Heart failure (HF) is a major public health problem worldwide entailing high morbidity and mortality as well as high costs. This chronic syndrome associates with a low functional status and quality of life. Most patients with HF are elderly, constituting up to 80% of patients suffering from this disease with both incidence and prevalence of the condition increasing with age. This is due to the progressive aging of the population as well as improved and better survival after cardiac insults, such as myocardial infarction, especially in developed countries. Notably, acute HF is the leading cause of hospitalization in patients over 65 years. Accordingly, early diagnosis and proper treatment are critical as they both influence prognosis in these patients.[3] Major therapeutic advancements, including drug development and some technological improvements related to HF therapies, have occurred in the last decade.[4] However, there is concern about whether patients treated every day in our clinical practice are similar to those included in clinical trials where these therapeutic strategies clearly demonstrated clinical efficacy. This is especially so for elderly patients, often under-represented or excluded in such large clinical trials. Of note, the term “elder” has been applied until recently to patients with more than 65 years of age. Nevertheless, given the aging population, the age group that includes “elderly patients” has shifted to over 70–80 years, with all the implications that this change implies. These patients are even more underrepresented in large controlled clinical trials. Therefore most experts consider further evidence is required, especially regarding issues related to specific characteristics of the elderly population. In addition, the importance of proper diagnosis and adequate and optimized therapy, which also refers to treatment of comorbidities, should be highlighted. The progressive aging of the population has risen awareness and concern about the impact of geriatric syndromes on both clinical outcomes and health-related quality of life in elderly patients, as well as issues regarding the rising economic burden associated with healthcare. Recent studies have demonstrated frailty and other geriatric syndromes such as functionality, dependency and comorbidity can identify the most vulnerable patients, thus allowing to better estimate the individual prognosis in terms of disability, readmission and mortality. Such information should also be considered in the decision making process, ensuring the benefit derived from each treatment in each patient, optimizing the resources and avoiding futility.

Owing to the intrinsic nonlinearities of the system,a contracting mechanism,such as myogenic response,may induce different oscillatory patterns.Many specialists discussed the relations of oscillatory patterns with intrinsic control system or some pathological condition,but there is no single,well-defined criterion to achieve the identification of regular,stochastic,and chaotic activities.In this paper,we focus on the Mallat algorithm of wavelet packet and use it in the identification of the regular periodic,stochastic,and chaotic fluctuations.According to the specific frequency configuration of the chaos activity,we select proper layers of decomposition of wavelet packet and did fine segments to the frequency of signals.The frequency band of energy convergence could be recognized.The signal of periodic,stochastic,and chaotic could be distinguished depending on it.Numerical experiment is given to show its efficiency.Experiments on 12 babies' lung data have been done.This identification by means of wavelet packet could support the cardiologist or cerebral specialist to do more observation and deeper analysis to physic signals.

The relation between narratives and agency can be sometimes considered as mutually constitutive. There are cases in which telling a story expresses higher degrees of agency, and respectively, agency is shaped as a narrative that expresses the agent"s reasons. From henceforth, I will contend that a narrative theory, beyond the personal identity problem, can also enlighten how the agent attains giving reasons for the action by making sense of sequences of events. In order to explain how agency is constituted from understanding and control, we must assume a primitive competence for narratives. Agency supposes an ordering that cannot be reduced to temporality, but expresses a certain sense of accomplishment that can be narratively constituted. In this context, I will examine Welleman"s theory of emotions as an ordering framework of the events in structures of means-ends. A possible objection to his explanation of the agential narratives is that emotions themselves need to be understood as narrative processes. I suggest then that an enactivist approach would be a way of explaining the narrative constitution of agency. One virtue of this approach is that it harmonizes the biological and cultural components of the agency from the most basic layers to the ordinary folk-psychological narratives.

Cardiovascular disease (CVD) is the main cause of death in older adults. There is strong evidence that regular physical activity (PA) reduces the risk of CVD mortality in this population group. However, these studies used baseline data and do not account for potential changes in PA. Previous research on the mortality effect of changes in PA focused on all-cause deaths; hence, this is the first study aimed to examine the association of changes in PA with subsequent CVD mortality in older adults.

The study analysed the behaviour of titanium dioxide (TiO2) in polyethylene teraphthalate (PET) polymer coatings on ECCS steel plates. This work determined the thermal source affecting rutile distribution in the PET coating: it originated in the manufacturing processes and thermomechanical treatments of the composite, and affected PET adherence on the steel. These results were contrasted with the behaviour of a non-heated coating to determine the conditions under which the rutile exhibits changes on the PET coating. Measurements at cross-section and parallel intervals from the surface to the metal interface were made by FTIR-ATR and Raman vibrational spectroscopy techniques to characterise the rutile distribution in the PET thickness.
