genomes(MAGs)from the surface sediment microbes of the Venice Lagoon(northern Adriatic Sea)in distinct sub-basins exposed to various natural and anthropogenic *** were explored for biodiversity,major marine metabolic processes,anthropogenic activity-related functions,adaptations at the microscale,and biosynthetic gene *** from 126 MAGs,a non-redundant dataset of 58 was compiled,the majority of which(35)belonged to(Alpha-and Gamma-)*** the broad microbial metabolic repertoire(including C,N,and S metabolisms)the potential to live without oxygen emerged as one of the most important *** was also found as a successful *** analysis showed that different MAGs encoded the same metabolic patterns(e.g.,C fixation,sulfate oxidation)thus suggesting metabolic *** and toxic compounds resistance genes were coupled,a condition that could promote the spreading of these genetic *** showed a high biosynthetic potential related to antimicrobial and biotechnological classes and to organism defense and interactions as well as adaptive strategies for micronutrient uptake and cellular *** results highlighted that bacteria living in an impacted environment,such as the surface sediments of the Venice Lagoon,may benefit from metabolic plasticity as well as from the synthesis of a wide array of secondary metabolites,promoting ecosystem resilience and stability toward environmental pressures.
