Laparoscopic approach in gastrointestinal emergencies

This review focuses on the laparoscopic approach to gastrointestinal emergencies and its more recent indications. Laparoscopic surgery has a specific place in elective procedures, but that does not apply in emergency situations. In specific emergencies, there is a huge range of indications and different techniques to apply, and not all of them are equally settle. We consider that the most controversial points in minimally invasive procedures are indications in emergency situations due to technical difficulties. Some pathologies, such as oesophageal emergencies, obstruction due to colon cancer, abdominal hernias or incarcerated postsurgical hernias, are nearly always resolved by conventional surgery, that is, an open approach due to limited intraabdominal cavity space or due to the vulnerability of the bowel. These technical problems have been solved in many diseases, such as for perforated peptic ulcer or acute appendectomy for which a laparoscopic approach has become a wellknown and globally supported procedure. On the other hand, endoscopic procedures have acquired further indications, relegating surgical solutions to a second place; this happens in cholangitis or pancreatic abscess drainage. This endoluminal approach avoids the need for laparoscopic development in these diseases. Nevertheless, new instruments and new technologies could extend the laparoscopic approach to a broader array of potentials procedures. There remains, however, a long way to go.

Traumatic brain injury, chronic traumatic encephalopathy, and Alzheimer's disease are three distinct neurological disorders that share common pathophysiological mechanisms involving neuroinflammation. One sequela of neuroinflammation includes the pathologic hyperphosphorylation of tau protein, an endogenous microtubule-associated protein that protects the integrity of neuronal cytoskeletons. Tau hyperphosphorylation results in protein misfolding and subsequent accumulation of tau tangles forming neurotoxic aggregates. These misfolded proteins are characteristic of traumatic brain injury, chronic traumatic encephalopathy, and Alzheimer's disease and can lead to downstream neuroinflammatory processes, including assembly and activation of the inflammasome complex. Inflammasomes refer to a family of multimeric protein units that, upon activation, release a cascade of signaling molecules resulting in caspase-induced cell death and inflammation mediated by the release of interleukin-1β cytokine. One specific inflammasome, the NOD-like receptor protein 3, has been proposed to be a key regulator of tau phosphorylation where it has been shown that prolonged NOD-like receptor protein 3 activation acts as a causal factor in pathological tau accumulation and spreading. This review begins by describing the epidemiology and pathophysiology of traumatic brain injury, chronic traumatic encephalopathy, and Alzheimer's disease. Next, we highlight neuroinflammation as an overriding theme and discuss the role of the NOD-like receptor protein 3 inflammasome in the formation of tau deposits and how such tauopathic entities spread throughout the brain. We then propose a novel framework linking traumatic brain injury, chronic traumatic encephalopathy, and Alzheimer's disease as inflammasomedependent pathologies that exist along a temporal continuum. Finally, we discuss potential therapeutic targets that may intercept this pathway and ultimately minimize long-term neurological decline.

Prognostic impact of inflammatory and nutritional biomarkers in pancreatic cancer

Pancreatic cancer is usually associated with a poor *** is the main curative treatment but pancreatic operations are aggressive and new tools that help clinicians to predict surgical and prognostic outcomes are *** et al recently published a retrospective,single centre cohort study evaluating the impact of seven nutritional and inflammatory markers in pancreatic cancer surgical patients:The albumin-to-globulin ratio,prognostic nutritional index(PNI),systemic immune-inflammation index(SII),neutrophil-to-lymphocyte ratio(NLR),platelet-to-lymphocyte ratio(PLR),nutritional risk index,and the geriatric nutritional risk index.A significant correlation was found between the PNI,SII,NLR,and PLR and a hospital discharge of less than 15 *** a univariable analysis,PNI,SII,NLR and PLR were significantly related to recurrence-free survival and,in a multivariable analysis PNI was associated with overall *** meta-analyses corroborate the results in terms of prognosis but individual studies are discordant on their ***,the cut-off values for these markers vary significantly between studies and there are no clinical trials comparing them to identify the most relevant *** are limitations when implementing nutritional and inflammatory biomarkers into clinical practice and further studies are needed in order to answer these questions.

In recent years, a family of numerical algorithms to solve problems in real algebraic and semialgebraic geometry has been slowly growing. Unlike their counterparts in symbolic computation they are numerically stable. But their complexity analysis, based on the condition of the data, is radically different from the usual complexity analysis in symbolic computation as these numerical algorithms may run forever on a thin set of ill-posed inputs.

On the Tractability of Shortest Path Problems in Weighted Edge-Coloured Graphs

A weighted edge-coloured graph is a graph for which each edge is assigned both a positive weight and a discrete colour, and can be used to model transportation and computer networks in which there are multiple transportation modes. In such a graph paths are compared by their total weight in each colour, resulting in a Pareto set of minimal paths from one vertex to another. This paper will give a tight upper bound on the cardinality of a minimal set of paths for any weighted edge-coloured graph. Additionally, a bound is presented on the expected number of minimal paths in weighted edge-bicoloured graphs. These bounds indicate that despite weighted edge-coloured graphs are theoretically intractable, amenability to computation is typically found in practice.

Automation in detection of recirculation in a booster fan ventilation network

Recirculation is prohibited in many coal mining countries because of the fear that the re-use of return air would allow the build-up of air contaminants at the workings. The incorrect design and location of a booster fan in any ventilation network can create unsafe condition due to recirculation. The current approach to investigating recirculation using simulation software requires manual effort which becomes tedious in a complex and a large network. An algorithm-based C++ program was designed to detect the recirculation in a booster fan ventilation networks. This program needed an input file prepared from output file generated by any ventilation simulator. This program created an output file for recirculation. This program demonstrated the strong capability to detect the recirculation in a sample network and a coal mine ventilation network. The outcomes of this program were documented in this paper.

Axonal regeneration after injuries to the nervous system has been extensively studied due to its implication in motor and sensory functional recovery. Distinct types of regeneration has been identified, such as canonical axonal regeneration, defined as the growth of axons from the transected axonal stump to reinnervate the original target, or regenerative sprouting, in which the growth occurs from a region of the damaged axon either close or far from the injury site(Tuszynski and Steward, 2012).

Influence of wire spacing and plate spacing on electrostatic precipitation of nanoparticles: An approach involving electrostatic shielding and diffusion charging

Electrostatic precipitation is a process widely used as gas cleaning device,to removal particles from gas ***,in a conventional and well-sized precipitator,the collection efficiency decreases for ultrafine particles,making it difficult to employ this equipment for controlling nanoparticle *** paper investigates the influence of plate spacing(4 and 6.5 cm)and wire spacing(4,6,and 12 cm)on the electric current and nanoparticle collection efficiency,considering the effect of diffusion charging and electrostatic *** laboratory-scale dry wire-plate electrostatic precipitators with different plate spacings were tested for the collection of nanoparticles(6.15–241.4 nm)at three air velocities(1.9,2.9,and 3.9 cm/s).The results demonstrated the effectiveness of the equipment in removing nanoparticles(99.9%)under the highest electric *** values of the wire spacing led to increases in the current and the collection *** was associated with reduced electrostatic shielding,which is more evident in smaller ducts with a higher density of field *** is expected that the findings should improve knowledge on electrostatic precipitation of nanoparticles,enabling optimization of collection efficiency by considering the effects of geometric parameters.

Inequalities for the Extended Best Polynomial Approximation Operator in Orlicz Spaces

In this paper we pursue the study of the best approximation operator extended from L~Φ to L~φ, where φ denotes the derivative of the function Φ. We get pointwise convergence for the coefficients of the extended best approximation polynomials for a wide class of function f, closely related to the Calder′on–Zygmund class t_m^p(x) which had been introduced in 1961. We also obtain weak and strong type inequalities for a maximal operator related to the extended best polynomial approximation and a norm convergence result for the coefficients is derived. In most of these results, we have to consider Matuszewska–Orlicz indices for the function φ.
