Background: The prevalence of both atrial fibrillation (FA) and diabetes mellitus (DM) is increasing and they often occur together and constitute a high risk of thrombosis. Rivaroxaban is a Factor Xa inhibitor with a rapid onset and disappearance of action after oral administration;it acts by inhibiting the active form of the coagulation factor. In order to reflect the effect of the action of Rivaroxaban, we used the prothrombin time (PT);however, it′s not the most accurate, but it is the one available in our community. Methods: This was a prospective, randomized, analyst-blinded, parallel group clinical study to verify the efficacy of Rivaroxaban Leti 20 mg (RL) (12 volunteers vs Rivaroxaban Bayer 20 mg (RB) (13 volunteers). The variables were determination of PT and Partial Thromboplastin Time (aPTT) at baseline and at 24, 48 and 72 hours after administering a daily dose of 20 mg for three days. The determination was carried out with the IDG method (Integrated Diagnostics Group Sanzay Corporation) with an International Sensitivity Index (ISI) of 1.17 PT and aPTT were taken before the first dose, and then, every day during the next 3 days, three hours after the ingestion of their daily dose at 7 am. Results: The 25 healthy volunteers were similar in age, BMI, and SBP/DBP level with a greater number of men in the Bayer group. The efficacy of rivaroxaban was similar in both groups with prolongation of PTT to the 2nd day of treatment with PT, and percentage changes from baseline (14.46 ± 0.97 for RB vs 14.17 ± 0.94 RL p: 0.45), PTT results and percentage changes from the base (RB: 34 ± 4.53 RL: 33.46 ± 2.82). The safety of rivaroxaban was good in both groups with no serious adverse events. The equivalence in the logarithmically transformed PT result (ln) on day two, Mean and CI (90%) 99.2 (94.4-104) and 100 (99.5-100.8);neither the means nor the 90% confidence intervals of the PT variable transformed logarithmically to ensure its normality, were far from the 80%-125% allowed for declaration of similarity. Recent advances in neuroscience have illuminated the central role of glutamate dysregulation in various neurological disorders.The glutamatergic system has emerged as a central player in the pathophysiology of various neurological disorders.The dysregulation of glutamate signaling,leading to excitotoxicity and neuronal cell death,has been a focal point in understanding the underlying mechanisms of these conditions.This has prompted a paradigm shift in neuroprotection research,with a growing emphasis on targeting ionotropic glutamate receptors(iGluRs)to restore glutamatergic homeostasis.This review provides a comprehensive overview of recent advancements in the field of iGluR-targeted neuroprotection.We further investigate the implications of glutamate dysregulation in the central nervous system(CNS)disorders,highlighting the complex interplay between excitotoxicity and neuroprotection.We elucidate the multifaceted factors that render neurons vulnerable to excitotoxic damage,emphasizing the need for innovative therapeutic approaches.This review provides an extensive survey of the burgeoning field of iGluR-targeted neuroprotection.It showcases the significant potential of a wide array of compounds,encompassing both natural and synthetic agents,to modulate iGluRs and ameliorate excitotoxicity and oxidative stress-induced neuronal damage.These compounds have demonstrated impressive neuroprotective effects in diverse experimental models,from glutamate-induced toxicity to traumatic brain injuries.We advocate for further research and clinical investigations to harness the full therapeutic potential of iGluR modulation,heralding a promising era in neuroprotection and CNs disorder management.

Despite organ transplantation being the most successful treatment for end-stage organ dysfunction,the number of annual solid organ transplantations is much lower than that required to satisfy the demand of patients on waiting lists.The explanation for this phenomenon is the relative scarcity of non-living organ donors due to several factors,such as:(1)Late arrival of patients with a neurocritical condition to an emergency service;(2)lack of detection of those patients as possible organ donors by health professionals dedicated to procurement or by clinicians at emergency and intensive care units,for instance;(3)late transfer of the patient to an intensive care unit to try to recover their health and to provide hemodynamic,ventilatory,and metabolic support;(4)lack of confirmation of the physiological status of the possible donor;(5)late or incorrect positive diagnosis of the subject’s death,either due to brain or cardiac death;(6)difficulty in obtaining legal authorization,either by direct relatives or by the authority,for the extraction of organs;and(7)deficient retrieval surgery of the organs actually donated.The recent reports of relatively successful xenotransplants from genetically modified pigs open the possibility to fix this mismatch between supply and demand,but some technical(organ rejection and opportunistic infections),and economic issues,still remain before accepting a progressive replacement of the organ sources for transplantation.An approximate economic cost analysis suggests that the hypothetical acquisition cost of any genetically modified pig derived organ is high and would not even satisfy the solid organ demand of the wealthiest countries.

Traditionally,food sustainability has been considered solely in the stage of agricultural production.However,globalization,the expansion of the food production industry,and the emergence of supermarket chains that control the retail food market require specific significant changes in supply chains in the food sector and,therefore,we need to address the economic,social,and environmental impacts of these events.On the other hand,social selling has increased rapidly in recent years,with a further boom,following current events related to the coronavirus disease(COVID-19).This explosion of social sales,where there are usually no control and regulation entities,can bring problems associated with mishandling items.In this paper,we expose how Blockchain technology supports the traceability of social sales by validating the data provided by the chain participants such as digital health passports,production and transport data in the sale process;the proposed solution generates recommendations on productmanagement considering the agreements previously made by the network actors.To evaluate the proposed smart contracts,we useHyperledger Caliper,obtaining an average throughput of 12.6 transactions per second and an average latency of 0.3 s for the asset update process.We also use a study case to evaluate the proposed project platform’s selling-transport stage using Internet of Things(IoT)sensors.

Complement-dependent neuroinflammation in spinal cord injury:from pathology to therapeutic implications

Spinal cord injury remains a major cause of disability in young adults,and beyond acute decompression and rehabilitation,there are no pharmacological treatments to limit the progression of injury and optimize recovery in this population.Following the thorough investigation of the complement system in triggering and propagating cerebral neuroinflammation,a similar role for complement in spinal neuroinflammation is a focus of ongoing research.In this work,we survey the current literature investigating the role of complement in spinal cord injury including the sources of complement proteins,triggers of complement activation,and role of effector functions in the pathology.We study relevant data demonstrating the different triggers of complement activation after spinal cord injury including direct binding to cellular debris,and or activation via antibody binding to damage-associated molecular patterns.Several effector functions of complement have been implicated in spinal cord injury,and we critically evaluate recent studies on the dual role of complement anaphylatoxins in spinal cord injury while emphasizing the lack of pathophysiological understanding of the role of opsonins in spinal cord injury.Following this pathophysiological review,we systematically review the different translational approaches used in preclinical models of spinal cord injury and discuss the challenges for future translation into human subjects.This review emphasizes the need for future studies to dissect the roles of different complement pathways in the pathology of spinal cord injury,to evaluate the phases of involvement of opsonins and anaphylatoxins,and to study the role of complement in white matter degeneration and regeneration using translational strategies to supplement genetic models.

Selenium(Se)has been defined as the“Janus element”,with one face showing antioxidant activity and the other pro-oxidant activity.The biological effect of Se depends on both dose and speciation.Se nanoparticles are attracting major interest,although their large-scale preparation for biomedical applications is not trivial.We hypothesize that acid anhydride-coated carbon dots(AACD)are an attractive platform for preparing nanoparticles containing chemically defined Se.The reaction of AA-CD with 3-selenocyanatopropan-1-amine yields carbon dots bearing selenocyanate and carboxylate groups(CD-SeCN)that allow for tuning the hydrosolubility.CD-SeCN has a Se content of 0.36μmol per mg of nanoparticles,they show the typical photoluminescence of carbon dots.The selenocyanate groups(SeCN)exhibited glutathione peroxidase-like activity and cytotoxicity.Data show that antioxidant behavior differs between normal and tumor cells,the evaluation on HEK293 and A549 cells reveals that the toxicity of CD-SeCN depends on dose,time,intracellular glutathione(GSH)content.The toxicity of CD-SeCN decreases with the time of incubation and the cell death mechanism switches from necrosis to apoptosis,indicating that CD-SeCN is neutralized.Additionally,high levels of intracellular GSH exert a protective effect.These results support a pharmacological potential in cancers with low levels of intracellular GSH.The use of AA-CD as nanoplatforms is a general strategy that paves the way for the engineering of advanced nanosystems.

Background: Myocardial ischemia in addition to other several cardiac syndromes represent a pathological proinflammatory state alongside a complex cellular microenvironment that can be modified by using cannabinoids. Cannabidiol (CBD), a non-psychoactive compound of cannabis has been recently proposed as an immudomodulatory and cardioprotective drug. Objectives: In this systematic review we sought to clarify and summarize the clinical and preclinical evidence of potential benefit of the use of CBD in coronary syndromes. Methods: We conducted a systematic search and review according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) and Review of Animal Data from Experimental Studies (CAMARADES) guidelines, in the electronic database from PubMed, Web of Science and Scopus up to April 2022 using predefined search terms. Pre-specified exclusion and inclusion criteria were considered, finally 11 articles were chosen to be included for this peer review. Results: Currently there are no good-quality clinical trials with the use of CBD in acute or chronic coronary syndromes. A total of 11 preclinical studies where prescreened and 5 demonstrated reproducible positive cardiovascular outcomes on in-vivo models treated with CBD. Mechanisms of CBD cardioprotection observed: 1) reduction in oxidative stress and inflammation, 2) activation of adenosine receptors and 3) increased expression of angiotensin type 2-receptor. Experimental models included ischemia/reperfusion injury, myocardial infarction, arrhythmias, and metabolic syndrome-like conditions. Conclusion: No clinical recommendation can be issued with the current evidence, on the use of CBD in acute and chronic coronary syndromes. Based on preclinical evidence, we considered there is enough evidence to propose the development of well-designed clinical trials that include CBD in the management of coronary syndromes.

Plumage brightness and uropygial gland secretions in barn swallows

The uropygial gland has been hypothesized to play a role in sexual signaling through a "make-up" function derived from the effects of secretions from the gland on the appearanee of the plumage and bare parts of the body. Here we show that plumage brightness of dorsal feathers of individual barn swallows Hirundo rustica was greater in mated than in unmated individuals. In addition, plumage brightness in creased with colony size. Furthermore, plumage bright ness was positively correlated with the amount of wax in the uropygial gland, negatively correlated with time of sampling of uropygial wax (perhaps because more wax is present early in the morning after an entire night of wax production without any preening), and negatively correlated with the number of chewing lice that degrade the plumage. Experimentally preventing barn swallows from access to the uropygial gland reduced plumage brightness, showing a causal link between secretions from the uropygial gland and plumage brightness. These findings provide evidence consistent with a role of uropygial secretions in signaling plumage brightness.

目的在重症监护病房(IntensiveCareUnit,ICU),护土难免会遇到需要限制治疗努力(limiting therapeutie efort,LTE)的患者,尽管多数护士还缺乏为这些临终患者及其家庭提供支持的准备。本综述旨在描述ICU护士在LTE中需要具备的能力。方法对2010年--2023年发表的文献进行综述。系统检索了5个数据库:Medline、Wiley Online Library、SciELO、ScienceDirect和Web of Science。使用关键评估项目工具西班牙语版(Critical Appraisal Skills Programme in Spanish,CASPe)作为评判性阅读工具。使用牛津循证医学中心(Center for Evidence-Based Medicine.,CEBM)评估工具确定证据级别和推荐强度。结果共纳入25篇文献。ICU护士LTE护理的能力包括对患者的直接护理、以家庭为中心的护理以及护士在团队中的作用。该综述确定了3个类别的能力:作为患者、家庭与跨学科团队之间维护者的能力,在LTE方面的决策能力,在患者生命末期治疗和护理干预的能力。结论ICU护士在LTE方面需要具备相应的能力,这对患者的生活质量死亡尊严以及患者家庭应对悲伤的全面支持至关重要。
