On the electrophysiology of the atrial fast conduction system:an uncertain quantification study

Cardiac modeling entails the epistemic uncertainty of the input parameters,such as bundles and chambers geometry,electrical conductivities and cell parameters,thus calling for an uncertainty quantification(UQ)*** the cardiac activation and the subsequent muscular contraction is provided by a complex electrophysiology system made of interconnected conductive media,we focus here on the fast conductivity structures of the atria(internoda. pathways)with the aim of identifying which of the uncertain inputs mostly influence the propagation of the depolarization ***,the distributions of the input parameters are calibrated using da.a available from the literature taking into account gender *** output quantities of interest(Qols)of medical relevance are defined and a set of metamodels(one for each Qol)is then trained according to a polynomial chaos expansion(PCE)in order to run a global sensitivity analysis with non-linear variance-based SoboF indices with confidence intervals evaluated through the bootstrap *** most sensitive parameters on each Qol are then identified for both genders showing the same order of importance of the model inputs on the electrical ***,the probability distributions of the Qols are obtained through a forward sensitivity analysis using the same trained *** results that several input parameters-including the position of the internoda. pathways and the electrical impulse applied at the sinoatrial node一have a little influence on the Qols ***-versa the electrical activation of the atrial fast conduction system is sensitive on the bundles geometry and electrical conductivities that need to be carefully measured or calibrated in order for the electrophysiology model to be accurate and predictive.

liver and the defects of cholesterol and bile acids biosynthesis: rare disorders many diagnostic pitfalls

In recent decades, biotechnology produced a growth of knowledge on the causes and mechanisms of metabolic diseases that have formed the basis for their study, diagnosis and treatment. Unfortunately, it is well known that the clinical features of metabolic diseases can manifest themselves with very different characteristics and escape early detection. Also, it is well known that the prognosis of many metabolic diseases is excellent if diagnosed and treated early. In this editorial we briefly summarized two groups of inherited metabolic diseases, the defects of cholesterol biosynthesis and those of bile acids. Both groups show variable clinical manifestations but some clinical signs and symptoms are common in both the defects of cholesterol and bile acids. The differential diagnosis can be made analyzing sterol profiles in blood and/or bile acids in blood and urine by chromatographic techniques(GC-MS and LC-MS/MS). Several defects of both biosynthetic pathways are treatable so early diagnosis is crucial. Unfortunately their diagnosis is made too late, due either to the clinical heterogeneity of the syndromes(severe, mild and very mild) that to the scarcity of scientific dissemination of these rare diseases. Therefore, the delay in diagnosis leads the patient to the medical observation when the disease has produced irreversible da.ages to the body. Here, we highlighted simple clinical and laboratory descriptions that can potentially make you to suspect a defect in cholesterol biosynthesis and/or bile acids, as well, we suggest appropriate request of the laboratory tests that along with common clinical features can help to diagnose these defects.

Extinction selectivity determines the direction of macroevolution,especially during mass extinction;however,its driving mechanisms remain poorly *** investigating the physiological selectivity of marine animals during the Permian-Triassic mass extinction,we found that marine clades with lower O2-carrying capacity hemerythrin proteins and those relying on O2 diffusion experienced significantly greater extinction intensity and body-size reduction than those with higher O2-carrying capacity hemoglobin or hemocyanin *** findings suggest that animals with high O2-carrying capacity obtained the necessary O2 even under hypoxia and compensated for the increased energy requirements caused by ocean acidification,which enabled their survival during the Permian-Triassic mass ***,high O2-carrying capacity may have been crucial for the transition from the Paleozoic to the Modern Evolutionary Fauna.

Comparisons and Contrasts between Asymmetry and Nestedness in Interacting Ecological Networks

We compare and contrast asymmetry and nestedness, two concepts used in the characterisation of the specialist-generalist balance in bipartite ecological interaction networks. Our analysis is relevant to mutualistic networks such as those consisting of flowering plants and pollinators, or fruiting plants and frugivores, or antagonistic networks such as those consisting of plants and herbivores, in an ecological community. We shall refer to the two sets of species in the bipartite network as plants and animals, the usual but not the only ecological situation. By asymmetry we mean either connectivity asymmetry or dependence asymmetry, which are essentially equivalent. Asymmetry expresses two attributes: generalists interact preferentially with specialists, and specialists avoid interacting with each other. Nested patterns, in principle, should express these same two features and one more: the presence of a core of interactions among generalists. We compute the full set of perfectly nested patterns that are possible in an L × L matrix with N interactions representing an ecological network of L plants and L animals, and point out that the number of nested arrangements grows exponentially with N. In addition, we analyse asymmetry for the full set of perfectly nested patterns, and identify extremes of asymmetry inside the universe of nested patterns. The minimal asymmetry is marked by a modular core of interactions between species that are neither specialists nor generalists. On the other hand, the case of maximal asymmetry is formed by a set of few generalists and many specialists with equal connectivity. The stereotypic case of nestedness with a core of interactions among generalists has intermediate asymmetry.

Hepatocellular carcinoma treated with transarterial chemoembolization: Dynamic perfusion-CT in the assessment of residual tumor

AIM: To asses the value of computed tomography (CT)-perfusion in the detection of residual hepatocellular carcinoma (HCC) vascularization after transarterial chemoembolization (TACE). METHODS: Thirty-two consecutive patients were pro-spectively included in this study. All patients had liver cirrhosis and a conf irmed HCC lesion which was treated with TACE. One month after treatment, perfusion measurements of treated lesions were carried out. The CTperfusion (CT-p) protocol was performed with 16 slice multidetector computed tomography which included the following parameters: 8 dynamic slices/scan per 40 scans after iv injection of 50 mL of iodinated contrast (350 mg/mL) at a flow rate of 6 mL/s. Treated lesions were evaluated using dedicated perfusion software, which generated a quantitative colour map of perfusion. The following parameters were considered: hepatic perfusion (HP), arterial perfusion (AP), blood volume (BV), hepatic perfusion index (HPI), and time to peak (TTP). Perfusion parameters were described with quartile values of their distribution and statistically analyzed. RESULTS: Perfusion parameters of the treated lesions could be quantitatively assessed using CT-p analysis. The presence of residual tumor tissue was observed in 13 of the 32 patients. The values of the perfusion parameters measured within the relapse tissue were: HP (mL/100 g per minute): median = 44.4 (1stqt = 31.3, 3rdqt = 55.8); BV (mL/100 g): median = 18.7 (1stqt = 11.5, 3rdqt = 22.5); AP (mL/min): median = 39.0 (1stqt = 36.5, 3rdqt = 61.3); HPI (%): median = 34.0 (1stqt = 30.4, 3rdqt = 38.9); TTP (s): median = 17.3 (1stqt = 15.8, 3rdqt = 26.5). With the use of the univariate paired Wilcoxon signed rank test, HP, AP and HPI were shown to be significantly higher (Pion. Animal and Plant Species in Antagonistic and Mutualistic Networks, a Multispecies Mean Field Model

We construct a dynamic system model of a web of NA?animal species interacting with NP plant species using a system of coupled differential equations. The model has a parameter which represents the effect of animal on plant species that can be positive (mutualistic interaction) or negative (antagonistic interaction). We work a Multispecies Mean Field Model MMFM in which the full set of NA and NP variables are reduced to a couple of an average animal and plant species. We study the conditions for existence of the MMFM and relate the result to the difference between animal and plant species in the network. We compare our results with empirical da.a from pollinators (mutualistic) and herbivorous insect (antagonistic) networks. We conclude by combining analytical and empirical work that antagonistic networks present fewer animal species in relation to plant species than mutualistic ones.

To the Editor：Airway management is a crucial skill for emergency physician, who＇s often called to deal with difficult airways and requests for quick, simple and effective responses, as the many factors responsible for difficulties might be enhanced by emergency setting.[1] We now have many rescue devices as the LMA, I-gel, but they do not provide a definitive airway, and recent studies evaluating the use of the videolaryngoscopes in emergency show conflicting results.

MCS （mechanical circulatory support） has shown great efficacy in treatment of respiratory mid cardiovascular diseases when conventional therapy is insufficient. The use of ECMO （extracolporeal membrane oxygenation） for the treatment of cardiogenic shock seconda.y to sepsis is still controversial. Studies have shown that ECMO should be considered as a valuable therapeutic option in cases of refractory cardiovascular dysfunction in severe septic patients, although more studies ale needed to consolida.e its efficacy. We report the case of two patients, without previous history of cardiopathy, subjected to temporary MCS, with use of ECMO for the treatment of refractory cardiogenic shock seconda.y to sepsis.

Anesthetic management of cytoreductive surgery and hyperthermic intraperitoneal chemotherapy procedures

Cytoreductive surgery and hyperthermic intraperitoneal chemotherapy procedure are performed with increasing frequency to treat patients with diffused peritoneal carcinomatosis. These procedures have showed to increase life expectancy in what was previously considered a "terminal condition". Anyway patients face major and life threatening derangements of their hemodynamic, respiratory and metabolic physiologic balance during the surgery and in the immediate postoperative period. Despite the need of an advanced organ monitoring and support all these derangements seem to be mild and short-lived when timely addressed, at least in the majority of patients. Intensive care physicians are involved in providing surveillance and organ support till the patient is effectively weaned after the operation. Moreover, the anesthesiologist as perioperative physician is involved in pain control, metabolic and nutritional support of this cohort of patients. This task can be challenging considering that part of the patients are already on a long list of pain control medication after previous surgery or chemotherapy. A malnourished state is common too and it is seconda.y to diffi cult feeding, wasting syndrome from the tumor and massive ascites. The last issue the anesthesiologists need to be aware of is the impact over the quality of life(Qo L) of this procedure. The patient's underlying pathology is unlikely to be defi nitively cured so no treatment is an acceptable choice. The possibility to withhold the treatments must be part of the consultation process like the discussion about the Qo L in the immediate, as well as in the long-term, after the operation. Careful monitoring and treatment of every aspect that can impact the Qo L must be taken and the efforts to be poured into an effective preservation of the Qo L must be doubled when compared with a patient scheduled for major abdominal surgery.
