Bladder calculi concomitant with benign prostatic enlargement:is prostate surgery mandatory in patients who have never received medical therapy?

The historical dogma that bladder calculi comprise the main indication for prostatic surgery has recently been questioned.In this study,we aimed to predict which patients should undergo simultaneous prostate and bladder calculi surgery or only bladder calculi removal by evaluating preoperative risk factors.One hundred and seventeen men with bladder stones and concomitant benign prostate enlargement(BPE)who had not received medical treatment before were included in the study.In the first step,only the bladder calculi of patients were removed and medical treatment was given for BPE.The patients who benefited from medical treatment during the follow-up were defined as Group 1 and the patients who required prostate surgery for any indication comprised Group 2.Risk factors for prostate surgery requirements were determined by comparing preoperative characteristics between the two groups with a cox regression model.In the follow-up of 117 patients with bladder stones removed and medical treatment initiated,49(41.9%)patients had prostate surgery indications.The indication for 33(67.3%)of 49 patients was medical treatment failure.The presence of intravesical prostatic protrusion(IPP;hazard ratio:2.071,95%confidence interval[Cl]:1.05-4.05,P=0.034),and high postvoiding residual urine volume(hazard ratio:1.013,95%Cl:1.007-1.019,P<0.001)were found to be preoperative risk factors for needing future prostate surgery.In patients who have not received medical treatment for BPE before,bladder calculi developing secondary to BPE do not always constitute an indication for prostate surgery.

The main objective of this study was to examine how different stand types influenced the tendency of visitors for varying recreational activities(i.e.,hiking,trekking,camping and hunting)from 1993 to 2018 in the surrounding areas of Uluyayla Natural Recreation Site in Bart1n,northern Turkey.A total of 627 visitors were selected on a voluntary basis,and questioned in the form of questionnaire.Most of the participants were in the age of 21-40 years old,and they were mostly(62.4%)male.Most of the participants(82.7%)lived in cities,while 62.8%of them were tourists when visiting the site.In determining the preferences of the visitors regarding the stand types for different recreational activities,several images of each stand were shared with them,and they were asked which stand type they would prefer for a given recreation activity(i.e.,hiking,tracking,camping and hunting).A range of options from 1 to 5 in Likert's scale was used in evaluating the given responses.The degree of positive and negative effects was examined via trend analysis.It was found that stand type had influence on the preferences of visitors regarding the recreational activities.In general,unmanaged and moderately treated stands were more preferred by visitors for the recreational purposes,compared to the previously managed stands.On the other hand,visitors'preference for the activities were different during two periods(i.e.,1993-2005 and 2006-2018).For each recreational activity,there was an increasing trend from 1993 to 2018.There had been a continuous increase for hunting since 1996,and for trekking and camping after 1999.The increases between 1993-1996 and 1996-1999 were likely due to the increasing awareness of the society as a result of the training and consciousness raising activities for nature conservation.The demand on the recreational activities continuously increased in unmanaged and moderately treated stands,while no significant changes were examined in the previously managed stands.The study points out that forest planning and manageme

AIM:To identify the frequency of bacterial growth,the most commonly grown bacteria and their antibiotic susceptibility,and risk factors for bacterial colonization in bile collected from patients with different biliary diseases.METHODS:This prospective study was conducted between April 2010 and August 2011.Patients with various biliary disorders were included.Bile was aspirated by placing a single-use,5F,standard sphincterotome catheter into the bile duct before the injection of contrast agent during endoscopic retrograde cholangiopancreaticography (ERCP).Bile specimens were transported to the microbiology laboratory in blood culture bottles within an anaerobic transport system.Bacteria were cultured and identified according to the standard protocol used in our clinical microbiology laboratory.The susceptibilities of the organisms recovered were identified using antimicrobial disks,chosen according to the initial gram stain of the positive cultures.RESULTS:Ninety-one patients (27% male,mean age 53.7 ± 17.5 years,range:17-86 years) were included in the study.The main indication for ERCP was benign biliary disease in 79 patients and malignant disease in 12 patients.The bile culture was positive for bacterial growth in 46 out of 91 (50.5%) patients.The most frequently encountered organisms were Gram-negative bacteria including Escherichia coli (28.2%),Pseudomonas (17.3%) and Stenotrophomonas maltophilia (15.2%).There were no significant differences between patients with malignant and benign disease (58% vs 49%,P = 0.474),patients with acute cholangitis and without acute cholangitis (52.9% vs 50%,P = 0.827),patients who were empirically administered antibiotics before intervention and not administered (51.4% vs 60.7%,P = 0.384),with regard to the bacteriobilia.We observed a large covering spectrum or low resistance to meropenem,amikacin and imipenem.CONCLUSION:We did not find a significant risk factor for bacteriobilia in patients with biliary obstruction.A bile sample for microbiological analysis may become a valuabl

A Price-elastic Approach for Optimal Scheduling of Small-scale Storage Devices in Smart Houses with Short-term and Long-term Constraints

Consecutive charging and discharging of storage devices(SDs)might deem beneficial from the perspective of short-term operation.However,it highly impacts the life span of the embedded battery and render restrictions on energy storage capacity.We investigate short-term and long-term constraints of SDs through a three-stage price-elastic approach to the optimal operation of small-scale SDs in smart houses.The first stage deals with data and scenario characterization where the data for determining short-term and long-term operation constraints of SD are acquired.Proper number of scenarios are generated to represent uncertain parameters such as long-term demand forecasting,daily load profile,electricity price,and photovoltaic(PV)generation.The second stage optimizes the long-term operation of SD using the envisioned scenarios subject to the long-term operation constraints and the installment costs of SDs.The outputs of this stage are two indicators referred to as price elasticity and price offset coefficients,which are used as the inputs for the third stage.The third stage is responsible for decision-making on short-term operation of SDs.The outputs of the second stage along with short-term forecasting for daily electricity price,daily load and daily PV generation are acquired.Based on the acquired data,proper price elasticity and price offset are determined for optimal operation.Comprehensive simulations are performed for different demand forecasting and electricity prices.Simulation results confirm the effectiveness of the proposed approach.

Adhesive Single Lap Joints have been subjected to tensile and bending investigations by many researchers. However, the joint is also likely to experience buckling loading in some aerospace applications. The aim of this work is to investigate the joint behaviour under quasi-static buckling conditions. For this purpose, the joints with three different adherend thicknesses and 25 mm overlap length were tested using two different types of adherends and an adhesive film. They were modelled using a non-linear Finite Element Method via the ABAQUS Explicit package programme.Load to failure and stress distributions in the joints were predicted and compared with the experimental results, which were found in a good agreement. The adhesive layer in the joint was assumed to experience shear stresses under the buckling mode, similar to that in tensile loading, yet, the stress concentrations at the ends of the overlap, the main cause of the failure, resulted in different effects on the joint performance;for the buckling mode the critical stresses were in compression but for the tensile case in peeling. Unlike the latter, the former was found to prevent failure of the layer depending on the adherend thickness, causing different failure mechanisms. There were two different failure modes of the joints;a complete failure in the adhesive layer and large plastic deformation of adherends which could be a good source for crashworthiness situations. Mechanical properties of the adherends were found to play important roles on the joint performance.

Nonlinear oscillations in micro-and nanoelectromechanical systems have emerged as an exciting research area in recent years due to their promise in realizing low-power,scalable,and reconfigurable mechanical memory and logic devices.Here,we report ultralow-power mechanical memory operations utilizing the nonlinear oscillation regime of GaN microcantilevers with embedded piezotransistive AlGaN/GaN heterostructure field effect transistors as highly sensitive deflection transducers.Switching between the high and low oscillatory states of the nonlinear oscillation regime was demonstrated using a novel phase-controlled opto-mechanical excitation setup,utilizing a piezo actuator and a pulsed laser as the primary and secondary excitation sources,respectively.Laser-based photoacoustic excitation was amplified through plasmonic absorption in Au nanoparticles deposited on a transistor.Thus,the minimum switching energy required for reliable memory operations was reduced to less than a picojoule(pJ),which translates to one of the lowest ever reported,when normalized for mass.

The transport of externally overdriven particles confined in entropic barriers is investigated under various types of oscillating and temporal forces.Temperature,load,and amplitude dependence of the particle current and energy conversion efficiency are investigated in three dimensions.For oscillating forces,the optimized temperature–load,amplitude–temperature,and amplitude–load intervals are determined when fixing the amplitude,load,and temperature,respectively.By using three-dimensional plots rather than two-dimensional ones,it is clearly shown that oscillating forces provide more efficiency compared with a temporal one in specified optimized parameter regions.Furthermore,the dependency of efficiency to the angle between the unbiased driving force and a constant force is investigated and an asymmetric angular dependence is found for all types of forces.Finally,it is shown that oscillating forces with a high amplitude and under a moderate load lead to higher efficiencies than a temporal force at both low and high temperatures for the entire range of contact angle.

In this study, considering the temporarily unbiased force and different forms of oscillating forces, we investigate the current and efficiency of Brownian particles in an entropic tube structure and present the numerically obtained results. We show that different force forms give rise to different current and efficiency profiles in different optimized parameter intervals. We find that an unbiased oscillating force and an unbiased temporal force lead to the current and efficiency, which are dependent on these parameters. We also observe that the current and efficiency caused by temporal and different oscillating forces have maximum and minimum values in different parameter intervals. We conclude that the current or efficiency can be controlled dynamically by adjusting the parameters of entropic barriers and applied force.

The Gross-Pitaevskii equation, which is the governor equation of Bose-Einstein condensates, is solved by first order perturbation expansion under various q-deformed potentials. Stationary probability distributions reveal one and two soliton behavior depending on the type of the q-deformed potential. Additionally a spatial shift of the probability distribution is found for the dark soliton solution, when the q parameter is changed.
