Effectiveness of mindfulness-based interventions on the well-being of healthcare workers:a systematic review and meta-analysis

Background Growing evidence attests to the efficacy of mindfulness-based interventions(MBIs),but their effectiveness for healthcare workers remains *** To evaluate the evidence for MBIs in improving healthcare workers’psychological *** A systematic literature search was conducted on Medline,Embase,Cumulative Index for Nursing and Allied Health Literature,PsycINFO and Cochrane Central Register of Controlled Trials up to 31 August 2022 using the keywords‘healthcare worker’,‘doctor’,‘nurse’,‘allied health’,‘mindfulness’,‘wellness’,‘workshop’and‘program’.Randomised controlled trials with a defined MBI focusing on healthcare workers and quantitative outcome measures related to subjective or psychological well-being were eligible for inclusion.R V.4.0.3 was used for data analysis,with the standardised mean difference as the primary outcome,employing DerSimonian and Laird’s random effects *** of Recommendations,Assessment,Development and Evaluation framework was used to evaluate the quality of ***’s Risk of Bias 2 tool was used to assess the risk of bias in the included *** A total of 27 studies with 2506 participants were included,mostly from the USA,involving various healthcare *** such as stress reduction programmes,apps,meditation and training showed small to large effects on anxiety,burnout,stress,depression,psychological distress and job strain outcomes of the *** effects were also seen in self-compassion,empathy,mindfulness and ***,long-term outcomes(1 month or longer postintervention)varied,and the effects were not consistently *** MBIs offer short-term benefits in reducing stress-related symptoms in healthcare *** review also highlights limitations such as intervention heterogeneity,reduced power in specific subgroup analyses and variable study quality.

Causal relationship between circulating vitamin C and 25-hydroxyvitamin D concentrations and common mental disorders-a Mendelian randomization study

Mental disorders seriously affect people’s health and social *** Mendelian randomization(MR)study was designed to investigate the causal relationship between circulating vitamin C(VC)or 25-hydroxyvitamin D(25(OH)D)levels and mental *** data used for the MR analysis were derived from the summary genome-wide association studies(GWAS)database for VC and 25(OH)D and from the finn Gen consortium for fourteen mental *** on the inverse variance weighted(IVW)method,we found a potential causal association between circulating VC and anxiety disorders(IVW:OR=1.139,95%CI:1.023-1.269,P=0.018).However,no causal association was found between VC or 25(OH)D and other mental disorders(P>0.05).In the reverse MR analysis,individuals with Alzheimer’s disease was causally associated with higher concentrations of circulating VC(P=0.012),while individuals with anxiety disorders had a negative association between the concentrations of 25(OH)D(P=0.012).However,the current evidence does not support a causal relationship between VC or 25(OH)D and other mental *** addition,there was no causal association between circulating VC and 25(OH)D(P>0.05).Future studies are needed to confirm these findings and to elucidate the mechanisms of potential causality.

轮班工作与心脑血管疾病的孟德尔随机化研究

目的使用孟德尔随机化（Mendelian randomization, MR）方法评估轮班工作（shift work）与心脑血管疾病（cardiovascular and cerebrovascular diseases,CCVD）的因果关系。方法从英国生物样本库（UK biobank,UKBB）及欧洲血统最大的全基因组关联研究（GWAS）提取轮班和9个CCVD的汇总统计数据用于MR分析，并使用finn Gen生物库重复分析进行验证，之后进一步使用meta分析合并发现队列和复制队列以扩大样本量来获得更可靠的因果效应。同时，使用两步MR方法评估CCVD相关危险因素对轮班工作和CCVD之间因果效应的潜在中介作用。多种敏感性分析方法验证结果的可靠性。结果这项研究基于IVW方法显示轮班工作会增加心血管疾病（cardiovascular disease,CVD）(OR=1.06,95%CI:1.02～1.10,FDR=0.003)、冠心病（coronary heart disease）(OR=1.32,95%CI:1.10～1.60,FDR=0.007)、心肌梗死（myocardial infarction）(OR=1.64,95%CI:1.24～2.20,FDR=0.003)和心绞痛（angina pectoris）(OR=1.40,95%CI:1.15～1.70,FDR=0.003)的发病风险。水平多效性效应（MR-Egger截距和MR-PRESSO全局检验的P值均>0.05）对结果产生偏差的可能性较小。此外，原发性高血压（essential hypertension,EHT）、2型糖尿病（type 2 diabetes,T2DM）、身体质量指数（body mass index,BMI）、抑郁状态、当前吸烟部分介导了轮班工作对CVD、冠心病、心肌梗死及心绞痛的因果效应，其中BMI对CVD、冠心病、心肌梗死的调节比分别为81.7%、47.03%、45.04%;EHT对冠心病、心肌梗死、心绞痛的调节比分别为42.87%、40.72%、57.05%;T2DM对CVD、冠心病、心肌梗死、心绞痛的调节比分别为12.51%、7.58%、5.27%、8.39%；抑郁状态对CVD、冠心病、心肌梗死、心绞痛的调节比分别为237.12%、26.00%、22.06%、32.60%；当前吸烟对CVD的调节比为11.42%。结论这项MR研究提供了可靠的证据支持轮班工作增加CVD、冠心病、心肌梗死及心绞痛的发病风险，EHT、T2DM、BMI、抑郁状态及当前吸烟部分介导了这种作用。

Multilayer network analyses as a toolkit for measuring social structure

The formalization of multilayer networks allows for new ways to measure sociality in complex social systems,including groups of *** same mathematical representation and methods are widely applicable across fields and study systems,and a network can represent drastically different types of *** such,in order to apply analyses and interpret the results in a meaningful way the researcher must have a deep understanding of what their network is representing and what parts of it are being measured by a given *** social networks can represent social structure with more detail than is often present in single layer networks,including multiple"types"of individuals,interactions,or relationships,and the extent to which these types are *** networks can also encompass a wider range of social scales,which can help overcome complications that are inherent to measuring *** this paper,I dissect multilayer networks into the parts that correspond to different components of social structures.I then discuss common pitfalls to avoid across different stages of multilayer network analyses-some novel and some that always exist in social network analysis but are magnified in multi-layer *** paper serves as a primer for building a customized toolkit of multilayer network analyses,to probe components of social structure in animal social systems.

Wildfire events are increasing globally which may be partly associated with climate change,resulting in significant adverse impacts on local,regional air quality and global *** September 2020,a small wildfire(burned area:36.3 ha)event occurred in Souesmes(Loiret-Cher,Sologne,France),and its plume spread out over 200 km on the following day as observed by the MODIS *** on measurements at a suburban site(~50 km northwest of the fire location)in Orléans and backward trajectory analysis,young wildfire plumes were *** increases in gaseous pollutants(CO,CH_(4),N_(2)O,VOCs,etc.)and particles(including black carbon)were found within the wildfire plumes,leading to a reduced air *** factors,defined as EF(X)=ΔX/ΔCO(where,X represents the target species),of various trace gases and black carbon within the young wildfire plumes were determined accordingly and compared with previous *** in the ambient ions(such as ammonium,sulfate,nitrate,chloride,and nitrite in the particle-and gasphase)and aerosol properties(e.g.,aerosol water content,aerosol p H)were also quantified and ***,we estimated the total carbon and climate-related species(e.g.,CO_(2),CH_(4),N_(2)O,and BC)emissions and compared them with fire emission *** biomass burning emission inventories have uncertainties in estimating small fire burned areas and *** instance,we found that the Global Fire Assimilation System(GFAS)may underestimate emissions(e.g.,CO)of this small wildfire while other inventories(GFED and finn)showed significant *** that it is the first time to record wildfire plumes in this region,related atmospheric implications are presented and discussed.

Pregnancy and diabetic retinopathy-considerations for evaluation and treatment:a review

The prevalence of diabetic retinopathy(DR)continues to increase in pregnant females;these individuals are also at a higher risk of disease *** lack of evidence regarding the safety and efficacy of current treatment options in pregnancy makes disease management particularly *** pregnant women with diabetes should have a prenatal DR screening,as well as receive counseling regarding the progression and management of DR during *** blood glucose and blood pressure control should be *** patients with proliferative diabetic retinopathy(PDR)in the absence of visually significant diabetic macular edema(DME),panretinal photocoagulation(PRP)remains a safe and effective treatment *** significant DME can be treated with focal laser if areas of focal leakage are identified in the macula on fluorescein angiogram,intravitreal steroids or anti-vascular endothelial growth factor(VEGF)agents,The theoretical risk of anti-VEGF agents to the fetus should be considered and the patients should be extensively counselled regarding the risks and benefits of initiating anti-VEGF therapy before initiating *** the decision is made to treat with anti-VEGF agents,Ranibizumab should be the agent of *** conclusion,ophthalmologists should make treatment decisions in pregnant patients with DR on a case-by-case basis taking into consideration disease severity,risk of permanent threat to vision,gestational age,and patient preferences.
