Biliary complications after liver transplantation:current perspectives and future strategies

Importance:Liver transplantation(LT)is a life-saving therapy for patients with end-stage liver disease and with acute liver failure,and it is associated with excellent outcomes and survival rates at 1 and 5 *** incidence of biliary complications(BCs)after LT is reported to range from 5%to 20%,most of them occurring in the first three months,although they can occur also several years after ***:The aim of this review is to summarize the available evidences on pathophysiology,risk factors,diagnosis and therapeutic management of BCs after *** Review:a literature review was performed of papers on this topic focusing on risk factors,classifications,diagnosis and treatment Findings:Principal risk factors include surgical techniques and donor’s characteristics for biliary leakage and anastomotic biliary strictures and vascular alterations for non-anastomotic biliary *** is the gold standard both for intra-and extrahepatic BCs,while invasive cholangiography should be restricted for therapeutic uses or when MRCP is *** treatment,endoscopic techniques are the first line of treatment with success rates of 70-100%.The combined success rate of ERCP and PTBD overcome 90%of *** leaks often resolve spontaneously,or with the positioning of a stent in ERCP for major bile leaks Conclusions and Relevance:BCs influence morbidity and mortality after LT,therefore further evidences are needed to identify novel possible risk factors,to understand if an immunological status that could lead to their development exists and to compare the effectiveness of innovative surgical and machine perfusion techniques.

BACKGROUND Coronavirus disease(COVID)is a new and highly contagious infectious disease caused by the coronavirus(COVID-19 or severe acute respiratory syndrome coronavirus 2).There is limited data regarding the incidence and management of COVID-19 in immunocompromised patients’*** the pre-COVID-19 era,these patients were already at an increased risk of developing opportunistic *** often manifested with atypical *** SUMMARY We report another case of uneventful COVID-19 pneumonia in a 58-year old male who was 18 mo’post liver *** received tacrolimus monotherapy since July *** clinical manifestations included only epigastric pain radiating to the right hypochondrium,nausea and *** had no fevers,cough,shortness of breath,anosmia or dysgeusia even if the chest computed tomography scan revealed an extension of the multiple patchy ground-glass density shadows to the upper lobe of the left lung *** was hospitalised and received a course of oral chloroquine(200 mg×3 per day)for a period of 10 ***,the COVID 19 infection was uneventful though there were no modifications to his tacrolimus *** was successfully *** performed subsequent follow-up via *** In light of the current pandemic,it is even more important to identify how the liver recipient’s patients present and are managed,especially for immunosuppression treatment.

Peutz-Jeghers syndrome(PJS)is a rare autosomal dominant disorder characterized by mucocutaneous pigmentation and gastrointestinal(GI)hamartomatous polyposis and is associated with an increased risk of gastrointestinal,breast,gynecologic and other extra-GI *** serine/threonine kinase 11(STK11)gene has been identi-fied as a pathogenic factor in ***11 is a tumor sup-pressor gene located on chromosome 19p13.3 and includes 9 coding exons.1 The STK11 protein is composed of 433 amino acids(aa)and comprises a kinase catalytic region(aa 49e309)as well as N-and C-terminal regulatory domains.

Acinar cell carcinoma(ACC)is a rare malignant epithelial neoplasm accounting for 1%-2%of all pancreatic exocrine neoplasm,affecting more frequently man with an age between 50 and 70 *** patients present with nonspecific symptoms,which may give rise to difficulties in clinical diagnosis[1].ACC can manifest with diarrhea,weight loss,abdominal pain and,in up to 10%-15%,with lipase hypersecretion syndrome,characterized by elevated lipase production,diffuse subcutaneous fat necrosis and polyarthralgia[1,2].Biliary obstruction and jaundice are infre-quent clinical manifestations,unlike ductal adenocarcinoma,due to pushing rather than infiltrating growth of ACC.

Polysaccharide composition during cotton seed fibre development:temporal differences between species and in different seasons

Background:Cotton seed fibres are long single-celled epidermal trichomes that first appear on the surface of the ovule at anthesis and then elongate rapidly over a period of 15-25 days until a secondary cell wall(SCW)begins to develop through a rapid increase in the deposition of microfibrillar cellulose between the plasma membrane and the primary cell wall that eventually terminates *** measurements of the different polysaccharide components in both wall types over time and how they influence fibre quality can direct studies involved in enhanc-ing fibre properties for yarn quality through cell wall manipulation or molecular ***:A detailed chemical analysis of cell wall composition by differential solvent fractionation was used to identify the range of polysaccharides present in mature cotton fibres and used to validate a simpler total cell wall monosaccharide linkage analysis protocol for wall compositional *** of fibres from 5 days post-anthesis through maturity for three cultivated species,Gossypium hirsutum,***,and ***,showed the dynamic nature of cell wall polysaccharide composition through fibre development and that it progressed differently for each *** grown in the glasshouse during either autumn to winter or spring to summer and within each species had fibre qualities and temporal aspects of cell wall development that were different for each ***,the timing of the deposition of the SCW was delayed in winter grown plants and appeared to influence key fibre qual-ity ***:These results suggest that the temporal aspects of cell wall polysaccharide biogenesis during fibre development influence final fibre quality,and this timing is determined by both genetic and environmental *** onset of SCW synthesis appears to be a critical factor coinciding with termination of fibre elongation and specify-ing the duration of wall thickening that then affects fibre le
