Lower Limb Arteriopathy in Diabetics at the Oueme-Plateau Provincial Hospital in Porto-Novo: Frequency and Associated Factors

Introduction: The distribution of arteriopathy of the lower limbs (ALL) is not familiar in the city of Porto-Novo. The aim of this study was to determine the prevalence and associated factors of ALL in diabetic patients in treatment at the Oueme-Plateau Provincial Teaching Hospital. Methods: This was a cross-sectional, descriptive, and analytical study from July 12 to October 10, 2021, at the Oueme-Plateau Provincial Teaching Hospital in Porto-Novo. ALL was screened by performing systolic pressure index (SPI) in all diabetic patients aged at least 18 years, seen in consultation or hospitalization during the study period and who had given their consent. Results: The study registered 165 diabetics and 98.2% of them were type 2. Subjects aged between 55 and 65 years were the most represented (33.9%), with an average age of 59.4 years. Women were in the majority (61.2%), with a sex ratio of 0.6. High blood pressure was associated with diabetes in 61.2% of patients. The prevalence of ALL was 48.5% (80 patients). Obliterative arteriopathy of the lower limbs (OALL) was observed in 70 patients (42.4%), mediacalcosis in 8 patients (4.8%), and mixed arteriopathy in 2 patients (1.2%). In 88.5% of patients, the pathology was symptomless. Factors significantly associated with ALL included advanced age (p = 0.01), the age of the glycemic imbalance (p = 0.03), lack of regular physical activity (p = 0.001) and dyslipidemia (p = 0.008). Conclusion: Pelvic limb arteriopathy is usual among diabetics at Oueme-Plateau Provincial Teaching Hospital. The use of SPI, which enables diagnosis, requires simple, low-cost means, and should be encouraged.

Use of the “Veda-Lab Easy Reader+” for the Determination of T3, T4 and TSH Hormones in the Mountainous Population of Benin: Case of Natitingou

The thyroid, an endocrine gland located at the base of the neck, produces thyroid hormones (triiodothyronine (T3) and thyroxine (T4)). The production of these hormones is possible by iodine and other nutrients such as selenium and certain vitamins. To assess the thyroid disturbances in the mountain population of Benin, a survey was conducted in Natitingou, a mountain town located in the department of Atacora, in the northwest of Benin, on a sample of thirty (30) adults (15 men and 15 women), most of whom are educated. The results of the questionnaire revealed that 43% of the surveyed population acknowledged having knowledge on the mentioned subject and have dietary habits based on the consumption of seafood, and also legumes (20%). The examination of the serum results of the dosage of T3, T4 and TSH hormones revealed cases of thyroid disturbances in the region (36.32% in men and 44.98% in women). The analysis of a comparative table including the “VEDALAB Easy Reader+” and five (05) other readers, presents the performance, reading techniques, principles, advantages and disadvantages of each device. Pending further studies, some recommendations were made at the end of this study to the academic authorities regarding probable cases of dysthyroidism for which additional examinations are required and an awareness for the improvement of dietary habits.

Blood Glucose Measurement in Diabetic Patients Undergoing Comparative Treatment with a Medicinal Plant

The management of non-communicable chronic diseases such as diabetes and hypertension remains a concern in developing countries. In Guinea, whether by choice or necessity, traditional medicine and herbal remedies are the primary recourse for the population, both in rural and urban areas. Previous investigations led to the development of two phytomedicines designed for managing essential hypertension (HTN) and type 2 diabetes (DT2), named “Guinex-HTN®” and “Sattagas.” An evaluation of their therapeutic benefits was conducted during a prevalence survey carried out in Dubréka. This survey revealed a prevalence of 73.52% (211/287) for HTN, 36.58% (105/287) for DT2, and 33.45% (96/287) for combined DT2/HTN. The therapeutic potential of Sattagas and the combination of Sattagas with Guinex-HTN were assessed in diabetic and hypertensive diabetic patients, respectively. Comparisons were made between Sattagas, Metformin, Sattagas plus Metformin, and Sattagas plus Guinex-HTN regarding their impact on blood glucose levels and/or blood pressure values. After 6 months of treatment with 2 x 2 capsules per day, the anti-diabetic potential of Sattagas was confirmed by a significant reduction in blood glucose levels (from 2.59 ± 0.43 to 1.22 ± 0.43) in 80.95% (85/105) of patients, as well as a significant decrease in blood pressure values (from 177 ± 20.77/98 ± 10.70 to 149 ± 29.02/85 ± 13.64) in 90.16% (55/61) of hypertensive diabetics. The anti-diabetic effect of Sattagas (from 2.19 ± 0.29 to 1.22 ± 0.31 g/l) was similar to that of Metformin (from 2.14 ± 0.23 to 1.28 ± 0.41 g/l), and a synergistic effect was observed when Sattagas was combined with Metformin (from 2.21 ± 0.49 to 1.10 ± 0.20 g/l). In hypertensive diabetics, the combination of Sattagas and Guinex-HTN demonstrated a beneficial effect on blood pressure values. Except for some symptoms (epigastric pain, nausea, diarrhea, drowsiness, fatigue) reported in 17 out of 103 patients that did not interfere with the continuation
