Background and aim Severe acute respiratory syndrome coronavirus 2(SARS-CoV-2),the virus responsible for the current pandemic,can have multi-organ impact.Recent studies show that liver injury could be a manifestation of the disease,and that liver disease could also be related to a worse prognosis.Our aim was to compare the characteristics of patients with severe coronavirus disease 2019(COVID-19)due to SARS-CoV-2 who required intubation versus stable hospitalized patients to identify the early biochemical predictive factors of a severe course of COVID-19 and subsequent requirement for intubation,specifically in Mexican.Methods This was an observational case-control study nested in a cohort study.Complete medical records of patients admitted for confirmed COVID-19 at a tertiary level center in Mexico City were reviewed.Clinical and biochemical data were collected,and the characteristics of patients who required invasive mechanical ventilation(IMV)(cases)were compared with stable hospitalized patients without ventilation(controls).Results We evaluated 166 patients with COVID-19 due to SARS-CoV-2 infection;114(68.7%)were men,the mean age was 50.6±13.3 years,and 27(16.3%)required IMV.The comparative analysis between cases and controls showed(respectively)significantly lower blood oxygen saturation(SpO_(2))(73.5±12.0%vs.83.0±6.8%,P<0.0001)and elevated alanine aminotransferase(ALT)(128(14–1123)IU/L vs.33(8–453)IU/L,P=0.003),aspartate aminotransferase(AST)(214(17–1247)vs.44(12–498)IU/L,P=0.001),lactic dehydrogenase(LDH)(764.6±401.9 IU/L vs.461.0±185.6 IU/L,P=0.001),and D-dimer(3463(524–34,227)ng/mL vs.829(152–41,923)ng/mL,P=0.003)concentrations.Patients in the cases group were older(58.6±12.7 years vs.49.1±12.8 years,P=0.001).Multivariate analysis showed that important factors at admission predicting the requirement for IMV during hospitalization for COVID-19 were AST≥250 IU/L(odds ratio(OR)=64.8,95%confidence interval(CI)7.5–560.3,P<0.0001)and D-dimer≥3500 ng/mL(OR=4.1,95%CI 1.2–13.7,P=0.02).Conclusions Our study confi

Introduction The Gram-positive bacterium,Streptococcus pneumoniae(S.pneumoniae),often colonizes the nasopharynx;however,it is one of the major microorganisms that cause pneumonia and meningitis.[[1],[2],[3]]It presents several virulence factors,such as surface proteins and enzymes,pneumolysin toxin,and a polysaccharide capsule,which is mainly responsible for the immune response;the capsule also facilitates classification of the bacterium into several serotypes.[1,2]In vulnerable individuals,the bacterium may lead to the development of invasive pneumococcal disease(IPD),which is defined as an infection of normally sterile locations.

Bruton’s tyrosine kinase inhibitors in primary central nervous system lymphoma:New hopes on the horizon

In this editorial,we comment on the article by Wang et al.This manuscript explores the potential synergistic effects of combining zanubrutinib,a novel oral inhibitor of Bruton’s tyrosine kinase,with high-dose methotrexate(HD-MTX)as a therapeutic intervention for primary central nervous system lymphoma(PCNSL).The study involves a retrospective analysis of 19 PCNSL patients,highlighting clinicopathological characteristics,treatment outcomes,and genomic biomarkers.The results indicate the combination’s good tolerance and strong antitumor activity,with an 84.2%overall response rate.The authors emphasize the potential of zanubrutinib to modulate key genomic features of PCNSL,particularly mutations in myeloid differentiation primary response 88 and cluster of differentiation 79B.Furthermore,the study investigates the role of circulating tumor DNA in cerebrospinal fluid for disease surveillance and treatment response monitoring.In essence,the study provides valuable insights into the potential of combining zanubrutinib with HD-MTX as a frontline therapeutic regimen for PCNSL.The findings underscore the importance of exploring alternative treatment modalities and monitoring genomic and liquid biopsy markers to optimize patient outcomes.While the findings suggest promise,the study’s limitations should be considered,and further research is needed to establish the clinical relevance of this therapeutic approach for PCNSL.

Analysis of Naval Ship Evacuation Using Stochastic Simulation Models and Experimental Data Sets

The study of emergency evacuation in public spaces,buildings and large ships may present parallel characteristic in terms of complexity of the layout but there are also significant differences that can hinder passengers to reach muster stations or the lifeboats.There are many hazards on a ship that can cause an emergency evacuation,the most severe result in loss of lives.Providing safe and effective evacuation of passengers from ships in an emergency situation becomes critical.Recently,computer simulation has become an indispensable technology in various fields,among them,the evacuation models that recently evolved incorporating human behavioral factors.In this work,an analysis of evacuation in a Landing Helicopter Dock(LHD)ship was conducted.Escape routes specified by the ship’s procedures were introduced in the model and the six emergency scenarios of the Naval Ship Code were simulated.The crew and embarked troops were introduced with their different evacuation behavior,in addition,walking speeds were extracted from data set collected in experiments conducted at other warships.From the results of the simulations,the longest time was chosen and confidence intervals constructed to determine the total evacuation time.Finally,results show that evacuation time meets regulatory requirements and the usefulness and low cost of the evacuation simulation for testing and refining possible ships’layouts and emergency scenarios.

AIM:To assess the fecal immunochemical test(FIT)accuracy for colorectal cancer(CRC)and advanced neoplasia(AN)detection in CRC screening.METHODS:We performed a multicentric,prospective,double blind study of diagnostic tests on asymptomatic average-risk individuals submitted to screening colonoscopy.Two stool samples were collected and the fecal hemoglobin concentration was determined in the first sample(FIT1)and the highest level of both samples(FITmax)using the OC-sensor.Areas under the curve(AUC)for CRC and AN were calculated.The best FIT1and FITmax cut-off values for CRC were determined.At this threshold,number needed to scope(NNS)to detect a CRC and an AN and the cost per lesion detected were calculated.RESULTS:About 779 individuals were included.An AN was found in 97(12.5%)individuals:a CRC in 5(0.6%)and an advanced adenoma(≥10 mm,villous histology or high grade dysplasia)in 92(11.9%)subjects.For CRC diagnosis,FIT1 AUC was 0.96(95%CI:0.95-0.98)and FITmax AUC was 0.95(95%CI:0.93-0.97).For AN,FIT1 and FITmax AUC were similar(0.72,95%CI:0.66-0.78 vs 0.73,95%CI:0.68-0.79,respectively,P=0.34).Depending on the number of determinations and the positivity threshold cut-off used sensitivity for AN detection ranged between 28%and 42%and specificity between 91%and 97%.At the best cut-off point for CRC detection(115 ng/mL),the NNS to detect a CRC were 10.2 and 15.8;and the cost per CRC was 1814€and 2985€on FIT1 and FITmax strategies respectively.At this threshold the sensitivity,NNS and cost per AN detected were 30%,1.76,and 306€,in FIT1 strategy,and 36%,2.26€and 426€,in FITmax strategy,respectively.CONCLUSION:Performing two tests does not improve diagnostic accuracy,but increases cost and NNS to detect a lesion.

Wood is an organic material that is a source of carbon of organisms called Wood-decay fungi,and to preserve the wood,various toxic compounds to man and the environment have been used.To analyze the effect of N,NDimethylhexadecylamine(DMHDA)on wood attacked by the rotting fungus Trametes versicolor L.We used an in vitro system to expose the fungus T.versicolor to different concentrations of the DMHDA(50,150 and 450µM).We quantified the diameter of mycelial growth and laccase activity,also,under these experimental conditions we studied morphological details of the organisms using different scanning equipment including scanning electron microscopy.The growth of T.versicolor exposed to DMHDA for 60 days,showed a concentration-dependent dose behavior,also,the electron microscopy analysis revealed that the morphology and mycelial density was affected by the DMHDA,showing a formation of atypical morphological and thickener folds.Finally,the pieces of wood treated with DMHDA and exposed to the fungus had a lower mass loss,after a period of 60 days of exposure,the values obtained were 0.7,1.0 and 0.5 g of mass lost for the control,LoC and LoDMHDA treatments respectively.Wood-rot fungi have represented economic losses worldwide,the strategies used have been supported by toxic compounds for the environment.The DMHDA both in the Petri dish system and as a wood preservative was shown to significantly inhibit the growth of T.versicolor.

研究了在机械合金化过程中Ag和Zn以及随后的热处理中的显微组织演变。机械合金化在SPEX8000D铣床中进行。采用X射线衍射(XRD)﹑透射电子显微镜(TEM)和扫描电子显微镜(SEM)观察样品的显微组织。使用差示扫描量热计(DSC)研究样品的热性能。结果表明,在合金化初期的碾磨过程中能观察到ε,β,β′,α固溶体和剩余的Zn。随后,ε,β,β′和Zn相消失,α固溶体中的Zn浓度剧烈升高。在碾磨7.2ks后,机械合金化过程达到稳定状态。在此期间,α固溶体的组分和晶粒尺寸保持不变。后续热处理表明α固溶体也能够通过机械合金化结合热处理得到。最后,为了得到α固溶体,根据碾磨和热处理过程中样品的微观组织演变,提出一个最佳的工艺路线。

Spatial Distribution of Nymphs Populations Bactericera cockerelli Sulc in Tomato Crops(Physalis ixocarpa Brot)

Tomato crops(Physalis ixocarpa Brot.)are produced in almost all Mexico,part of the United States and Central America.Recently the tomato production has suffered economic losses of 70%to 80%due the presence of yellowing and floral abortion,whose causal agent has been attributed to the presence of phytoplasma;an insect vector of these phytoplasma is Bactericera cockerrelli Sulc.Alternative control of this psyllid has lacked effectiveness because their spatial distribution is unknown within tomato plots.This study aimed to determine the spatial distribution of populations of nymphs of B.cockerelli in four tomato plots,the determination of the spatial distribution was performed by means of geostatistics.The experimental semivariogram was determined to adjust to a theoretical model(spherical,exponential or Gaussian)through the program Variowin 2.2,the adjustment was validated with the method of cross-validation and aggregation maps of the pest were obtained through Kriging with Surfer 9.0 program.The short-term time-space stability of the pest was determined through the tests Crámer-von Mises.The results showed that populations of nymphs of B.cockerelli have a distribution of aggregate type,which was corroborated by density maps.Infestations are not present in 100%of the surface of the experimental plots,which helps to direct control measures on specific areas of infestation.

The use of antimicrobial peptides(AMPs)synthesized by bacteria(bacteriocins)is an alternative for combating multidrug resistant bacterial strains and their production by recombinant route is a viable option for their mass production.The bacteriocin E-760 isolated from the genus Enterococcus sp.has been shown to possess inhibitory activity against Gram-negative and Gram-positive bacteria.In this study,the expression of a chimeric protein coding for E-760 in the nucleus of C.reinhardtii was evaluated,as well as,its antibacterial activity.The synthetic gene E-760S was inserted into the genome of C.reinhardtii using Agrobacterium tumefaciens.A transgenic line was identified in TAP medium with hygromycin and also by PCR.The increment in the culture medium temperature of the transgenic strain at 35°C for 10 minutes,increased the production level of the recombinant protein from 0.14(Noninduced culture,NIC)to 0.36%(Induced culture,IC)of total soluble proteins(TSP);this was quantified by an ELISA assay.Recombinant E-760 possesses activity against Staphylococcus aureus in 0.34 U log,Streptococcus agalactiae in 0.48 U log,Enterococcus faecium in 0.36 U log,Pseudomonas aeruginosa in 2 U log and for Klebsiella pneumoniae,the activity was 0.07 U log.These results demonstrate that the nucleus transformation of C.reinhardtii can function as a stable expression platform for the production of the synthetic gene E-760 and it can potentially be used as an antibacterial agent.

Clinical significance of"anti-HBc alone"in human immunodeficiency virus-positive patients

AIM:To determine the prevalence and clinical relevance of isolated antibodies to hepatitis B core antigen as the only marker of infection("anti-HBc alone")among human immunodeficiency virus(HIV) type-1 infected patients.Occult hepatitis B infection frequency was also evaluated. METHODS:Three hundred and forty eight histories from 2388 HIV-positive patients were randomly reviewed.Patients with serological markers of hepatitis B virus(HBV)infection were classified into three groups:past hepatitis,"anti-HBc alone"and chronic hepatitis.Determination of DNA from HBV,and RNA and genotype from hepatitis C virus(HCV)were performed on"anti-HBc alone"patients. RESULTS:One hundred and eighty seven(53.7%) HIV-positive patients had markers of HBV infection: 118 past infection(63.1%),14 chronic hepatitis (7.5%)and 55"anti-HBc alone"(29.4%).Younger age[2.3-fold higher per every 10 years younger;95% confidence intervals(CI)1.33-4.00]and antibodies to HCV infection[odds ratio(OR)2.87;95%CI 1.10-7.48]were factors independently associated with the"anti-HBc alone"pattern.No differences in liver disease frequency were detected between both groups. Serum levels of anti-HBs were not associated with HCV infection(nor viral replication or HCV genotype),or with HIV replication or CD4 level.No"anti-HBc alone" patient tested positive for HBV DNA. CONCLUSION:"Anti-HBc alone"prevalence in HIVpositive patients was similar to previously reported data and was associated with a younger age and with antibodies to HCV infection.In clinical practice,HBV DNA determination should be performed only in those patients with clinical or analytical signs of liver injury.
