Epidemiology of work-related hand and wrist injuries in a referral center: A descriptive study

BACKGROUND Occupational hand and wrist injuries(OHWIs)account for 25%of work-related accidents in low-and middle-income *** Colombia,more than 500000 occupational accidents occurred in 2021,and although the rate declined to less than 5%in 2020 and 2021,at least one in four accidents involved a hand or wrist *** To describe the OHWIs in workers seen at the emergency room at a second-level hospital in *** An observational study was performed using data from workers who experienced OHWIs and attended a second-level hospital,between June,2020 and May,*** overall frequency of OHWIs,as well as their distribution by sociodemo-graphic,clinical,and occupational variables,are ***,association patterns between sex,anatomical area(fingers,hand,wrist),and type of job were analyzed by correspondence analysis(CA).RESULTS There were 2.101 workers treated for occupational accidents,423(20.3%)were cases of OHWIs,which mainly affected men(93.9%)with a median age of 31 years and who worked mainly in mining(75.9%).OHWIs were more common in the right upper extremity(55.3%)and comprised different types of injuries,such as contusion(42.1%),laceration(27.9%),fracture(18.7%),and crush injury(15.6%).They primarily affected the phalanges(95.2%),especially those of the first finger(25.7%).The CAs showed associations between the injured anatomical area and the worker’s job that differed in men and women(explained variance>90%).CONCLUSION One out of five workers who suffered occupational accidents in Cundinamarca,Columbia had an OHWI,affecting mainly males employed in *** occupational profile is likely to lead to prolonged rehabilitation,and permanent functional *** results might be useful for adjusting preventive measures in cluster risk groups.

The Role of Hydropower in Climate Change Mitigation and Adaptation： A Review

Hydropower is a clean,renewable,and environmentally friendly source of *** produces 3930(TW·h)·a–1,and yields 16% of the world's generated electricity and about 78% of renewable electricity generation(in 2015).Hydropower and climate change show a double *** the one hand,as an important renewable energy resource,hydropower contributes significantly to the avoidance of greenhouse gas(GHG) emissions and to the mitigation of global *** the other hand,climate change is likely to alter river discharge,impacting water availability and hydropower *** contributes significantly to the reduction of GHG emissions and to energy supply *** with conventional coal power plants,hydropower prevents the emission of about 3 GT CO2 per year,which represents about 9% of global annual CO2 *** projects may also have an enabling role beyond the electricity sector,as a financing instrument for multipurpose reservoirs and as an adaptive measure regarding the impacts of climate change on water resources,because regulated basins with large reservoir capacities are more resilient to water resource changes,less vulnerable to climate change,and act as a storage buffer against climate *** the global level,the overall impact of climate change on existing hydropower generation may be expected to be small,or even slightly ***,there is the possibility of substantial variations across regions and even within *** conclusion,the general verdict on hydropower is that it is a cheap and mature technology that contributes significantly to climate change mitigation,and could play an important role in the climate change adaptation of water resource ***,careful attention is necessary to mitigate the substantial environmental and social *** more than a terawatt of capacity could be added in upcoming decades.

Background The quality of cotton fiber determines its value in the textile market, influencing agricultural profitability and the efficiency of textile processing. The selection of genotypes with superior fibers is a key factor for genetic improvement programs seeking adaptability and sustainability in the face of climate change. This demonstrates the strategic importance of this plant for sustainable agriculture and the global textile industry. The objective of this research was to decipher the fiber quality of Gossypium barbadense var. brasiliensis in the native Amazonian communities of La Convención, Cusco-Perú, and to evaluate other critical aspects of native cotton that have not yet been identified. The methodology included non-probability sampling for accessibility, qualitative and quantitative analyses, and multivariate analyses. The fiber length(mm), micronaire index(maturity/fineness), fiber strength(gf/tex), length uniformity index(%), fiber elongation(%), maturation index(%), and short fiber index(%) were the fiber characteristics evaluated using the HVI method in cotton *** Cotton accessions collected from Koribeni(Gossypium spp.) and Shivankoreni(Gossypium barbadense var. brasiliensis) stood out for their fiber quality properties, especially length, strength, and uniformity, which highlights their relevance for advanced textile applications and potential for use in plant genetic improvement *** These findings reinforce the need to conserve and study these native cotton accessions from the Peruvian Amazon region, which can offer promising perspectives for the textile industry and agricultural biodiversity.

Vestibular and cochlear dysfunction in aging:Two sides of the same coin?

Objective:Nonspecific complaints of hearing loss,vertigo,imbalance,and instability,without a defined etiology,are very prevalent in the elderly population,with a great impact on morbidity and mortality in this age *** objectives of this study were to verify whether there is age-related vestibular dysfunction and to test the association of vestibular dysfunction with presbycusis in the elderly ***:Original retrospective analytical cross-sectional study,carried out with 80 patients who underwent a videonystagmography and complete audiometric evaluation due to nonspecific vestibular complaints,without a specific vestibular disorder *** were selected and divided into two distinct age groups(group A:>60 years;group B:18-50 years)and,in both groups,we analyzed the caloric tests and the pure-tone ***:In the vestibular evaluation,we found that there was a statistically significant difference(P<0.05)between groups in the prevalence of bilateral vestibular weakness(group A:22.5%;group B:5%),and that the increase in age,above 60 years,is negatively correlated with the mean total caloric ***,we obtained a reasonable negative and statistically significant correlation(r=-0.320,P<0.05)between the mean bone conduction thresholds at high frequencies and total caloric responses in group ***:In patients with hearing loss,it is essential to perform a complete vestibular study to diagnose vestibular disorders and,consequently,prevent adverse outcomes that may result from these alterations.

Background There is increasing epidemiological evidence supporting the association between onchocerciasis and seizures, reinforcing the concept of onchocerciasis-associated epilepsy (OAE). The aim of this paper is to provide an update on the new knowledge about OAE and to propose recommendations to the World Health Organization how to address this public health *** text During the 2nd International Workshop on OAE held on 19-21 September, 2023, in Antwerp, Belgium, participants recognised OAE as a substantial yet neglected public health problem, particularly in areas of sub-Saharan Africa where onchocerciasis remains hyperendemic. Evidence from prospective population-based studies suggest that strengthening onchocerciasis elimination efforts leads to a significant reduction of OAE incidence. There is a need to validate an OAE case definition to estimate the burden of disease and identify onchocerciasis-endemic areas requiring intensification of onchocerciasis elimination programmes and integration of epilepsy care. It is expected that raising awareness about OAE will boost the population uptake of ivermectin. The implementation of a community-based epilepsy treatment programme offering free anti-seizure medications (ASMs) has shown high effectiveness in reducing the frequency of seizures and improving the overall quality of life of people with *** To reduce OAE burden, enhanced collaboration between onchocerciasis and mental health programmes at community, national, and international levels is required. Urgent efforts are needed to ensure the uninterrupted provision of free ASMs in onchocerciasis-endemic areas. Furthermore, OAE should be included in the quantification of the onchocerciasis disease burden.

Treatment of Bilateral Recurrent Dislocation of Hip Pros-thesis with Malpositioned Well-Fixed Shell: A Case Report

Dislocations of total hip prostheses cause pain and patient dissatisfaction. Recurrent dislocations are difficult to treat mainly when the acetabular metal shell is well-fixed. The purpose of this article was to describe the surgical technique used for the treatment of a bilateral recurrent posterior dislocation after a cementless total hip prosthesis, caused by excessive inclination of acetabular components, in a 72-year-old patient. On both sides, acetabular metal shell, porouscoated, was well-fixed. Revision of the entire acetabular component could be an appropriate therapeutic option because it was malpositioned. Nevertheless, a conservative operation was performed. The metal shell was left in situ and the preexisting polyethylene liner was removed and replaced by a new undersized cross-linked polyethylene liner, then, cemented into the shell and properly oriented. An acetabular cemented augmentation reinforced by 3 cortical screws was associated with the reconstruction. This report suggests that cementation of new liner into a malpositioned well- fixed metal shell associated with an acetabular cemented augmentation is a simple and safe technique for the management of recurrent hip dislocation, for elderly patients in which it is advisable to avoid a major revision hip surgery by medical comorbidities. Nonetheless, further studies with medium-and long-term follow-up are needed to validate this technique.

Risk Factors Associated with Unsuccessful Vaginal Birth after One Cesarean (VBAC-1) in Puerto Rico

Background: Cesarean section (CS) has increased steadily over the last decade, with an estimated one-third of women delivering by cesarean section worldwide. Objective: Our study aimed to investigate the demographic and associated factors influencing vaginal birth after one cesarean (VBAC-1) success focusing on variables like pre-pregnancy BMI, diabetes, hypertension, education, and smoking. Study Design and Methods: In this retrospective study, we analyzed 285 cases (81 unsuccessful VBAC-1, 204 successful VBAC-1) from San Juan City Hospital (Puerto Rico) between January 1, 2019, and December 31, 2020. We used odds ratios and model selection comparison to assess the impact of variables on successful VBAC-1, using a significance threshold of 95% CI. Model selection assessed binomial model combinations using a generalized linear approach to identify key risk factors. Results: Unsuccessful VBAC-1 (a repeat cesarean), was associated with diabetes (OR: 0.376, p = 0.086), hypertension (OR: 0.23, p = 0.006), and university-educated women (OR: 1.372, p = 0.711). High school-educated women had an OR of 3.966 (p = 0.105), while overweight women were 0.481 times more likely to have unsuccessful VBAC-1 (p = 0.041). Significant associations were not found with obesity (OR: 0.574, p = 0.122), underweight/normal (OR: 1.01, p = 0.810), or smoking (OR: 1.227, p = 0.990). Conclusion: Results revealed women with higher education levels, hypertension, or diabetes are less likely to have a successful VBAC-1. Understanding the complex interactions affecting these outcomes is aimed at establishing guidelines for healthcare professionals to conduct systematic risk/benefit assessments. This study lays a foundation for evidence-based practices and policies, offering initial insights into VBAC-1 success factors in Puerto Rico.

Structural Patterns of Immunoreactivity to Cytokeratin 18 in Normal Prostate and Benign Prostate Hyperplasia: Global and Local Differences

Background: Benign prostate hyperplasia (BPH) is the most common benign disease of the human prostate. The comparison between global versus local changes in spatial patterns of pathological lesions provoked a growing interest in some fields such as neuropathology. To date, there is little data on this subject in prostatic pathology. Given the interest of local parameters to distinguish between normal and pathological structures, the present study will apply first and second order stereological tools to find out if the cytokeratin18 (ck18) immunoexpression shows relevant local changes in BPH compared to normal prostate, independently if global estimates were similar in both groups. Methods: To verify if the global and local changes in immuno-expression of ck18 are relevant to ascertain differences between normal (CTR) and BPH cases, the following parameters will be applied: Volume fraction of epithelium immunostained for ck18 (VV ck18), both in global and local estimates;dispersion indices of VV ck18;estimates of local variance of VV ck18 (positional and of scale) using wavelet analysis;and lacunarity analysis to measure the tissue heterogeneity. Then, the set of values from the parameters studied that show significant differences between CTR and BPH will be employed to perform stepwise linear discriminant analyses to determine if locally estimated parameters were able to classify accurately the cases in CTR and BPH groups. Results and Conclusions: The findings of the present study indicate that changes in the expression of ck18 by the hyperplastic prostatic epithelium are not homogeneous. This limits the use of a single biopsy based markers to predict biological behavior in BPH. On the other hand, the local changes in the expression of ck18 are more evident in terms of VV ck18 and its local variability, whereas other parameters that are useful in other pathologies, such as lacunarity, are less relevant In prostatic hyperplasia.
