BACKGROUND Malnutrition in cirrhotic patients is correlated with mortality and a better response to liver ***,recovery of the nutritional status in these patients is a challenge due to the difficulty in establishing a reliable nutritional *** bioelectrical impedance vector analysis(BIVA)method appears as a feasible tool in clinical practice to define the physiological state of cirrhotic patients by assessing hydration and body *** To evaluate body composition in cirrhotic patients using *** This retrospective cross-sectional study was carried out by following cirrhotic outpatients at a hospital in porto Alegre,Brazil.A tetrapolar bioelectrical impedance analysis device was used to evaluate cellularity and hydration and to perform the *** BIVA graphic was elaborated by software and for statistical analysis a significance level of 5%(p≤0.05)was *** One hundred and ninety patients,61.1%males,with a mean age of 56.6±11.0 years,were *** these,56.3%had Child-Turcotte-pugh(CTp)A score,and the prevalent etiology was hepatitis C virus(47.4%).The patients were classified according to cellularity and hydration by the quadrants and ellipses of the BIVA method,quadrant 1(47.9%);quadrant 2(18.9%);quadrant 3(14.2%);and quadrant 4(18.9%).Those classified in quadrant 1 and 2 had a higher phase angle compared to those in quadrants 3 and 4(ppatients had a lower average age than the other *** association with CTp score showed that patients in quadrant 2 had a higher proportion of CTp A,and those in quadrant 4 had a higher proportion of CTp C(ppatients,and its association with clinical factors determines the disease severity,age and prognostic index.

Objective:We describe the first-year implementation experience of an Instrumented Sensory Integration Therapy program in Audiological&Balance Center ***:This is a retrospective descriptive *** included Seventy-three adults with diagnoses of acute,episodic,or chronic vestibular *** were classified into the following two groups:group 1 included 46 individuals treated with ISIT plus VRT,and group 2 included 27 individuals treated only with ***:The Sensory Organization Test(SOT)for both groups showed a statistical significance for all three sensory inputs;visual systems(G1:p=0.0003;G2:p=0.0337),vestibular system(G1:pp=0.0003),and balance as demonstrated by compound balance score(G1:pp=0.0035),and balance percentage deficit(G1:pp=0.0078).Conclusions:The severity and complexity of functional neurological disorders in the context of vestibular syndromes seem to require between 10 and 20 therapy sessions,and combined ISIT plus VRT appears to be more effective than ISIT as a monotherapy.

Gastric cancer remains one of the leading causes of cancer-related death worldwide,and most of the cases are associated with Helicobacter pylori *** bacterium promotes the production of reactive oxygen species(ROS),which cause DNA damage in gastric epithelial *** this study,we evaluated the expression of important genes involved in the recognition of DNA damage(ATM,ATR,and H2AX)and ROS-induced damage repair(ApE1)and the expression of some miRNAs(miR-15a,miR-21,miR-24,miR-421 and miR-605)that target genes involved in the DNA damage response(DDR)in 31 fresh tissues of gastric *** v3.1.1 was used to construct the postulated miRNA:mRNA interaction *** performed by real-time quantitative pCR exhibited significantly increased levels of the ApE1(RQ=2.55,pp=0.0002)genes beyond the miR-421 and miR-605 in the gastric cancer *** addition,significantly elevated levels of miR-21,miR-24 and miR-421 were observed in diffuse-type gastric *** analysis reinforced some of the gene:gene(ATM/ATR/H2AX)and miRNA:mRNA relationships obtained also with the interaction ***,our findings show that tumor cells from gastric cancer presents deregulation of genes and miRNAs that participate in the recognition and repair of DNA damage,which could confer an advantage to cell survival and proliferation in the tumor microenvironment.

Incidence of hepatocellular carcinoma in outpatients with cirrhosis in Brazil: A 10-year retrospective cohort study

AIM To determine the incidence of hepatocellular carcinoma(HCC) and the impact of HCC surveillance on early diagnosis and survival of cirrhotic outpatients. METHODS In this retrospective cohort study, cirrhotic outpatients undergoing HCC surveillance between March 2005 and March 2014 were analyzed. Exclusion criteria were HIV coinfection; previous organ transplantation; diagnosis of HCC at first consultation; missing data in the medical chart; and less than 1 year of follow-up. Surveillance was carried out every six months using ultrasound and serum alpha-fetoprotein determination. Ten-year cumulative incidence and survival were estimated through Kaplan-Meier analysis. RESULTS Four hundred and fifty-three patients were enrolled, of which 57.6% were male. Mean age was 55 years. Hepatitis C virus and heavy use of alcohol were the main etiologic agents of cirrhosis. HCC was diagnosed in 75 patients(16.6%), with an estimated cumulative incidence of 2.6% in the 1st year, 15.4% in the 5th year, and 28.8% in the 10 th year. Median survival was estimated at 17.6 mo in HCC patients compared to 234 mo in non-HCC patients(p patients who underwent surveillance every 6 mo or less(p = 0.05). However, survival was not different between patients with early stage vs non-early stage tumors [HR = 0.54(0.15-1.89), p = 0.33].CONCLUSION HCC is a frequent complication in patients with cirrhosis and adherence to surveillance programs favors early diagnosis.

Atypical Calcified Ganglioglioma: A Rare Case Report

Considered rare tumors, gangliogliomas account for 0.4% - 0.9% of intracranial neoplasms. The peak of its incidence occurs between 10 and 20 years of age. These tumors are composed of glial and ganglion cells and they are relatively low-grade neoplasms associated with good prognoses. We report a case of an atypical calcified ganglioglioma in an 18-year-old woman with history of four months of stabbing right-sided parietal headache, paroxysmal. On image studies were noted the presence of thick wall calcification in gangliogliomas. Although rare, this atypical ganglioglioma should be included in the differential diagnosis of lesions occurring in this area of the brain.

Study on Strategies for Methane Injection in a DISI （Direct Injection Spark Ignition） Engine

This paper deals with the application of two different injection strategies in a natural gas direct injection spark ignition single-cylinder engine model. The analysis includes the air flow characterization during compression stroke and also the charge stratification. Two different swirl ratios are applied and piston bowl effects are analyzed during the gas jet deflection towards the spark plug. Achieving wall-guided effect, consequently, the charge stratification in spark plug region is obtained. The goal is to study the mixture formation differences for a single and a bi pulsed CNG （compressed natural gas） injection profiles and check the evolution of the methane concentration at the spark plug at the start of combustion.

poly（lactic acid）（pLA）-containing nerve guidance conduits（NGCs） are currently being investigated for nerve repair as an alternative to autograft, which leads to permanent functional impairment in the territory innervated by the removed nerve. Combination of polymers modifies the physical properties of the conduits, altering their nerve-guidance properties. Conduits made from pLA-only or combined with other polymers have been used successfully for nerve repair, but their efficiency has not been compared. We compared the morphological and functional outcomes of peripheral nerve repair by using NGCs made of poly（lactic acid） and combined or not with polycaprolactone（pLA/pCL） or polyvinylpyrrolidone（pLA/pVp）. To assess the functional recovery, we employed a mechanical hyperalgesia analysis, sciatic functional index（SFI）, and electroneuromyography. The mechanical hyperalgesia analysis showed that the pLA group improved more rapidly than the pLA/pVp and pLA/pCL groups; similarly, in the electroneuromyography assay, the pLA group exhibited higher amplitude than the pLA/pCL and pLA/pVp groups. However, the SFI improvement rates did not differ among the groups. Morphologically, the pLA group showed more vascularization, while the nerve fiber regeneration did not differ among the groups. In conclusion, the pLA-only conduits were superior to the other NGCs tested for nerve repair.

Injuries to the spinal cord result in permanent disabilities that limit daily life *** main reasons for these poor outcomes are the limited regenerative capacity of central neurons and the inhibitory milieu that is established upon traumatic *** decades of research,there is still no efficient treatment for spinal cord *** strategies are tested in preclinical studies that focus on ameliorating the functional outcomes after spinal cord *** these,molecular compounds are currently being used for neurological recovery,with promising *** molecules target the axon collapsed growth cone,the inhibitory microenvironment,the survival of neurons and glial cells,and the re-establishment of lost *** this review we focused on molecules that are being used,either in preclinical or clinical studies,to treat spinal cord injuries,such as drugs,growth and neurotrophic factors,enzymes,and *** mechanisms of action of these molecules are discussed,considering traumatic spinal cord injury in rodents and humans.

Fungal endophytic species inhabiting the leaves of eucalypts are capable of utilising leaf sugars and can influence both plant growth and health. Endophytic fungal symbionts can use simple soluble sugars in leaves as their main carbon source. This study set out to determine the diversity and distribution of the endophytic filamentous fungal leaf community in the hybrid Eucalyptus urograndis due to its economic importance. The fungal leaf community was characterised using denaturing gradient electrophoresis (DGGE) and correlated with levels of leaf nutrients and sugars throughout plant development. Sequencing of DGGE bands revealed the presence of Basidiomycota and Ascomycota phyla. Fourteen species and three genera of filamentous fungi were identified, and the population structure was affected by the plant developmental stage. Levels of K, Cu, N and Mn influenced communities from the clonal garden, whereas leaves in the field had higher glucose, fructose and sucrose. Many fungi were found to be specific to a certain development stages: Diplomitoporus crustulinus, podosphaera tridactyla and Aspergillus restrictus to the clonal garden stage;Chaetomella acutiseta and Ascotricha chartarum to the shading stage;Erratomyces patelii and Saxomyces sp. to the shading output stage;Lepidostroma sp. and Saxomyces sp. to the dispatch stage;and Mycosphaerella populicola to the field stage. Teratosphaeria toledana and Teratosphaeria acidotherma were found at more than one developmental stage. Cladosporium sp. and Rhodosporidium fluviale colonized and persisted in plants at the dispatch and field stages. This is the first report of p. tridactyla, A. restrictus, E. patelii, Saxomyces and Lepidostroma sp. as endophytes in eucalipt.

AIM To determine the prevalence of epstein-barr virus(eb V)-associated gastric carcinomas in the North Region of portugal and to study its clinicopathological characteristics. METHODS We have performed a retrospective study including a total of 179 consecutive patients with gastric cancer(GC) submitted to gastrectomy during 2011 at the portuguese Oncology Institute of porto. Clinical and pathological data was collected from individual clinical records and inserted on a database with unique codification. Tumour tissues were collected from the institutional tumour bank. eb V was detected by in situ hybridization for the detection of eb V-encoded small RNAs(ebe Rs) and eb V latent proteins(LMp1 and LMp2 A) were detected by *** The analysis showed that eb V-associated gastric carcinomas(eb Va GC) represents 8.4%(15/179) of all GC cases, with a significant differential distribution among histological types(p poorly cohesive carcinomas. The analysis revealed a higher predominance of eb Va GC in the upper third and middle(cardia, fundus and body) of the stomach(p = 0.041), a significant lower number of regional lymph nodes invasion(p = 0.025) and a tendency for better prognosis(p = 0.222). eb V latent protein expression revealed that all eb Va GC cases were LMp1-negative, nevertheless 6 cases(40%) expressed LpM2 A, which reveals that these cases show a distinct eb V-Latency profile(latency II-like).CONCLUSION eb Va GC represents 8.4% of all GC in the North Region of portugal. The eb V-infected patients have specific clinic-pathological features that should be further explored to develop new strategies of management and treatment.
