COVID-19 in children:epidemic issues and candidate vaccines

A large-scale vaccination of coronavirus disease-19(COVID-19)in adults has been conducted for nearly a year,and there is a growing recognition that immunization for children is also essential.It has been months since emergency use of pediatric COVID-19 vaccine was approved,we reviewed the prevalence and transmission of COVID-19 in children.The prevalence of COVID-19 in children is reduced due to vaccination even in a Delta prevalent period,so an increase in the vaccination rate is needed in children.Although the precise role of children in the transmission requires more research to uncover,they likely played a significant role,according to the available literature.We also described four candidate COVID-19 vaccines for children on their safety and immunogenicity and the impact of severe acute respiratory syndrome coronavirus 2 variants on childhood vaccination.Safety issues on pediatric vaccines post-approval,like adverse events following immunization and adverse events of special interest require studies on long-term and effective regulatory mechanisms.

Summary What is already known about this topic?Healthcare workers(HCWs)and previously infected patients(PIPs)may experience a wave of epidemic following the modification of the country’s coronavirus disease(COVID)-zero policy in China.What is added by this report?As of early January 2023,the initial wave of the COVID-19 pandemic among HCWs had effectively subsided,with no statistically significant differences observed in infection rates compared to those of their co-occupants.The proportion of reinfections among PIPs was relatively low,particularly in those with recent infections.What are the implications for public health practice?Medical and health services have resumed normal operations.For patients who have recently experienced severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)infections,appropriate relaxation of policies may be considered.

The possible dual epidemic of coronavirus disease 2019(COVID-19)and influenza in the coming winter raises serious concerns for the northern hemisphere as it will likely result in an expanded disease burden and overloading of public health institutions and clinical capacities as a result of continued increasing mental stress for the public and depletion of public resources.Several questions remain regarding how the flu season might affect the COVID-19 pandemic and vice versa.

Background:The new emerging avian influenza A H7N9 virus,causing severe human infection with a mortality rate of around 41%.This study aims to provide a novel treatment option for the prevention and control of H7N9.Methods:H7 hemagglutinin(HA)-specific B cells were isolated from peripheral blood plasma cells of the patients previously infected by H7N9 in Jiangsu Province,China.The human monoclonal antibodies(mAbs)were generated by amplification and cloning of these HA-specific B cells.First,all human mAbs were screened for binding activity by enzyme-linked immunosorbent assay.Then,those mAbs,exhibiting potent affinity to recognize H7 HAs were further evaluated by hemagglutination-inhibiting(HAI)and microneutralizationin vitro assays.Finally,the lead mAb candidate was selected and tested against the lethal challenge of the H7N9 virus using murine models.Results:The mAb 6-137 was able to recognize a panel of H7 HAs with high affinity but not HA of other subtypes,including H1N1 and H3N2.The mAb 6-137 can efficiently inhibit the HA activity in the inactivated H7N9 virus and neutralize 100 tissue culture infectious dose 50(TCID_(50))of H7N9 virus(influenza A/Nanjing/1/2013)invitro,with neutralizing activity as low as 78 ng/mL.In addition,the mAb 6-137 protected the mice against the lethal challenge of H7N9 prophylactically and therapeutically.Conclusion:The mAb 6-137 could be an effective antibody as a prophylactic or therapeutic biological treatment for the H7N9 exposure or infection.

lIn response to the severe acute respiratory syndrome coronavirus-2(SARS-CoV-2)pandemic,over 200 vaccine candidates againstcoronavirus disease 2019(COVID-2019)are under development and currently moving forward at an unparalleled speed.Theavailability of surrogate endpoints would help to avoid large-scale filed efficacy trials and facilitate the approval of vaccine candidates,which is crucial to control COVID-19 pandemic.Several phase 3 efficacy trials of COVID-19 vaccine candidates are underway,which provide opportunities for the determination of COVID-19 correlates of protection.In this paper,we review currentknowledge for existence of COVD-19 correlates of protection,methods for assessment of immune correlates of protection andissues related to COVID-19 correlates of protection.

Patients with cancer are at increased risk of SARA-CoV-2 infection or developing severe COVID-19 cases due to malignancy or immunosuppressive therapy,but they are generally excluded from the target population for COVID-19 vaccination.In general,inactivated vaccines are safe and immunogenic for patients with cancer,but live attenuated vaccines are not recommended.The study suggested that the safety of the mRNA COVID-19 vaccine in patients with cancer is similar to that in healthy people,but immunogenicity is slightly weaker,and a booster dose may be needed.This paper aims to summarize the results of COVID-19 vaccine clinical studies conducted in patients with cancer worldwide and the relevant guidelines released by authorities,so as to provide evidence for promoting COVID-19 vaccination for patients with cancer.

Ⅱ/Ⅲ期无缝设计是将独立的Ⅱ期和Ⅲ期临床试验整合为一个连续的、分阶段的适应性临床试验设计。相较于传统的独立Ⅱ期和Ⅲ期临床试验,无缝设计在加速药物或疫苗上市、提高临床试验效率方面具有显著的优势。目前该设计在抗肿瘤药物研究领域的应用上日益成熟,也逐渐扩展到疫苗的临床试验中,包括九价人乳头瘤病毒疫苗、Sabin株脊髓灰质炎灭活疫苗等。本文旨在阐述Ⅱ/Ⅲ期无缝设计的概念,并以九价人乳头瘤病毒疫苗的Ⅱ/Ⅲ期无缝设计的临床试验为例,阐述实施Ⅱ/Ⅲ期无缝设计时需注意的细节和潜在的挑战,以期为我国在优化疫苗临床试验方面提供参考。

Analysis on the Epidemiological Characteristics of Escherichia coli O157:H7 Infection in Xuzhou,Jiangsu,China,1999

Objective： To determine epidemiologic features of an Escherichia coli O157：H7 outbreak occurred in Xuzhou, Jiangsu Province, China in 1999, and assess the incidence of E. coli O157：H7 in diarrhea patients and host animals and its relationship with disease onset, and provide a scientific basis for establishing prevention and control strategies. Methods： Epidemiological, microbiological, and molecular methods were performed to identify risk factors and describe the ecology of E. coli O157：H7 in the enviromnent. Results： From May to September, in 1999, 99 cases of E. coli O157：H7 infection were confirmed. Fifty-six patients were enrolled in the case-control study. Bad personal health habits and poor sanitary conditions in the kitchen were associated with increased risks of infection, whereas hand washing was protective. The household survey indicated that residents in the epidemic region during the outbreak had higher than expected rates of diarrhea. The total E. coli O157：H7 carrier rate in the livestock was 12.36%（22/178）, specifically 19.15% in cattle, 12.50% in goat, and 11.11% in swine. Numerical analysis of pulsed-field gel electrophoresis（PFGE） profiles divided strains into two clusters with 77.5% homology. One cluster contained 11 strains isolated from diarrheal patients, foods, and animals. The other cluster comprised 10 strains from patients and environment. Conclusion： In a large outbreak of E. coli O157：H7 infection among predominantly elderly residents in Xuzhou, high rates of carriage of E. coli O157：H7 among host animals most likely resulted in contamination of the environment, thereby leading to the outbreak. Effective and preventive control measures should be taken to avoid contamination, including environmental and family health improvement, good personal hygiene, and safe food handling practices.
