Background: Exercise-Induced Bronchospasm (EIB) is an inflammatory condition characterized by severe airway constriction following the mobilization of inflammatory cells and interleukin-6 (IL-6). When severe, EIB can require the use of pressurized salbutamol to treat athletes. This study investigated the nature of the systemic changes in inflammatory cells and post-exercise IL-6 concentrations after salbutamol treatment in EIB-susceptible distance runners. Materials and Methods: This was an experimental study that enrolled 12 long-distance runners. In Session A, the participants completed a treadmill exercise test, and those who had a maximum expiratory volume per second (FEV1) that was decreased by at least 10% compared to their base value were placed in the EIB-susceptible group (EIB+) (n = 6). Those whose FEV1 did not meet this criterion were placed in the nonresponsive (EIB?) group (n = 6). Before the Session B exercise, athletes in the BIE+ group inhaled two puffs of salbutamol (EIB+ Salb), while their EIB? counterparts received no treatment. Spirometry was performed before and after the exercise using a Spirobank G portable spirometer. Blood samples were taken before, immediately after and 2 hours after the stress test. Results: The mean post-exercise FEV1 values were not significantly different (p > 0.05) between the EIB+ Salb group and the EIB? group. The systemic changes in inflammatory cells and IL-6 concentrations in the EIB+ runners after salbutamol treatment were similar to those observed in their EIB? counterparts. Conclusion: Salbutamol pretreatment improved the systemic immune status of EIB-susceptible athletes.

Heart Failure with Preserved Ejection Fraction: A Report about 64 Cases Followed at the Heart Institute of Abidjan

Objective: To describe the epidemiological and etiological features of heart failure with preserved ejection fraction in our context. Materials and Methods: This is a retrospective study that was carried out over a 12-monthperiod, from 1 January 2015 to 31 December 2015, in the hospitalization department of the Heart Institute of Abidjan. It was about patients hospitalized for heart failure who had received a dose of NT-proBNP and who had had an electrocardiogram, and a Doppler echocardiography. Heart failure with preserved ejection fraction (HFPEF) was defined from the symptoms and signs of heart failure, the level of NT-proBNP and from echocardiography data based on the left ventricular ejection fraction (LVEF) > 50%, the dilatation of the left atrium. The diastolic dysfunction of the left ventricle was assessed by the ratio E/E’ > 13. We defined as heart failure with decreased ejection fraction (HFDEF) symptoms and signs of heart failure and left ventricular dysfunction LVEF 40%. A computer file was completed. It included epidemiological and etiological data. Results: The study involved sixty-four patients with heart failure with preserved ejection fraction out of 257 patients with heart failure that is a prevalence of 25%. The mean age was 57.3 ± 16 years. There was a male predominance that is 52% of cases. Congestive heart failure was predominant in 67%. NT-proBNP levels were 365 pg/ml on average. The average length of stay was 5.5 ± 3.1. Intra-hospital deaths were 4.6%. The etiologies are dominated by high blood pressure in 85.9%, followed by obesity in 28.1%, then by ischemic heart disease in 4.6%. There were no diabetic patients in this group. Conclusion: Heart failure with preserved ejection fraction is characterized in our regions by its occurrence in?young male subjects. Congestive heart failure prevailed. The dominant etiology was high blood pressure.

Unsafe abortions constitute a public health problem. It is one of the causes of maternal mortality in the world and particularly in developing countries. Despite the progress made, maternal mortality remains high in Cameroon. The scarcity and disparity of data on abortions lead to a lack of strong evidence to advocate to decision-makers on the extent of the problems associated with abortions in Cameroon. Our objective is to estimate the rates of mortalities and complications related to unsafe abortions, as well as the difficulties of accessing safe abortions in Cameroon. We will carry out a systematic and meta-analytical review in the biomedical databases MEDLINE (Pubmed), Google Scholar and African Journal Online (AJOL) concerning unsafe abortions and/or difficulties in accessing safe abortions in Cameroon, without date or language restriction. Gray literature will be also consulted. Two authors will simultaneously select the studies and data extraction will be done using a Google Form. Proportions will be estimated on a random-effect model. The I2 and Q statistics will be used to assess the extent of heterogeneity across the studies. The outcome of both the quantitative and qualitative parts of the study will be commented. Death and morbidity due to abortions can be prevented. A concerted multidisciplinary and multicentric action would be essential.

The slope soil analysis remains a corporate concern in construction activities. Because of its significance, the evaluation of slope soil stability has called widespread attention to several researchers all over the world. Many methods have been technologically advanced to evaluate the stability of slopes soils founded on distinct expectations and circumstances. Every method has specific benefits and limits. This work makes a comparison among safety factors and slip surfaces of slopes soils based on using Limit Equilibrium and Finite Element methods. Therefore, SLIDE 6.0 and PLAXIS 8.0 software were used for Limit Equilibrium and Finite Element methods, respectively. The computations of safety factors were performed for diverse shapes of slopes including different types of soils. Failure surfaces and values of safety factors obtained were compared for both methods used. It was noticed that the safety factors obtained from Limit Equilibrium methods were larger than of which is obtained by the finite element code. Moreover, an important change is noticed between the slip surfaces obtained by using both approaches.

Pulse Pressure as a Risk Factor of Atrial Fibrillation in Black African Elderly Patients

Objective: The aim of this study was to show the prognostic role of High Pulse Pressure (PP) in the occurrence of atrial fibrillation in Black African elderly patients. Materials and Methods: In a comparative retrospective study related to 2000 patients admitted to the Institute of Cardiology of Abidjan, from January 1991 to December 2010 for atrial fibrillation, we matched pulse pressure and atrial fibrillation in Black African elderly patients. Results: The mean age of patients was 65 ± 10 years. Patients with high pulse pressure were 4.8 times more at risk to present atrial fibrillation than those with normal pulse pressure. The threshold of high risk pulse pressure was 65 mmHg. Conclusion: Pulse pressure is a factor of bad prognosis of atrial fibrillation in Black African elderly patients.

Traditional Medicine Followed at the Heart Institute of Abidjan

Objective: To determine the prevalence of the use of traditional medicine in?hypertensive patients and to identify the socio-demographic characteristics of these patients. Materials and Methods: This is a single cross-sectionalstudy with descriptive purpose that was carried out over a 4 month-period, from 1 January 2017 to 30 April 2017, in the outpatient department of the Heart Institute of Abidjan. It involved patients who consulted during this period for high blood pressure. An informed questionnaire was submitted to patients. A pre-test was performed on 20 patients prior to the start of the survey. Results: The prevalence was 34%, with a slight male predominance (52.4%). The average age of our patients was 51.7 years ± 20 years. Males (52.4%) and females (47.6%) were roughly in the same proportions in our study with a slight male predominance. Patients with a higher level of education used traditional medicine in 30.6% that is about one third of the cases. Nearly one third of the patients had a monthly income above 300,000 FCFA (28.5%). Almost half of the patients (45%) had social insurance coverage. Conclusion: The use of traditional medicine by hypertensive patients is a practice that exists and is growing rapidly. The profile of hypertensive patients using traditional medicine can be summarized as a young subject, a male with a higher education level, a high monthly income and social coverage. It is the place to insist on the therapeutic education of our hypertensive patients’ only way for a good control of the blood pressure figures.

Non-Laboring Uterine Rupture of an Unscared Uterus before Term Discover during Obstetric Ultrasound

Uterine rupture is defined as the occurrence of communication between the abdominal and uterine cavity and may be complete or incomplete depending on the degree of involvement of the different layers of the uterus and surrounding organs. It is a rare complication whose consequences often involve the maternal and fetal prognosis. The majority of uterine rupture occurs on the scarred uterus, its incidence in France is estimated according to the series between 1/1000 and 1/2000 births, it represents 30% of causes of maternal death in the developing countries. The authors report here a case of uterine rupture outside of labor at 33 weeks of age in 32 years old woman, gravida 9 para 8, with no history of uterine surgery discovered during obstetric ultrasound for abdominal pain. During the interrogation, she was alarge multipara and had a child of 15 months. The abdominal ultrasound showed a right lateral corporeal rupture with hemoperitoneum of medium sized and a dead fetus. The emergency laparotomy revealed a right lateral uterine wound approximately 15 cm long with intra-abdominal placenta and a haemoperitoneum of medium sized of about 600 cc and a bladder lesion. After opening the amniotic sac, there was extraction of a dead fetus. The uterine and bladder lesions were repaired followed by bilateral tubal ligation. The patient received 500 cc of whole blood during and 500 cc after the procedure. The postoperative follow-up was simple. This case contributes to the knowledge of this rare and atypical event, and emphasizes the importance of maintaining a suspicion.

Local Drug Delivery Strategy for Cancer Treatment: Use of Biocompatible Sol-Gel-Derived Porous Materials

Porous silica xerogel materials have been developed to use as drug-release agents to be implanted directly in or near cancerous tissues. In order to test the capacity of the materials to absorb and then to release medicinal substances, a battery of examinations (UV and visible micro-Raman, porosity measurements, UV-visible absorption spectra) have been made using test drug molecules (clotrimazole, primaquine diphosphate and the anti-cancer agent vinblastine sulphate). Results show that the molecules can be post-doped into the gels and the Raman data provide indications of the best conditions for detecting the substances absorbed in the gels. Spectroscopic results show that the drug molecules are released by the xerogel over a period of 10 days. These results are promising for the development of these materials as drug-release agents.
