Differential modulation of bradykinin-induced relaxation of endothelin-1 and phenylephrine contractions of rat aorta by antioxidants

Aim:We tested the hypothesis that bradykinin （BK）-induced relaxation of phe-nylephrine （PE） and endothelin-1 （ET-1） contractions can be differentially modu-lated by reactive oxygen species （ROS）.Methods:Aortic rings isolated fromSprague-Dawley rats were used for the *** contribution of ROS to PE(1×10-9-1×10-5mol/L)-and ET-1 (1×10-10-1×10-8mol/L)-induced contractions andthe influence of ROS in BK (1×10-9-1×10-5mol/L) relaxation of PE (1×10-7mol/L) orET-1 (1×10-9mol/L)-induced tension was evaluated in the aorta in the presence orabsence of the following antioxidants:catalase （CAT,300 U/mL）,superoxidedismutase （SOD,300 U/mL）,and vitamin C (1×10-4mol/L).Results:Tension gen-erated by ET-1 (1×10-9mol/L) or PE (1×10-7mol/L) was differentially relaxed by BK(1×10-5mol/L),producing a maximal relaxation of 75%±5% and 35±4%,*** BK (1×10-5mol/L)-induced relaxation of PE (1×10-7mol/L) tension was signifi-cantly enhanced from 35%±4% （control） to 56%±9%,60%±5%,and 49%±6% bySOD,CAT,and vitamin C,respectively （Pfrom 75%±5% （control）to 37%±9%,63%±4%,and 39%±7% by SOD,CAT,and vitamin C,respectively（P<0.05,n=8）.On the other hand,CAT had no effect on PE-induced tension,whileSOD enhanced PE-induced tension （36%,P<0.05,n=10） and vitamin C attenuated（66%,P<0.05,n=8） the tension induced by *** contrast,SOD or vitamin C hadno effect,but CAT attenuated （44%,P<0.05,n=9） the tension induced by ***:We have demonstrated that O2-and H2O2differentially modulate BKrelaxation in an agonist-specific manner.O2-attenuates BK-induced relaxation ofPE contraction,but contributes to the relaxation of ET-1 contraction.O2-seems toinhibit PE contraction,while H2O2contributes to ET-1-induced ***,ROS differentially modulate vascular tone depending on the vasoactive agentthat is used to generate the tone.

On the Stability Analysis of a Coupled Rigid Body

In this research article, we investigate the stability of a complex dynamical system involving coupled rigid bodies consisting of three equal masses joined by three rigid rods of equal lengths, hinged at each of their bases. The system is free to oscillate in the vertical plane. We obtained the equation of motion using the generalized coordinates and the Euler-Lagrange equations. We then proceeded to study the stability of the dynamical systems using the Jacobian linearization method and subsequently confirmed our result by phase portrait analysis. Finally, we performed MathCAD simulation of the resulting ordinary differential equations, describing the dynamics of the system and obtained the graphical profiles for each generalized coordinates representing the angles measured with respect to the vertical axis. It is discovered that the coupled rigid pendulum gives rise to irregular oscillations with ever increasing amplitude. Furthermore, the resulting phase portrait analysis depicted spiral sources for each of the oscillating masses showing that the system under investigation is unstable.

Breastfeeding Awareness and Practices in Abakaliki, Southeast, Nigeria

Background: The Federal Teaching Hospital is the only tertiary hospital in Ebonyi State Nigeria. It receives referrals from the general and private hospitals, primary health centers in the state as well as the surrounding state. Breast feeding is a social norm among the indigenes of the state hence practiced commonly. Aim: The study was aimed at determining the knowledge and practice of exclusive breast feeding among the women presenting in the hospital. Methods: This was a cross-sectional study in the maternity unit of the hospital over three months’ period from March to May 2015. A structured data sheet was administered to each mother presenting in the maternity unit of the hospital and willing to participate which were consecutively recruited into the study. Results: A total of 500 mothers participated in the study. About 98.4% were aware of exclusive breast feeding while 90.2% practiced it for different reasons and duration. Only 76.4% practiced it for a period of 4 - 6 months. Majority of the mothers (90.0%) got the information about exclusive breast feeding from the hospital. Times of initiation of breast feeding after delivery were 38.2% within 30 minutes and 33.2% an hour respectively. Seventy six percent of the mothers’ breastfed on demand. About 91.6% were willing to practice exclusive breast feeding for subsequent babies. Surprisingly 10.2% of the mothers discarded their colostrums. Practice of exclusive breast feeding was significantly related to educational status and occupation. Conclusion: Greater awareness and practice of exclusive breastfeeding has been achieved but more still needed to be done especially about the time of initiation, giving of colostrums and the duration of exclusive breast feeding since up to 10.4% of the participants still discarded their colostrums particularly those that delivered outside the hospital.

Incidence, Presentation and Management of Bartholin’s Gland Cysts/Abscesses: A Four-Year Review in Federal Teaching Hospital, Abakaliki, South-East Nigeria

Background: The Bartholin’s gland cysts and abscesses are one of the most common vulva cyst or abscesses in gynaecological practice. Symptomatic cases give significant discomfort to sufferers and have a negative impact on their quality of life. Objective: To investigate the incidence, pattern of presentation and management of Barthholin’s gland cysts and abscesses in the Federal Teaching Hospital Abakaliki (FETHA) Ebonyi State, Nigeria. Methodology: This was a four-year retrospective study of cases of Bartholin’s gland cysts and abscesses in FETHA. We studied all cases of Bartholin’s gland cysts and abscesses that were managed at the Federal Teaching Hospital Abakaliki from 1st January 2012 to 31st December 2015. Results: During the study period, there were 1015 gynaecological surgical cases of which 18 were for Bartholin’s gland cysts or abscess giving an incidence of 1.78%. The mean age of the patients was 28.8 ± 5.6 years with 61% of the patients within the age range of 21 to 30 years. The commonest risk factor that was found for the occurrence of Bartholin’s gland cyst or abscess was previous history of the disease in 14 (77.8%) followed by previous history of sexually transmitted diseases 8 (44.4%). Pain was the commonest presenting symptom in 14 (77.8%) of cases. The left vulva was the commonest site of disease as noted in 15 (83.3%) of patients. Escherichia coli and Staphylococcus aureus were the commonest isolates on swabs with 16 (88.9%) and 14 (77.8%) prevalence respectively. The disease presented commonly in form of abscess as observed in 10 (55.6%) of patients. All the patients had Marsupialization as the modality of treatment. Conclusion: Symptomatic Bartholin’s gland cyst and abscess cause significant morbidity for the sufferers and decreased quality of life. Declining Rate of Operative Vaginal Deliveries in Nigeria

Background: The application of invasive obstetric procedures has an end point of reducing the Caesarean section rate. The declining rate of use of these procedures is one of the reasons for increasing Caesarean section rates in our environment. Objective: The aim of the study was to determine the practice of operative vaginal deliveries among obstetricians practicing in Nigeria and to evaluate the reasons for non-use of these procedures. Methodology: It was a questionnaire based study. The questionnaires were administered to practitioners of Obstetrics in the various centres in Nigeria. Data was collated and analyzed with Epi-Info statistical software version 7.0 (Center for Disease Control and Prevention, USA), and conclusions were drawn by means of descriptive statistics. Results: A total of 1200 questionnaire were distributed but 1104 were returned and used for analysis. This gave a response rate of 92%. The age distribution of the respondents showed that the 20 - 30 year age group had the least number of respondents 22 (2%) while 41 - 50 age group had the highest number of respondents 486 (4%). The majority of the respondents were males 839 (76%). In terms of duration of practice, 449 (40.7%) of the respondents had more than 6 years duration of practice while 256 (23.2) had practiced for less than 3 years. Tertiary centres had highest number of respondents 71.8%. The distribution of the respondents according to the geopolitical zones in Nigeria are;South-East (57.6%), South-South (19.6%), South West (11.4%), North-West (4.9%), North-East (3.8%), North-Central (2.7%). The practice of these procedures among respondents were: External cephalic version (68.0%), Symphysiotomy (41.7), Forceps delivery (68.8%), Destructive delivery (60.1%), Vaginal breech delivery (85.5%), Vacuum extraction (84.8%). A 5-Year Retrospective Study on the Use of Manual Vacuum Aspiration in the Federal Teaching Hospital Abakaliki

Introduction: Since the inception of our hospital in 2011, manual vacuum aspiration has been in use for the treatment first trimester miscarriages. Hence there is a need for operational review on its use. Aim: The aim of this study is to evaluate the determinants and outcome of Manual Vacuum Aspiration (MVA) use in our hospital. Method: This was a retrospective study on the use of MVA for various indications in our facility over a 5-year period. Results: There were 625 (19.7%) manual vacuum aspirations among 3179 gynaecological patients seen during the period. The age range of the women was from 15 to 48 years and the mean age was 28.5 ± 5.3 years. Incomplete abortion was the commonest indication for the use of MVA and accounted for 89.9% of cases. Other indications for the use of MVA include missed miscarriage (1.8%), and blighted ovum (1.8%). The complications were uterine perforation (0.3%), infection (3.7%) and severe anaemia (10.7%). The mean total hospital stay was 1.6 ± 0.6 days. Overall, 267 (42.7%) patients were managed as a day case. Conclusion: Manual vacuum aspiration is an effective tool in the management of early pregnancy complications. It is a safe, easily performed and possibly cost-effective procedure, with advantages for both the patient and the health care system.

Background: There are no known effective vaccines or drugs for curing the corona 19 virus infection as at present knowlede. This leaves the world with mainly controlling the source of infection and cutting off the transmission route through preventive measures to contain the pandemic. These entail active participation of people, high level of discipline and strict and meticulous adoption of preventive measures. Since it is known that knowledge, precautionary behavior and active social participation of the public are important to control the pandemic, this study sets out to evaluate these in the antenatal population in Ebonyi State. Objective: To evaluate the Knowledge, attitude and practice of the respondents to the corona virus 19 infection and its prevention. Methods: A self-administered pre-tested questionnaire was administered to pregnant women attending antenatal clinics in the chosen Mission hospitals and the only tertiary health institution that offer antenatal care to the bulk of antenatal population in the state. The results were analyzed using SPSS version 20 and Microsoft Excel 2016. Result: The participants had very good knowledge of corona virus 19 infection and its prevention but there were some gaps and misconceptions in knowledge and some of their attitudes and perceptions that raised concerns. Their practice of corona virus 19 preventive measures was poor despite their good knowledge. Conclusion: There is need to fashion out programs and policies that will address the gaps and concerns unearthed in the study to enhance the protection of this vulnerable study population and the general public.

Prevalence and Pattern of Abnormal Pap Smear among Pregnant Women Attending Antenatal Clinic in a Missionary Hospital in Abakaliki, Southeast Nigeria

BACKGROUND: Cervical cancer is the most common type of cancer affecting the genital tract among women in developing countries. Screening programmes have been well talked about in Nigeria, but they are not well organized. Hence, the incidence of cervical cancer does not appear to be reducing. A possible way of solving this problem is by doing an opportunistic pap smear among women attending antenatal clinics, since a good number of women do access antenatal care. OBJECTIVES: To determine the prevalence, knowledge, awareness, practice, risk factors and pattern of Pap smear among women attending antenatal clinic in Mile 4 hospital, Abakaliki. METHOD: This is a cross-sectional study carried out at the antenatal clinic of Mile 4 Hospital, Abakaliki, Nigeria. A total of 110 consecutive pregnant women who fulfilled the inclusion criteria and have given their consent were recruited into the study at the time of their first prenatal (booking clinic) visit for antenatal care. Data was collected using a questionnaire. Conventional Papanicolaou smear was taken using the standard procedure. The cytopathologic findings were documented in the questionnaire. RESULTS: Out of the 110 pregnant women who had cervical cytology by Pap smear, 7 had pre-invasive cervical lesion, giving a prevalence rate of 6.3%;5 (71.4%) were low-grade squamous intraepithelial lesion (LGSIL) while 2 (28.6%) were high-grade squamous intraepithelial lesion (HGSIL). Fifty women (45.5%) had negative smears. Inflammatory smears were seen in 50 women (45.5%). Insufficient smear was seen in 3 women (2.7%). Fifty-six women had knowledge about cervical cancer (50.9%), while thirty-six women (32.7%) were aware of a screening program for cervical cancer and only 2 women (1.8%) had been screened in the past. Hence, from this study, the population screening rate was approximately 2%.
