Objective The aim of this review is to objectively access the trial evidence on the role of omega-3, red yeast rice and garlic in preventing clinical cardiovascular events. Given the large number of clinical trials favoring statin use in cardiovascular disease, it is important to see if evidence is available for these supplements and whether they could replace statin therapy. Data source A PubMed search was conducted using the keywords ＇trial, omega-3, red yeast rice, xuezhikang, garlic, cholesterol, cardiovascular, outcomes＇; the resulting trials were reviewed together with the references quoted in the papers obtained. Study selection The studies selected are prospective, randomized, placebo-controlled studies with predefined clinical cardiovascular end-points recruiting at least 2000 patients, with a follow-up over 2 years. Results Modest dose omega-3 fatty acid has been shown in GISSI-P （11 324 patients, follow-up 3.5 years） to produce a reduction in sudden death of 45%, and in cardiac death of 35%, acting probably via an anti-arrhythmic effect. In JELLS （18 645 patients, follow-up 4.6 years）, high dose omega-3 given to Japanese patients on a high fish diet and already on statin treatment produced further benefit with a 19% reduction of nonfatal cardiovascular outcomes; fatal cardiac events are not affected. CCSPS （4870 patients, follow-up 4 years）, a secondary prevention trial using xuezhikang, a commercial red yeast rice preparation, produced a 46% reduction in nonfatal myocardial infarction and coronary death. There has been no trial to show that garlic reduces clinical cardiovascular outcomes. A rigorous trial with constant assessment of chemicals in the study material in 192 patients found that over a 6-month follow-up, raw garlic and 2 commercial preparations do not significantly affect lipid levels. Axon regeneration in the CNS is largely unsuccessful due to excess inhibitory extrinsic factors within lesion sites together with an intrinsic inability of neurons to regrow following injury. Recent work demonstrates that forced expression of certain neuronal transmembrane receptors can recapitulate neuronal growth resulting in successful growth within and through inhibitory lesion environments. More specifically, neuronal expression of integrin receptors such as alpha9beta1 integrin which binds the extracellular matrix glycoprotein tenascin-C, trk receptors such as trk B which binds the neurotrophic factor BDNF, and receptor PTPσ which binds chondroitin sulphate proteoglycans, have all been show to significantly enhance regeneration of injured axons. We discuss how reintroduction of these receptors in damaged neurons facilitates signalling from the internal environment of the cell with the external environment of the lesion milieu, effectively resulting in growth and repair following injury. In summary, we suggest an appropriate balance of intrinsic and extrinsic factors are required to obtain substantial axon regeneration.

Determination of Water Quality of Rivers under Various Land Use Activities Using Physico-chemical Parameters and Bacterial Populations in Northern Peninsular Malaysia

A study on water quality of rivers with different land use activities by using physico-chemical parameters and bacterial population was conducted around Kedah, Penang and Perak, Malaysia. The water quality parameters such as dissolved oxygen(DO), five-day biochemical oxygen demand(BOD5), chemical oxygen demand(COD), total dissolved solids(TDS), total suspended solids(TSS), nitrate, p H, ammonia-nitrogen, water temperature, water depth and velocity were measured and the densities of bacteria which were total coliform(TC) and fecal coliform(FC) were monitored simultaneously using IDEXX-Colilert?technique from selected rivers. The greatest population of TC and FC(>2 419.6 n MPN/100ml) were recorded from industrial rivers while the lowest reading recorded in less disturbed rivers with less than 1 MPN/100 ml. Total coliform count categorized all rivers into Class I and FC count showed five rivers fell into Class I(Sungai Sedim, Bukit Hijau, Teluk Bahang, Kurau and Tukun), two rivers fell into Class IIA(Sungai Batu Hampar and Salleh) and eight rivers fell into IIB categories(Sungai Jerejak,Pinang, Air Hitam, Kluang, Chelong, Titi Hayun and Cherok Tokun). There was a significant difference between the TC population with TSS content(r=0.43), TDS content(r=0.813), river depth(r=0.513) and DO level(r=-0.401) from all selected *** coliform population was greatly influenced by contents of BOD5(r=0.456), COD(r=0.531), TSS(r=0.813), TDS(r=0.724), and water temperature(r=0.711). The rivers with industrial land-use setting recorded the highest range of water quality index scores and fell into Class III(Sungai Jerejak, Air Hitam and Kluang) and Class V(Sungai Pinang). Both plantation rivers(Sungai Chelong and Sungai Salleh) fell into Class II and all recreational and less disturbed rivers fell into Class I.

Cyber-physical systems(CPSs)in critical infrastructure face serious threats of attack,motivating research into a wide variety of defence mechanisms such as those that monitor for violations of invariants,*** properties over sensor and actuator states that should always be *** approaches for identifying invariants attempt to do so automatically,typically using data logs,but these can miss valid system properties if relevant behaviours are not well-represented in the ***,as the CPS is already built,resolving any design flaws or weak points identified through this process is *** this paper,we propose a systematic method for deriving invariants from an analysis of a CPS design,based on principles of the axiomatic design methodology from design *** method iteratively decomposes a high-level CPS design to identify sets of dependent design parameters(*** and actuators),allowing for invariants and invariant checkers to be derived in parallel to the implementation of the *** apply our method to the designs of two CPS testbeds,SWaT and WADI,deriving a suite of invariant checkers that are able to detect a variety of single-and multi-stage attacks without any false ***,we reflect on the strengths and weaknesses of our approach,how it can be complemented by other defence mechanisms,and how it could help engineers to identify and resolve weak points in a design before the controllers of a CPS are implemented.

BACKGROUND Peroneal tendinopathies are an under-diagnosed and potentially under-treated pathology. If left untreated it can be a cause of chronic lateral hindfoot pain. Its diagnosis is challenging owing to its low incidence and vague clinical *** SUMMARY We share a case of a patient who experienced a chronic lateral ankle pain exacerbated after alighting from a bus. This patient came to our attention only after failing conservative management on two separate occasions. Plain radiographs and magnetic resonance imaging revealed rupture of the peroneus longus tendon(PLT). Findings were confirmed intra-operatively and tenodesis of the PLT to the peroneus brevis was performed. Patient was kept non-weight-bear with his foot everted and in plantarflexion before being converted to an offloading boot at two weeks. Patient was started on a progressive rehabilitation programme at six weeks and was able to return to work shortly after with excellent *** We aim to share our experience in managing this patient and propose some pointers guided by available literature to avoid missing this commonly overlooked pathology.
