Faecal calprotectin and magnetic resonance imaging in detecting Crohn's disease endoscopic postoperative recurrence

AIM To assess magnetic resonance imaging(MRI) and faecal calprotectin to detect endoscopic postoperative recurrence in patients with Crohn's disease(CD).METHODS From two tertiary centers, all patients with CD who underwent ileocolonic resection were consecutively and prospectively included. All the patients underwent MRI and endoscopy within the first year after surgery or after the restoration of intestinal continuity [median = 6 mo(5.0-9.3)]. The stools were collected the day before the colonoscopy to evaluate faecal calprotectin level. Endoscopic postoperative recurrence(POR) was defined as Rutgeerts' index ≥ i2b. The MRI was analyzed independently by two radiologists blinded from clinical *** Apparent diffusion coefficient(ADC) was lower in patients with endoscopic POR compared to those with no recurrence(2.03 ± 0.32 vs 2.27 ± 0.38 × 10^(-3) mm^2/s, P = 0.032). Clermont score(10.4 ± 5.8 vs 7.4 ± 4.5, P = 0.038) and relative contrast enhancement(RCE)(129.4% ± 62.8% vs 76.4% ± 32.6%, P = 0.007) were significantly associated with endoscopic POR contrary to the magnetic resonance index of activity(Ma RIA)(7.3 ± 4.5 vs 4.8 ± 3.7; P = 0.15) and MR scoring system(P = 0.056). ADC 100%(sensitivity = 0.75, specificity = 0.81, PPV = 0.75, NPV = 0.81) were the best cutoff values to identify endoscopic POR. Clermont score > 6.4(sensitivity = 0.61, specificity = 0.82, PPV = 0.73, NPV = 0.74), Ma RIA > 3.76(sensitivity = 0.61, specificity = 0.82, PPV = 0.73, NPV = 0.74) and a MR scoring system ≥ MR1(sensitivity = 0.54, specificity = 0.82, PPV = 0.70, and NPV = 0.70) demonstrated interesting performances to detect endoscopic POR. AIM: To investigate whether an endoscopy-based management could prevent the long-term risk of postoperative ***: From the pathology department database, we retrospectively retrieved the data of all the patients operated on for Crohn’s disease (CD) in our center (1986-2015). Endoscopy-based management was defined as systematic postoperative colonoscopy (median time after surgery = 9.5 mo) in patients with no clinical postoperative recurrence at the time of ***: From 205 patients who underwent surgery, 161 patients (follow-up > 6 mo) were included. Endoscopic postoperative recurrence occurred in 67.6%, 79.7%, and 95.5% of the patients, respectively 5, 10 and 20 years after surgery. The rate of clinical postoperative recurrence was 61.4%, 75.9%, and 92.5% at 5, 10 and 20 years, respectively. The rate of surgical postoperative recurrence was 19.0%, 38.9% and 64.7%, respectively, 5, 10 and 20 years after surgery. In multivariate analysis, previous intestinal resection, prior exposure to anti-TNF therapy before surgery, and fistulizing phenotype (B3) were postoperative risk factors. Previous perianal abscess/fistula (other perianal lesions excluded), were predictive of only symptomatic recurrence. In multivariate analysis, an endoscopy-based management (n = 49/161) prevented clinical (HR = 0.4, 95%CI: 0.25-0.66, P < 0.001) and surgical postoperative recurrence (HR = 0.30, 95%CI: 0.13-0.70, P = 0.006).CONCLUSION: Endoscopy-based management should be recommended in all CD patients within the first year after surgery as it highly decreases the long-term risk of clinical recurrence and reoperation.

Biotransformation of 6:2 fluorotelomer sulfonate(6:2 FTS)in sulfur-rich media by Trametopsis cervina

This pilot study on humor in cultural trauma uses innovative digital methodologies in speech recognition and social media analysis via Postman and Python in the Twitter API v2 to propose different forms to gather and analyze empirical data on semiotics and humor to be used in a broader *** given example of humorous instances on the cultural trauma of 9/11 in the fictional sitcom Rick and Morty is limited but corroborated by a second example that is structurally different from the first:humor about 9/11 in stand-up comedy by Louis *** on the data,humor in trauma seems to undergo a 5-phase-typology related to the Kübler-Ross model of grief,which is mended by the(digital)participation of ***,we argue,both shapes the discourse on humor in trauma and overcomes the traumatic effects through humorous discourse.

Expedition to the blockchain application potential for higher education institutions

In the education sector,blockchain is currently at the end of the peak of inflated expectations in Gartner’s Hype ***,it is crucial to understand whether this technology meets the expectations of Higher Education Institutions(HEIs).We go on an expedition to identify blockchain application scenarios and its potential for HEI administration-the universities are digitalized to just 23.3%.Current information systems research addresses classifications of blockchain-based projects(application level)rather than their technical realization(protocol level).Thus,when evaluating blockchain application scenarios in HEI administration,we intensively consider the technical side of blockchain-based *** perform a threestep approach:(1)systematic literature review,(2)qualitative exploratory semi-structured interviews to sup-plement information on market-ready solutions,and(3)an evaluation of the potential of the blockchain-based projects identified,based on HEI administration *** find that the leading blockchain application scenarios are credential verification and *** the protocol level,we obtain equivocal results regarding the technical realization of projects,e.g.,their underlying blockchain types and storage *** the application level,when discussing the potential of different projects,we find that most of them address adaptability,complexity decomposition,and cost reduction requirements between HEIs;interest diversity and stakeholder collaboration between HEIs and business actors;privacy and trust between HEIs and students.

Patterns of variation in the size of vulture foraging groups,and their ecological causes and consequences,remain litle explored despite strong links with the carrion recycling service that this key functional group *** documented the group size-frequency pattern of Andean con-dors Vultur gryphus gathered to feed on 42 equine carcasses experimentally placed in Bolivia,between 2012 and 2019,along an elevation range of 1,300-4,500 m *** on the location(altitude and latitude)of the foraging groups,we examined the relationship between their size and species'population parameters(size and trend),habitat conditions,and livestock carcass availability and *** utilized a high frequency(93%)of carcasses forming groups that ranged from 1 to 80 individuals(mean=25,median=18)and shaped a"lazy-J curve"typical pattern of size-frequency distribution whereby few groups(5,12%)were large(>55 individuals)and most(21,50%)were relatively small(<19 individuals).Group size related to altitude in that most larger groups formed at lower sites(below c.3,000 m asl),likely following an altitudinal gradient whereby larger groups are more likely to form around larger carcasses(i.e.,cattle),which are more likely to occur at lower *** of population size,group size could be an adaptive response of condors via local enhancement for improving individual scavenging *** information gaps on this topic still exist,thus we provide a set of questions to address them,especially amidst the unrestricted impacts of human activities that condition vulture survival globally.

A CFD Model to Evaluate Near-Surface Oil Spill from a Broken Loading Pipe in Shallow Coastal Waters

Oil spills continue to generate various issues and concerns regarding their effect and behavior in the marine environment,owing to the related potential for detrimental environmental,economic and social *** is essential to have a solid understanding of the ways in which oil interacts with the water and the coastal ecosystems that are located *** study proposes a simplified model for predicting the plume-like transport behavior of heavy Bunker C fuel oil discharging downward from an acutely-angled broken pipeline located on the water *** results show that the spill overall profile is articulated in three major flow *** first,is the source field,i.e.,a region near the origin of the initial jet,followed by the intermediate or transport field,namely,the region where the jet oil flow transitions into an underwater oil plume flow and starts to move horizontally,and finally,the far-field,where the oil re-surface and spreads onto the shore at a significant distance from the spill *** behavior of the oil in the intermediate field is investigated using a simplified injection-type oil spill model capable of mimicking the undersea trapping and lateral migration of an oil plume originating from a negatively buoyant jet spill.A rectangular domain with proper boundary conditions is used to implement the *** Projection approach is used to discretize a modified version of the Navier-Stokes equations in two dimensions.A benchmark fluid flow issue is used to verify the model and the results indicate a reasonable relationship between specific gravity and depth as well as agreement with the aerial data and a vertical temperature profile plot.

Seasonal influence of freshwater discharge on spatio-temporal variations in primary productivity, sea surface temperature, and euphotic zone depth in the northern Bay of Bengal

Ocean productivity is the foundation of marine food web,which continuously removes atmospheric carbon dioxide and supports life at sea and on ***-temporal variability of net primary productivity(NPP),sea surface temperature(SST),sea surface salinity(SSS),mixed layer depth(MLD),and euphotic zone depth(EZD) in the northern B ay of Bengal(BoB) during three monsoon seasons were examined in this study based on remote sensing data for the period 2005 to *** compare the NPP distribution between the coastal zones and open BoB,the study area was divided into five zones(Z1-Z5).Results suggest that most productive zones Z2 and Zl are located at the head bay area and are directly influenced by freshwater discharge together with riverine sediment and nutrient *** Z1-Z5,the NPP ranges from 5 315.38 mg/(m^(2)·d) to 346.7 mg/(m^(2)·d)(carbon,since then the same).The highest monthly average NPP of 5 315.38 mg/(m^(2)·d) in February and 5 039.36 mg/(m^(2)·d) in June were observed from Z2,while the lowest monthly average of 346.72 mg/(m^(2)·d) was observed in March from Z4,which is an oceanic *** values vary from 6-154 m for the study area,and it has an inverse correlation with NPP *** is deeper during the summer season and shallower during the wintertime,with a corresponding increase in *** the year,monthly SST shows slight fluctuation for the entire study area,and statistical analysis shows a significant correlation among NPP,and EZD,overall positive between NPP and MLD,whereas no significant correlation among SSS,and SST for the northern ***-term trends in SST and productivity were significantly po sitive in head bay zones but negatively productive in the open *** findings in this study on the distribution of NPP,SST,SSS,MLD,and EZD and their seasonal variability in five different zones of BoB can be used to further improve the management of marine resources and overall environmental condition in response to clim
