Background: Bot. vert.cal and t.ansverse sect.ons are used in t.e hist.logic int.rpret.t.on of alopecia biopsy specimens. Alt.ough a combinat.on of t.e t.o may be opt.mal, t.e pat.ologist.is frequent.y only provided wit. a single specimen. Even t.ough t.e t.end in recent.years has been t.ward t.ansverse sect.ons in t.is set.ing, we are not.aware of any published dat. direct.y comparing t.e t.o met.ods. Met.ods: One hundred t.o consecut.ve archived hair biopsy accessions t.at.demonst.at.d comparable vert.cal and t.ansverse sect.ons were examined t.ice, each t.me in a random order. t.e pat.ologist.s int.rpret.t.on based only on t.e vert.cal sect.ons and an int.rpret.t.on based only on t.e t.ansverse sect.ons were compared wit. t.e original biopsy report. which had been based on t.e combinat.on of vert.cal and t.ansverse sect.ons. Result.: In 76 cases, all 3 diagnoses were concordant.(ie, t.e diagnosis made wit. vert.cal sect.ons alone, t.e diagnosis made wit. t.ansverse sect.ons alone, and t.e original diagnosis were all in agreement.. In 2 cases, neit.er t.e diagnosis made wit. vert.cal sect.ons alone nor t.e diagnosis made wit. t.ansverse sect.ons alone were in full agreement.wit. t.e original diagnosis. In 20 cases, only t.e diagnosis made wit. vert.cal sect.ons was concordant.wit. t.e original diagnosis. In 4 cases, only t.e diagnosis made wit. t.ansverse sect.ons alone was concordant.wit. t.e original diagnosis. Limit.t.ons: Our pract.ce is heavily weight.d t.ward scarring alopecia, and t.e result. of our st.dy may not.be applicable t. pract.ces weight.d t.ward ot.er forms of alopecia. Because t.e cases had been signed out.over a period of several years, t.e nomenclat.re for some ent.t.es changed. For t.e purposes of our st.dy, we count.d t.e diagnoses of follicular degenerat.on syndrome and idiopat.ic pseudopelade t. be subt.pes of (and concordant.wit.)a diagnosis of cent.al cent.ifugal cicat.icial alopecia. In some cases, a definit.ve diagnosis was not.possible

Proliferat.ons of Langerhans cells can be hist.logically divided int. cyt.logically benign Langerhans cell proliferat.ons, which include t.e clinical syndromes of Langerhans cell hist.ocyt.sis, and cyt.logically malignant.Langerhans cell sarcoma. We report.a Langerhans cell sarcoma in a 33-year-old male t.at.arose on t.e post.rior t.igh wit. subsequent.regional lymph node involvement. Convent.onal microscopic, immunohist.chemical, and ult.ast.uct.ral analysis confirmed Langerhans cell different.at.on. Aberrant.CD31 expression, similar t. t.at.described previously in Langerhans cell hist.ocyt.sis, was prominent.in t.is t.mor, possibly enhancing it. migrat.ry capabilit.es.
