BACKGROUND There has been an increase in cases of inflammatory bowel disease(IBD)in recent *** is also greater access and availability of immunosuppressive and biological agents,which increase the risk of opportunistic infection despite improving the quality of life and promoting mucosal ***(TB)remains a public health problem,and it has a high incidence in several ***,knowledge of the risk of developing TB in patients with IBD is *** To evaluate the risk of active TB in patients with IBD under treatment from an endemic area in Latin *** A standard questionnaire included demographic variables,clinical aspects of IBD disease,history of active TB during treatment,active TB characteristics and evolution,initial screening and results and time from the start of anti-tumor necrosis factor alpha(TNFα)to TB *** Azathioprine,anti-TNFα and the combination of these two drugs were associated with a higher risk of active TB *** TNFα blockers increased the relative risk of developing active TB compared to other *** four multivariable models showed that the use of TNFα blockers alone or in combination with azathioprine was an important risk factor for the incidence of active *** adjustment for sex,age,type of IBD and latent TB,anti-TNFα with azathioprine increased the relative risk to 17.8 times more than conventional *** TB,which was diagnosed 3 mo after the start of anti-TNFα,was the most *** Treatment with anti-TNFα increased the risk of active TB in IBD patients from an endemic area in Latin *** risk was increased when anti-TNFα was combined with *** time from the beginning of the treatment to the active TB diagnosis suggests a new TB infection.

Background: Emerging evidence has recognized that anemia and iron deficiency are recurrent comorbidities in chronic heart failure (HF) and several trials have established that iron administration improves myocardial asset and clinical scenario in HF. Purpose: Recent acquisitions suggest that iron deficiency represents a concrete bias in the pathogenetic mechanism of chronic HF, so we have investigated the putative role of the hepcidin/ferroportin axis in the cardiovascular setting to advocate novel pharmacological and clinical approaches. Methods: Here, after an excursus on iron metabolism, we first reviewed the ongoing studies on novel iron targeted compounds. Then, we summarize large clinical interventional studies conducted on patient suffering from iron deficiency and HF which have tested the effects of drugging iron regard QoL, hospitalizations and cardiovascular death. Results: Novel compounds such as hepcidin agonist (PTG 300), synthetic human hepcidin (LJPC-401) and anti FPN (Vamifeport) are ongoing in iron overloaded patients, while the hepcidin blocker (PRS-080) is under investigation in anemic patients. Noteworthy, novel insights could arise from the results of a Phase IV interventional study regarding the modification of hepcidin pathway in a large cohort of HF patients (n = 1992) by sodium glucose cotransporter 2 inhibitors. To date, several studies highlight the beneficial effect of iron administration in cardiovascular setting and latest evidences consider hepcidin level as a novel biomarker of cardiac injury and atherosclerosis. Conclusions: We advocate that data from ongoing studies will suggest novel iron targeted therapies for diagnosis, prognosis and therapy transferable in selected heart failed patients.

Ecological Rehabilitation Proposal for Extracting Areas in Road Construction, Paraguay

This research work took into account two extracting areas that were used in the construction of the road between Pirayu and Yaguarun in Paraguay and describes the situation in the area taking into account the current related legislation. Signs of degradation have been observed, such as erosion in progress, compacted soil, scarce vegetative repopulation and lack of organic matter necessary to regenerate the flora, among others. Includes an analysis of the techniques applied in similar cases of degradation in other countries and those techniques that show more compatibility with the conditions of the areas under study have been selected; alternative rehabilitation techniques in road bank areas and surrounding surfaces are also recommended. The goal is that the selected areas will blend in with the landscape and will resume similar conditions to the surrounding landscape in a medium tenn. The most compatible techniques that were found were the reshaping of road banks in the degraded areas and the insertion of herbal species for the proposal for the implementation of bioengineering and soil improvements including the introduction of hardwood species after the insertion of herbal species.
