Epidemiology of isolated foot burns in children presenting to a Queensland paediatric burns centre—a two-year study in warmer climate

Background: European studies of paediatric foot burns report scalds as the leading cause. Mechanisms of injury are different in warmer climates. We sought to characterize the mechanisms and outcomes of isolated foot burns in our population. Methods: Retrospective review of a prospectively collected database of all children aged 0–15 years presenting to a Queensland paediatric burns centre over a 26-month period. Non-parametric analyses such as the Mann-Whitney U and Pearson Chi-square were used. Results: There were 218 children with foot burns treated over a period of 2 years and 2 months of which 214 had complete records. There were significantly more boys than girls (n=134, 62.6% cf. n=80, 37.4%, p<0.0001). The leading mechanism of injury was a contact burn accounting for 63.1% (n=135) followed by scalds (23.8%, n=51). Friction, flame and chemical burns were a minority but were significantly deeper (p=0.03) and significantly more likely to require grafting (p=0.04) and scar management (p<0.0001) compared to contact and scald burns. Conclusions: In our population, contact burns are the most common mechanism of injury causing burns to the feet. The leading aetiology is campfire burns, which account for one-third of all burns to the feet. Prevention campaigns targeted at this population could significantly reduce the burden of morbidity from these burns. Friction, flame and chemical burns constitute a minority of patients but are deeper and more likely to require skin grafting and scar management.

The number of people affected by spinal cord injuries （SCI） ranges from 2.5 to 4 million worldwide. Traumatic SCI induces a primary injury due to initial mechanical impact that causes focal cellular and blood-spinal cord barrier damages. Subsequently, secondary injuries resulting from infiltration of peripheral monocytes, ischemia, edema,inflammation, glial scar and cystic cavities formation as well as excitotoxicity trigger cellular demise and prevent spontaneous axonal regrowth. Secondary damages, including aggregation of extracellular matrix protein at the lesion epicenter, thus greatly amplify impairments induced by the initial mechanical damage.

Detections of mefA, ermB, and mphA Macrolides Resistant Genes in Bacteria Isolated from Covid-19 Patients from Selected Health Facilities in Ibadan, Nigeria

Background: COVID-19 is a disease caused by Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2). Epidemiological data indicated that bacterial complications in COVID-19 would decrease clearance rate of the infecting agent and increase mortality rate. Macrolides such as Azithromycin are usually administered to COVID-19 patients as palliative treatments. Currently, a considerable number of bacterial strains have developed resistance to various antibiotics, especially macrolides. Resistance is reported to be due to possession of mefA, ermB, and mphA genes by Gram positive and Gram negative bacteria. Therefore, this study determined antibiotic resistance patterns and identify mefA, ermB and mphA macrolide-resistant genes in bacterial pathogens isolated from COVID-19 cases in Ibadan, Nigeria. Methods: 400 Nasopharyngeal samples were collected from symptomatic cases before antibiotic medication;structured questionnaires were administered to collect socio-demographic data of participants. Samples were cultured on Blood, Chocolate, MacConkey and Mannitol salt agar at 37°C for 48 hrs. Bacterial identification was performed using VITEK 2.0 ID cards and API 20E for Gram positive and negative bacteria respectively. Antibiotic Susceptibility Testing was performed using Kirby Bauer disc diffusion methods and VITEK 2.0 AST card kits. DNA of multidrug resistant bacterial isolates was extracted;resistant genes were determined using a polymerase chain reaction with specific primers. Amplified genes were detected using agarose gel electrophoresis. Results: 240 (60%) had bacterial growth and 97 (22.2%) yielded no growth. From the 240 bacterial isolates, 38 (15.83%) were multi-drug resistant including resistance to macrolides (Azithromycin) 20 (52.63%) of which were positive for either mefA or ermB, and none (0.0%) possess mphA gene;14 (36.8%) isolates had mefA gene, 10 (26.3%) isolates carried ermB gene. Local mechanical properties in aluminum cast components are inhomogeneous as a con- sequence of spatial distribution of microstructure, e.g., porosity, inclusions, grain size and arm spacing of secondary dendrites. In this work, the effect of porosity is investigated. Cast components contain voids with different sizes, forms, orientations and distributions. This is approximated by a porosity distribution in the following. The aim of this paper is to in- vestigate the influence of initial porosity, stress triaxiality and Lode parameter on plastic deformation and ductile fracture. A micromechanical model with a spherical void located at the center of the matrix material, called the representative volume element （RVE）, is de- veloped. Fully periodic boundary conditions are applied to the RVE and the values of stress triaxiality and Lode parameter are kept constant during the entire course of loading. For this purpose, a multi-point constraint （MPC） user subroutine is developed to prescribe the loading. The results of the RVE model are used to establish the constitutive equations and to further investigate the influences of initial porosity, stress triaxiality and Lode parameter on elastic constant, plastic deformation and ductile fracture of an aluminum die castin~ alloy.

Effects of meteorological conditions and topography on the bioaccumulation of PAHs and metal elements by native lichen(Xanthoria parietina)

The bioaccumulation of PAHs and metal elements in the indigenous lichens Xanthoria parietina was monitored during two years at a quarterly frequency,in 3 sites of contrasted anthropic *** impact of the meteorological factors(temperature,relative humidity,rainfall,wind speed)was first estimated through principal component analysis,and then by stepwise multilinear regressions to include wind *** pollutants levels reflected the proximity of atmospheric emissions,in particular from a large industrial *** humidity and mild temperatures,and in a lower extent low wind speed and rainfall,also favored higher concentration *** contributions of these meteorological aspects became minor when including wind direction,especially when approaching major emission *** bioaccumulation integration time towards meteorological variations was on a seasonal basis(1–2 months)but the wind direction and thus local emissions also relied on a longer time scale(12 months).This showed that the contribution of meteorological conditions may be prevalent in remote places,while secondary in polluted areas,and should be definitely taken into account regarding long-term lichen biomonitoring and inter-annual *** the same time,a quadruple sampling in each site revealed a high homogeneity among supporting tree species and *** resulting uncertainty,including sampling,preparation and analysis was below 30%when comfortable analytical conditions were ***,the occurrence of unexpected events such as a major forest fire,permitted to evaluate that this type of short,although intense,events did not have a strong influence on PAH and metals bioaccumulation by lichen.

It has been estimated that Alzheimer＇s disease （AD）, the most common form of dementia, will affect approximately 81 million individuals by 2040. To date, the actual cause and cascade of events in the progression of this disease have not been fully determined. Furthermore, there is currently no definitive blood test or simple diagnostic method for AD. Considerable efforts have been put into proteomic approaches to develop a diagnostic blood test, but to date these efforts have not been successful. More recently, there has been a stronger focus on lipidomic studies in the hope of increasing our understanding of the underlying mechanisms leading to AD and developing an AD blood test. It is well known that the strongest genetic risk factor for AD is the e4 variant of apolipoprotein E （APOE）. Evidence suggests that the ApoE protein, a major lipid transporter, plays a key role in the pathogenesis of AD, and its role in both normal and aberrant lipid metabolism warrants further extensive investigation. Here, we review ApoE-lipid interactions, as well as the roles that lipids may play in the pathogenesis of AD.

Is the soil seed bank a reliable source for passive restoration of bush-cleared semi-arid rangelands of South Africa?

Soil Amendments Improved Tomato Growth, Yield and Soil Properties

Different soil amendments have varying effects on crop growth, yield and soil properties. The study evaluated the effect of poultry manure, inorganic fertilizer (NPK), and biochar-based fertilizer (organo-yield) on tomato growth, yield and post-cropping soil properties. The experiment was laid out in a Randomized Complete Block Design (RCBD) with three replications. Data on tomato growth and yield were collected and soils were tested before and after the cropping seasons. Analysis of variance was performed on the data and the effect of soil amendments on tomato growth and yield was determined. The effect of soil amendments on post-production soil properties was tested by comparing soil properties before and after the cropping seasons with published critical values. Results showed that all amendments increased the growth and yield of tomato significantly (P 0.05) compared to the control (without soil amendments). Plant height was higher in plots applied with soil amendments as compared to the control, while stem girth was highest in poultry-manured plots. Poultry manure and organo-yield applied plants exhibited high tomato fruit number and yield compared to control plots. In terms of soil properties, poultry manure and organo-yield improved the soil’s physical and chemical properties. Organo-yield decreased the soil bulk density and increased the pH from 4.8 to 5.5, while, application of NPK only increased calcium content in the soil. The findings confirmed that soil amendments applied increased tomato growth, and yield and also improved soil bulky density and pH.

Effects of Health Record Computerization in the Context of the COVID-19 Pandemic in Burundi’ Hospitals: Propensity Score Matching

Introduction: A great number of software are currently used to digitalize the patient records in order to optimize the quality of services offered to patients. The objective of this study was to evaluate the effects of Electronic Health Records use in Burundi’s hospitals, taking into account the COVID-19 pandemic context. Methods: This was a quasi-experimental study based on difference in difference method. Ten district hospitals were included in the study, five of them had the Electronic Health Records and five of which did not yet have the Electronic Health Records. The hospital’s control group were chosen using the propensity score matching method. The period before the project’s implementation was 2014 and the period after were 2019 and 2020. Results: After 5 years of the Electronic Health Record’s implementation, the results showed an increase in outpatient consultation (70%), deliveries (more than 100%), caesarean sections (56%) and major surgeries (43%) indicators. The overall quality score of hospitals’ care had a regressive effect of 37% and the income from performance-based funding had an increase by 31%. The indicators which were affected by the context of the COVID-19 pandemic were especially outpatient consultation, caesarian section, income from performance-based funding decreased by 3%, 5% and 20% respectively. Conclusion: The effects of Electronic Health Records use are effective. As the COVID-19 pandemic impacted the hospital’s indicators negatively, the resilient strategies alongside the potential shocks are recommended.
