High Response Rate to Second-Line Combination Antiangiogenic Chemotherapy in Patients with metastatic melanoma

Objectives: Despite the array of new treatment strategies for the management of metastatic melanoma, the prognosis remains poor when immune checkpoint inhibition and targeted therapy options are exhausted. The antiangiogenic monoclonal antibody bevacizumab has documented its efficacy in the treatment of several solid tumors, when used in combination with standard chemotherapy. Phase II studies and case series have led to the speculation that it may also improve the prognosis when used in stage IV melanoma. Therefore, we investigated the influence of bevacizumab, combined with a platinum-based chemotherapy, in the treatment of melanoma and sought to identify prognostic factors affecting the response. methods: Eight patients with metastatic melanoma, with documented progress after at least one previous therapy, received bevacizumab (5 mg/kg intravenously every two weeks) in combination with cisplatin (100 mg/m2 every four weeks) and carboplatin (200 mg/m2 every four weeks). The therapy was continued until renewed disease progression occurred. The response rate, progression-free and overall survival, and toxicity were evaluated. Results: We observed complete remission in two patients (25%) and partial response in an additional four patients. In one patient, the disease remained stable (total disease control rate of 87.5%). Only one patient (12.5%) had progressive disease. The median progression-free survival was 6 months (range 3 - 37 months). The median overall survival time was 15.5 months (range 6 - 77 months). Every patient experienced at least one adverse event of grade 3 - 4, most commonly bleeding associated with severe thrombocytopenia. Conclusion: Our observations indicate that bevacizumab, in combination with cisplatin and carboplatin, may represent an effective treatment option for patients with metastatic melanoma and disease progression.

Significance of an additional unenhanced scan in computed tomography angiography of patients with suspected acute aortic syndrome

AIm To assess potential benefits of an additional unenhanced acquisition in computed tomography angiography(CTA) in patients with suspected acute aortic syndrome(AAS).mETHODS A total of 103 aortic CTA(non-electrocardiography-gated, 128 slices) performed due to suspected AAS were retrospectively evaluated for acute aortic dissection(AAD), intramural hematoma(ImH), or penetrating aortic ulcer(PAU). Spiral CTA protocol consisted of an unenhanced acquisition and an arterial phase. If AAS was detected, a venous phase(delay, 90 s) was added. Images were evaluated for the presence and extent of AAD, ImH, PAU, and related complications. The diagnostic benefit of the unenhanced acquisition was evaluated concerning detection of *** Fifty-six(30% women; mean age, 67 years; median, 68 years) of the screened individuals had AAD or ImH. A triphasic CT scan was conducted in 76.8%(n =43). 56% of the detected AAD were classified as Stanford type A, 44% as Stanford type B. 53.8% of the detected ImH were classified as Stanford type A, 46.2% as Stanford type B. There was no significant difference in the involvement of the ascending aorta between AAD and ImH(P = 1.0) or in the average age between AAD and ImH(P = 0.548), between Stanford type A and Stanford type B in general(P = 0.650) and between Stanford type A and Stanford type B within the entities of AAD and ImH(AAD: P = 0.785; ImH: P = 0.146). Only the unenhanced acquisitions showed a significant density difference between the adjacent lumen and the ImH(P = 0.035). Subadventitial hematoma involving the pulmonary trunk was present in 5 patients(16%) with Stanford A AAD. The difference between the median radiation exposure of a triphasic(2737 mGy*cm) compared to a biphasic CT scan(2135 mGy*cm) was not significant(P = 0.135).CONCLUSION ImH is a common and difficult to detect entity of AAS. An additional unenhanced acquisition within an aortic CTA protocol facilitates the detection of ImH.

The steady progress in medical diagnosis and treatment of diseases largely hinges on the steady development and improvement of modern imaging *** spectroscopy has attracted increasing attention for clinical applications as it is label-free,non-invasive,and delivers molecular fingerprinting information of a *** combination with fiber optic probes,it also allows easy access to different body parts of a ***,image acquisition with fiber optic probes is currently not ***,we introduce a fiber optic probe-based Raman imaging system for the real-time molecular virtual reality data visualization of chemical boundaries on a computer screen and the physical *** approach is developed around a computer vision-based positional tracking system in conjunction with photometric stereo and augmented and mixed chemical reality,enabling molecular imaging and direct visualization of molecular boundaries of three-dimensional *** proposed approach achieves a spatial resolution of 0.5 mm in the transverse plane and a topology resolution of 0.6 mm,with a spectral sampling frequency of 10 Hz,and can be used to image large tissue areas in a few minutes,making it highly suitable for clinical tissueboundary demarcation.A variety of applications on biological samples,i.e.,distribution of pharmaceutical compounds,brain-tumor phantom,and various types of sarcoma have been characterized,showing that the system enables rapid and intuitive assessment of molecular boundaries.

Impact of crystalline and amorphous iron-and aluminum hydroxides on mechanisms of phosphate adsorption and desorption

Fourier-transform infrared（FT-IR） spectroscopic experiments were carried out during phosphate adsorption on highly crystalline gibbsite, poorly crystalline 2-line-ferrihydrite and amorphous iron–aluminum–hydroxide mixtures in the molar ratio 1：0, 10：1, 5：1, 1：1, 1：5, 1：10 and 0：1. The OH stretching vibrational bands were utilized to analyze changes in structural and surface OH groups during adsorption, because the position of characteristic P／O vibrational bands can shift depending on reaction conditions, pH or adsorbed phosphate *** and desorption kinetics were studied at pH6 and different initial phosphate concentrations to achieve varying phosphate coverage on the mineral surfaces. For gibbsite the formation of AlHPO4 and Al2HPO4 can be assumed, while for ferrihydrite, a FeHPO4 or Fe2PO4 complex and the precipitation of FePO4 with longer equilibration time were ***2HPO4 or a Fe2PO4 surface complex was deduced for Fe-hydroxides, an AlH2PO4 surface complex was identified for Al-hydroxide, and both displayed either hydrogen bonds to neighboring hydroxyl groups or hydrogen bonds to outer-sphere complexes. Fe：Al-hydroxide mixtures with high Al ratios showed a low phosphate desorption rate, while ferrihydrite and the Fe：Al-hydroxide mixtures with high Fe ratios had almost negligible desorption rates. It was concluded that within the weakly associated amorphous FeO（OH） materials, FePO4 precipitated, which was bound by outer-sphere hydrogen bonds. With high Al ratios, desorption increased, which indicated weaker phosphate binding of both inner-sphere and outer-sphere complexes and hence, either no or minor quantities of precipitate. Ferrihydrite showed a more rigid structure and a lower extent of precipitation compared to amorphous Fe-hydroxide.

Power flow forecasts at transmission grid nodes using Graph Neural Networks

The increasing share of renewable energy in the electricity grid and progressing changes in power consumption have led to fluctuating,and weather-dependent power *** ensure grid stability,grid operators rely on power forecasts which are crucial for grid calculations and *** this paper,a multi-Task Learning approach is combined with a Graph Neural Network(GNN)to predict vertical power flows at transformers connecting high and extra-high voltage *** proposed method accounts for local differences in power flow characteristics by using an Embedding multi-Task Learning *** use of a Bayesian embedding to capture the latent node characteristics allows to share the weights across all transformers in the subsequent node-invariant GNN while still allowing the individual behavioral patterns of the transformers to be *** the same time,dependencies between transformers are considered by the GNN architecture which can learn relationships between different transformers and thus take into account that power flows in an electricity network are not independent from each *** effectiveness of the proposed method is demonstrated through evaluation on two real-world data sets provided by two of four German Transmission System Operators,comprising large portions of the operated German transmission *** results show that the proposed multi-Task Graph Neural Network is a suitable representation learner for electricity networks with a clear advantage provided by the preceding embedding *** is able to capture interconnections between correlated transformers and indeed improves the performance in power flow prediction compared to standard Neural Networks.A sign test shows that the proposed model reduces the test RmSE on both data sets compared to the benchmark models significantly.

Autoencoder-based anomaly root cause analysis for wind turbines

A popular method to detect anomalous behaviour or specific failures in wind turbine sensor data uses a specific type of neural network called an *** models have proven to be very successful in detecting such deviations,yet cannot show the underlying cause or failure *** information is necessary for the implementation of these models in the planning of maintenance *** this paper we introduce a novel method:*** use ARCANA to identify the possible root causes of anomalies detected by an *** describes the process of reconstruction as an optimisation problem that aims to remove anomalous properties from an anomaly *** reconstruction must be similar to the anomaly and thus identify only a few,but highly explanatory anomalous features,in the sense of Ockham’s *** proposed method is applied on an open data set of wind turbine sensor data,where an artificial error was added onto the wind speed sensor measurements to acquire a controlled test *** results are compared with the reconstruction errors of the autoencoder *** ARCANA method points out the wind speed sensor correctly with a significantly higher feature importance than the other features,whereas using the non-optimised reconstruction error does *** though the deviation in one specific input feature is very large,the reconstruction error of many other features is large as well,complicating the interpretation of the detected ***,we apply ARCANA to a set of offshore wind turbine *** case studies are discussed,demonstrating the technical relevance of ARCANA.

Early CD8 T-cell memory precursors and terminal effectors exhibit equipotent in vivo degranulation

Early after priming, effector CD8 T cells are distinguished into memory precursor and short-lived effector cell subsets （mPECs and SLECs）. Here, we delineated a distinct in vivo heterogeneity in killer cell lectin-like receptor G1 （KLRG-1） expression, which was strongly associated with diverse mPEC and SLEC fates. These in vivo mPECs and SLECs expressed equivalent levels of cytotoxic molecules and effector cytokines. Using a unique in vivodegranulation assay, we found that the mPECs and SLECs similarly encountered infected target cells and elaborated equivalent levels of cytotoxicity in vivo. These data provide direct in vivoevidence that memory-fated ceils pass through a robust effector phase. Additionally, the preferential localization of the mPECs in the lymph nodes, where a lesser degree of cytotoxicity was elaborated, suggests that the mPECs may be protected from excessive stimulation and terminal differentiation by virtue of their differential tissue localization. These data provide novel mechanistic insights into the linear decreasing potential model of memory differentiation.
