AIM:To compare anterior segment parameters between two groups of type 2 diabetic with and without diabetic retinopathy(DR)and non-diabetic elderly subjects based on hemoglobin A1c(Hb A1c)levels and status of ***:This study was conducted on 997 residents aged 60y or over in Tehran,*** group had Hb A1c level≥6.4%with no other systemic *** non-diabetic participants had normal eye findings and no systemic diseases.K1,K2,mean K,Q-value,anterior,central,posterior,and total corneal densitometric findings,anterior chamber volume(ACV),anterior chamber depth(ACD),corneal volume(CV),and pachymetry were measured by Pentacam ***:A total of 678 non-diabetic(39%male)and 319 diabetic(35%male)subjects with mean age of 66.31±5.23 and 67.22±4.96y were examined,*** statistically significant difference was found in anterior segment parameters between non-diabetic and diabetic groups(all P>0.05).However,middle,posterior,and total corneal densitometric values were statistically different between two groups af ter controlling the ef fects of confounders(P=0.014,0.007,and 0.042,respectively).Corneal densitometric values in all layers,ACD,and ACV were different between diabetic subjects with and without DR(all P<0.05).In the diabetic group,only corneal densitometric values had a negative relationship with fasting blood sugar(P<0.001).ACD and ACV had a negative correlation with Hb A1c levels(all P<0.05,r=-0.129 and-0.146,respectively).However,the relationships were not observed after controlling the confounders(P=0.938,0.466,respectively).CONCLUSION:Considering the higher densitometric values of the cornea and lower ACD and ACV in diabetic subjects with DR,it is suggested that the examiners should perform comprehensive retinal examinations when faced with such conditions.

In this study,Au nanoparticles/poly 5-[（2-hydroxynaphthalen-l-yl）diazenyl]isophthalic acid film modified glassy carbon electrode（AuNPs/poly（NDI）/GCE） has shown excellent electrocatalytic activity toward the oxidation of adrenaline（ADR）,paracetamol（PAC）,and tryptophan（Trp）.The bare glassy carbon electrode（GCE） fails to separate the oxidation peak potentials of these molecules,while the poly（NDI） film modified electrode can resolve *** impedance spectroscopy（EIS）indicates that the charge transfer resistance of the bare electrode decreases as 5-[（2-hydroxynaphthalen-l-yl）diazenyl]isophthalic acid is electropolymerized on the bare ***,EIS exhibits enhancement of electron transfer kinetics between analytes and the electrode after electrodeposition of Au *** pulse voltammetry results show that the electrocatalytic current increases linearly in the ranges of 0.01-680.0 μmol L^-1 for ADR,0.05-498.0 μmol L^-1 for PAC,and 3.0-632.0 μmol L^-1 for Trp;with detection limits（S/N = 3） of 0.009 μmol L^-1,0.005 μmol L^-1,and 0.09 μmol L^-1 for ADR,PAC,and Trp,*** proposed method has been successfully applied for simultaneous determination of ADR,PAC,and Trp in biological samples.

Introduction: Clinical competency and knowledge of the effective factors are of critical importance in nursing. Competency, self-esteem, and self-knowledge are achievable through professional identity. Employees’ participation in and attachment to their professional role is contingent considerably upon their professional identity. In light of this, the present study is aimed at determining correlation between professional identity and clinical competence of the nurses working in psychiatric wards of the hospitals affiliated with Tehran-based medical sciences universities. Methodology: This correlative study was conducted on 125 nurses working in psychiatric wards of Tehran-based hospitals affiliated with medical sciences universities. Data gathering tool of the study was a researcher-designed questionnaire to collect demographic, clinical competency, and professional identity data of the participants. For data analyzing, Statistical Package for the Social Sciences (SPSS) version 18 was used. Findings: The results showed 91.2% of the participants evaluated their professional identity at good (47.2%) and very good (44%) levels;and 78.2% evaluated their clinical competency at very good level. The results also indicated positive and significant relationship between professional identity and clinical competency(r = 0.32, p = 0.001). In addition, none of the demographical variables were related to professional identity and clinical competency. Conclusion: There is positive relationship between clinical competency and professional identity, which means that development of positive attitudes regarding the psychiatric nursing can lead to higher clinical competency of the participants. The findings of the study highlighted the weight and role of positive attitude regarding the professional identity. It could result in increase of clinical competency and deserved attention of hospital’s managers.

To describe a case of metastatic lung carcinoma to the cavernous sinus and orb ital apex diagnosed by fine needle aspiration guided by computed tomography. Ca se report. A 52 year old man who presented with right sided ptosis and ophtha lmoplegia was found to have an enhancing mass of the right orbital apex and cave rnous sinus extending into the temporal fossa on magnetic resonance imaging. Che st computed tomography revealed hilar adenopathy and a lesion of the right lower lobe of the lung. Bronchial washings and transbronchial needle biopsies of the lung were nondiagnostic. Computed tomography guided fineneedle aspiration of th e temporal fossa portion of the mass was performed. Biopsy of the mass showed ma lignant cells consistent with metastatic non small cell lung carcinoma. Compute d tomography guided fine needle aspiration can be useful in the diagnosis and management of some masses involving the cavernous sinus.

PURPOSE: To report the development of a macular hole as a complication of cat scratch disease. DESIGN: Case report. METHODS: A 10- year-old girl was seen with unilateral neuroretinitis from serologically confirmed cat scratch disease. Twelve days later, she developed a macular hole in the involved eye. Fundus photography and optical coherence tomography (OCT) were obtained at presentation and after the development of a macular hole. RESULTS: Fundus photography and OCT on presentation revealed a nasal neurosensory detachment and multiple inflammatory foci throughout the macula, including a subfoveal lesion. Fundus photography and OCT 12 days later revealed the development of a partial posterior vitreous detachment and a full thickness macular hole. CONCLUSIONS: Macular hole should be included among posterior segment complications of cat scratch disease. In this case, the macular hole was associated with partial vitreous detachment and a preexisting subfoveal lesion, likely representing an inflammatory focus.

白内障术后复视1例

A 69-year-old man developed binocular, vertical diplopia after undergoing ca taract extraction in both eyes. He had normal extraocular motility and a 2 prism diopter right hypertropia that was comitant but could not be relieved with over lying prisms. Funduscopy revealed an epiretinal membrane within the macula on th e left more than the right. After his metamorphopsia worsened, and his visual ac uity decreased to 20/40 in the left eye, he underwent pars plana vitrectomy with removal of the epiretinal membrane and his diplopia resolved. Macular pathology including epiretinal membranes and choroidal neovascular membranes may rarely c ause binocular diplopia because of foveal displacement and rivalry between centr al and peripheral fusional mechanisms.

Background/aims: Vitamin A deficiency, often presenting with nyctalopia, has been described in a number of patients with malabsorption as a result of intestinal bypass surgery and, more recently, bariatric surgery. In these reports vitamin A deficiency developed within several years of gastric or intestinal surgery. Three patients who developed decreased vision from vitamin A deficiency more than 18 years after their intestinal surgery are reported. Methods: A retrospective review of the clinical findings of all patients diagnosed with vitamin A deficiency, as confirmed by serological testing, over the past year in a single neuroophthalmic practice. Results: Four patients with vitamin A deficiency were seen, three of whom had intestinal surgery more than 18 years before the development of visual symptoms. Conclusion: The authors suggest that vitamin A deficiency should be suspected in patients with unexplained decreased vision and a history of intestinal surgery, regardless of the timing of the surgical procedure.
