The Democratic Republic of Congo is one of the countries with a high preva-lence of multidrug-resistant tuberculosis (MDR-TB) in Africa and worldwide. This study aims to describe the trends and level of screening since 2013. This is a retrospective descriptive study based on quarterly reports from the provinces. The number of cases, past histories, age, sex and HIV Co-infection was identi-fied. It emerges that the number of cases increases significantly but does not reach the forecasts yet (912 cases of MDR/RR and XDR diagnosed in 2017 out of 3948 cases expected with a gap of 77%). Patients with a history of TB pre-dominate (63.5% in 2017);but the proportion of patients with no history of TB increases (36.5% in 2017). This suggests high rates among contacts. Women (37.8%) and children under 15 (2.7%) are less represented. TB/HIV Co-infection is 12% as for sensitive cases. Efforts still need to be made to improve screening.

Setting: Kinshasa, capital of Democratic Republic of Congo, has the high rate of multidrug resistant tuberculosis (MDR-TB) which is associated with poor treatment outcomes until 2013. A new approach was needed. Objectives: To implement a new strategy in order to improve treatment outcomes. Design: A retrospective cohort study reviewing all the patients diagnosed MDR Tuberculosis between 2014 and 2017. The study was conducted in the National Tuberculosis Program (NTP) framework comparing the short regimen and the WHO standard regimen. Results: From 1246 patients diagnosed RR/TB, 1073 were included in the analysis: 948 on shorter regimen, and 125 on WHO standard regimen. The strategy was based on patient-centered care. In the short regimen group, 62.7% were male, 61.4% were age 25 - 44 years, 52.6% had previous history of TB, 39.3% underweight, 12.5% HIV positive. The median time from diagnosis to treatment was 19 days (0 - 163). In the long regimen group, 75% were male, 37.6% were age 14 - 44 years, 61.6% underweight, 18.4% HIV positive. The median time from diagnosis to treatment was 19 days (0 - 114). Favorable outcomes represented 81.9% in the short regimen group versus 72% in the long regimen group. Death and loss to follow-up were more observed in long regimen group (27.2% versus 15.4%). Factors associated with unfavorable outcomes in the short regimen group included sex, age ≥ 45 years, previous TB history, HIV status, delay to begin treatment. For the long regimen, the factors age and delay emerged, underweight and HIV were borderline. Drug adverse events were reported respectively in 43.5% and 42.4% for short and long regimen;with gastrointestinal disturbances, vestibular troubles, ototoxicity, arthralgia and anemia as the most common in the 2 groups. Contribution of Agriculture in the Enhancement of Refugees Livelihoods in Nakivale Settlement

Uganda has significantly continued to receive a large number of refugees in the region and this is positively correlated with its proportional increase in peace and stability. The presence of favorable climatic conditions enhances agricultural productivity and this has triggered redirecting refugees into agricultural production as the major source of livelihood. However, in Ugandan setting, it has not yet been clearly established how agriculture has practically enhanced the livelihoods of the refugees. The study therefore aimed at identifying the agricultural activities practiced by refugees living in Nakivale refugee settlement, identifying the challenges faced by refugees involved in agriculture and their possible solutions as well as examining the contribution of agriculture towards livelihoods of refugees in Nakivale Refugee settlement. The study utilized a case-study approach to analyze the contribution of agriculture in the enhancement of livelihoods in the context of a single settlement and a sample size of 80 respondents was chosen using simple random sampling and an interview guide was used to collect primary data from the respondents. It was revealed from the study that crop production is the main agricultural activity practiced by refugees living in Nakivale refugee settlement in a bid to improve on their food security and household incomes. The study further revealed that climate change, diseases/parasites and diseases, limited land and low prices for the produce are the most serious challenges facing refugees’ farmers. It is concluded that food security in Nakivale refugee settlement is rather influenced by agricultural production and the associated dynamics that ultimately determine the well-being of the refugee communities. It is important to appreciate that any variation in factors of production, weather and produce prices will determine livelihood status of the community for such a specific period of time. Despite the uniquely high thermal conductivity of graphene is well known,the exploitation of graphene into thermally conductive nanomaterials and devices is limited by the inefficiency of thermal contacts between the individual nanosheets.A fascinating yet experimentally challenging route to enhance thermal conductance at contacts between graphene nanosheets is through molecular junctions,allowing covalently connecting nanosheets,otherwise interacting only via weak Van der Waals *** the bare existence of covalent connections,the choice of molecular structures to be used as thermal junctions should be guided by their vibrational properties,in terms of phonon transfer through the molecular *** this paper,density functional tight-binding combined with Green's functions formalism was applied for the calculation of thermal conductance and phonon spectra of several different aliphatic and aromatic molecular junctions between graphene *** of molecular junction length,conformation,and aromaticity were studied in detail and correlated with phonon tunnelling *** theoretical insight provided by this work can guide future experimental studies to select suitable molecular junctions,in order to enhance the thermal transport by suppressing the interfacial thermal *** is attractive for various systems,including graphene nanopapers and graphene polymer nanocomposites,as well as related *** a broader view,the possibility to design molecular junctions to control phonon transport currently appears as an efficient way to produce phononic devices and controlling heat management in nanostructures.

Background: Alpha-synuclein (α-syn) exhibits pathological misfolding in many human neurodegenerative disorders. We previously showed that α-syn is arginylated in the mouse brain and that lack of arginylation leads to neurodegen-eration in mice. Methods: Here, we tested α-syn arginylation in human brain pathology using newly derived antibodies in combina-tion with Western blotting, biochemical assays, and experiments in live neurons. Results: We found that α-syn was arginylated in the human brain on E46 and E83, two sites previously implicated in α-syn pathology and familial cases of Parkinson’s disease. The levels of arginylation in different brain samples ranged between ~ 3% and ~ 50% of the total α-syn pool, and this arginylation nearly exclusively concentrated in the subcellu-lar α-syn fraction that sedimented at low centrifugation speeds and appeared to be simultaneously targeted by multi-ple posttranslational modifications. Arginylated α-syn was less susceptible to S129 phosphorylation and pathological aggregation in neurons. The arginylation level inversely correlated with the overall α-syn levels and with patient age, suggesting a possible causal relationship between arginylation decline and α-syn-dependent neuropathology. Conclusion: We propose that α-syn arginylation constitutes a potential neuroprotective mechanism that prevents its abnormal accumulation during neurodegeneration and aging in the human brain.

A 43-year-old woman with an associated history of gynecological pathology and breast cancer with only one cryopreserved embryo wished to be a *** factors that influenced the success of the pregnancy in this case were analyzed. Favorable factors included: triple positive breast cancer [positive hormone receptors and positive human epidermal growth factor receptor 2],which is more hormosensitive and chemosensitive; absence of metastasis; correct endometrium preparation; and the patient's optimistic attitude and strict health habits. In contrast, the factors against success were: breast cancer; adjuvant breast cancer therapy gonadotoxicity; the age of the patient(> 40-year-old);endometriosis; ovarian cyst; hydrosalpinx; submucosal fibroids and the respective associated surgery done for the above-mentioned pathology(all resolved prior to the embryo transfer); and a low quantity of ovules(low ovarian reserve) after ovarian stimulation. This is a very special clinical case of a patient with theoretically low pregnancy success probability due to the consecutive accumulation of gynecological and oncological pathologies, who nonetheless became pregnant and delivered a full-term infant and was able to provide adequate breastfeeding.

Poor spermatogenesis in patients with inflammation of the genital tract is associated with scrotal *** patients can benefit from acupuncture *** conducted a study to verify whether the influence of acupuncture treatment on sperm output in patients with low sperm density is associated with a decrease in scrotal *** experimental group included 39 men who were referred for acupuncture owing to low sperm *** control group,which comprised 18 normal fertile men,was used to define a threshold(30.5℃)above which scrotal skin temperature was considered to be ***,34 of the 39 participants in the experimental group initially had high scrotal skin temperature;the other five had normal *** skin temperature and sperm concentration were measured before and after acupuncture *** five patients with initially normal scrotal temperatures were not affected by the acupuncture *** treatment,17 of the 34 patients with hyperthermia,all of whom had genital tract inflammation,had normal scrotal skin temperature;in 15 of these 17 patients,sperm count was *** the remaining 17 men with scrotal hyperthermia,neither scrotal skin temperature nor sperm concentration was affected by the *** 90%of the latter patients suffered from high gonadotropins or mixed etiological *** sperm count in patients with inflammation of the genital tract seems to be associated with scrotal hyperthermia,and,consequently,acupuncture treatment is recommended for these men.
