Ethical issues in primary (health) care (PC) are common and are confronted daily. In the scarce literature that was available, Zoboli and silva developed an instrument (IPE-APS) for perceiving ethical conflicts. Two empirical ex post facto studies to gather psychometric evidence for the instrument were outlined. In the first study, 88 nurses and physicians from Brazil’s Family Health (saúde da Família—SF) responded to the IPE-APS, and the results confirmed its validity and accuracy. In total, 14 items were grouped in Factor I (relationship/responsibility), which was described as having human rights and deontological nature (Cronbach’s alpha = 0.92), and 11 items were grouped in Factor II, which was described as having a labor structure nature (Cronbach’s alpha = 0.90). In the second study, 207 SF professionals in Goiania, Goiás responded to the IPE-APS. The results showed that Factor I had a high factorial congruence index (0.98), which revealed high similarity and showed that the instrument displayed practical validity to identify and assess ethical problem perception in PC.

Neisseria meningitidis is a gram-negative diplococcus which causes invasive disease. The most frequent clinical manifestations caused by infection with this pathogen, are meningitis and fulminant septic shock. More benign forms with a subacute or chronic pattern are rare but still can evolve to meningitis and have a fatal outcome. This entity is difficult to recognize, as febrile erythema with otherwise good general condition may simulate other diseases, namely viral infections. The authors reported the cases of two healthy young adults who were misdiagnosed with a viral infection. What they really had was a meningococcemia without meningitis presenting with mild and unspecific symptoms, such as, intermittent fever and a discrete macular rash. We pretend to highlight this rare, unspecific and heterogeneous symptoms which can provoke negligence of a potentially life threatening condition.

Background: In the last 6 months, cases of acute hemorrhagic fever (AHF) have been recorded in sao Tome and Principe (STP). Objective: To identify the clinical, epidemiological and laboratory parameters associated with AHF cases found in patients hospitalized in STP. Methods: descriptive and quantitative analysis of AHF cases hospitalized in STP in the period december 2021 to May 19, 2022 through the variables: demographic data;clinical data;laboratory data and clinical outcome. Results: Analyzed 18 of 22 AHF cases, 50% of them male, mean age 33.7 years, 85.7% residing in the 2 largest districts of the country, 66.7% rural workers, administrative near maritime areas, sailors and domestics. 66.7% were healthy individuals. ≥50% had a fever, asthenia/weakness, myalgia, headache, lethargy, nausea, vomiting, and diarrhea. 22.2% to 33.3% had retro-orbital pain, abdominal pain, decreased consciousness, dyspnea/hypoxia, and cough. In a smaller register (5.6% - 11.1%), exanthema, convulsion, arthralgia, low back pain, chills, and chest pain. Bleeding from the digestive tract was found in 72.2%, followed by vaginal (33.3%), urethral (27.8%), nasal (22.2%), and oral (16.7%). 50% had laboratory confirmation for dengue, and another 50% with suspected clinical diagnosis without laboratory confirmation of the etiologic agent. despite hemorrhage, 66.7% of confirmed cases were hemodynamically non-severe, and 88.9% of suspected cases were severe. Coinfection with malaria is evidenced in 11.1% of cases. 72.2% recovered and 27.8% died (deaths in 55.5% of suspected cases). Case deaths were male (100%), resident foreigners (40%), tool store employees, sailors and students, healthy (80%), and residing in the largest district in the country (60%). All deaths were hospitalized in serious condition, 80% were hospitalized for hemorrhage and shock, with bleeding arising between day 4 - 5 of illness in 60% and hospitalization on day 5 of illness. 60% died within ≤24 hours of hospitalization. 80

Elderly Feeding and Nutrition during Hospitalization: An Integrative Review

Objective: to search in the current literature the elements that exert an influence on feeding and nutrition in hospitalized aged people. Method: the following strategies were used for the integrative review stages: research question (Population or Patients;Exposure;Outcomes);analysis flowchart (Preferred Reporting Items for Systematic Reviews and Meta-analyses);and levels of evidence corresponding to the studies (Oxford Centre for Evidence-Based Medicine). The searches were conducted in the MEDLINE/PubMed, Biblioteca Virtual em saúde, Embase, CINAHL and Scopus databases. Results: the initial search yielded 1808 studies, of which 34 comprised the analysis corpus after applying the inclusion/exclusion criteria. The assessments corresponding to nutritional status and to the risk factors for hospitalized aged people are fundamental in defining the diet. An association was found between inadequate energy intake, lack of appetite, infections, malignity, delirium and need for assistance in feeding. Dysphagia affected nutritional status and was associated with multimorbidities, cognitive impairment, malnutrition, higher dependence for the activities of daily living, and greater care need. Nutritional support improved the perioperative nutritional status, in addition to reducing the hospitalization times and the number of infectious complications. Conclusion: the main disorder that hinders meeting the nutritional needs is dysphagia, and is associated with aged people’s multimorbidity and dependence. The results provide geriatric and gerontological knowledge about elderly nutrition, in addition to targeting preventive and intervention treatments and care during hospitalization. .

Skin is widely used as a drug delivery route due to its easy access and the possibility of using relatively painless methods for the administration of bioactive ***,the barrier properties of the skin,along with its multilayer structure,impose severe restrictions on drug transport and ***,bioengineered models aimed at emulating the skin have been developed not only for optimizing the transdermal transport of different drugs and testing the safety and toxicity of substances but also for understanding the biological processes behind skin *** though in vivo research is often preferred to study biological processes involving the skin,in vitro and ex vivo strategies have been gaining increasing relevance in recent ***,there is a noticeably increasing adoption of in vitro and ex vivo methods by internationally accepted ***,microfluidic organ-on-a-chip devices are nowadays emerging as valuable tools for functional and behavioural skin *** in miniaturization,automation and reliability still need to be addressed in order to create skin models that can predict skin behaviour in a robust,high-throughput manner,while being compliant with regulatory issues,standards and *** this review,skin models for transdermal transport,wound repair and cutaneous toxicity will be discussed with a focus on high-throughput *** microfluidic strategies driven by advancements in microfabrication technologies will also be revised as a way to improve the efficiency of existing models,both in terms of complexity and throughput.

There is much literature on the effects of fine particulate matter (PM2.5) on respiratory and cardiovascular health. However, few studies have evaluated the impact of PM2.5 on a population living in the vicinity of a massive deposit of mine tailings. A longitudinal panel study was performed to evaluate the association between exposure to PM2.5 and acute effects on lung function in schoolchildren from November 2012 to May 2013. Ambient levels of PM2.5 and its metal composition were measured. Lung function was evaluated using spirometric testing. Associations were quantified using GEE multilevel analysis controlling for confounders by using different lag time periods. The chemical characterization of PM2.5 had high levels of S > Na > Cl > Ca > Si > Fe > Al > Mg > K > Cu > Ti > and Zn, which would be associated with metals present in tailings. We found a negative association between the temporal variation of PM2.5 and changes in lung function specifically on forced vital capacity. Our results suggest that schoolchildren exposed to fine particulate matter from tailings deposited in the bay of Chanaral have their forced vital capacity decreased, which would affect their present and future lung development, increasing the risk of developing chronic respiratory diseases.
