Accuracy of gestalt perception of acute chest pain in predicting coronary artery disease

AIM To test accuracy and reproducibility of gestalt to predict obstructive coronary artery disease(CAD)in patients with acute chest *** We studied individuals who were consecutively admitted to our Chest Pain *** admission,investigators performed a standardized interview and recorded14 chest pain *** on these features,a cardiologist who was blind to other clinical characteristics made unstructured judgment of CAD probability,both numerically and *** the reference standard for testing the accuracy of gestalt,angiography was required to rule-in CAD,while either angiography or non-invasive test could be used to *** order to assess reproducibility,a second cardiologist did the same *** In a sample of 330 patients,the prevalence of obstructive CAD was 48%.Gestalt’s numerical probability was associated with CAD,but the area under the curve of0.61(95%CI:0.55-0.67)indicated low level of ***,categorical definition of typical chest pain had a sensitivity of 48%(95%CI:40%-55%)and specificity of 66%(95%CI:59%-73%),yielding a negligible positive likelihood ratio of 1.4(95%CI:0.65-2.0)and negative likelihood ratio of 0.79(95%CI:0.62-1.02).Agreement between the two cardiologists was poor in the numerical classification(95%limits of agreement=-71%to 51%)and categorical definition of typical pain(Kappa=0.29;95%CI:0.21-0.37).CONCLUSION Clinical judgment based on a combination of chest pain features is neither accurate nor reproducible in predicting obstructive CAD in the acute setting.

市政道路生态滤沟雨季积水渗透、旱季保墒是基本功能,而黄土生态滤沟回填土的设计无据可依,常导致景观植被雨季渍害或旱季枯死。为此,通过对重塑黄土进行小尺寸毛细水上升高度试验、大尺寸持水特性模型试验以及非饱和数值分析研究。结果表明:当干密度分别为1.3 g/cm^(3)、1.4 g/cm^(3)时,土柱的最终毛细水上升高度均可达到100 cm。土柱高度超过20 cm后,其含水率基本保持稳定,稳定含水率分别为30.0%、28.0%;在重塑黄土毛细水上升过程中,存在一个暂态饱和区,其中干密度越小,孔隙比越大,大孔隙数量增多,毛细吸力减弱,导致水分迁移速率较慢,同时被湿桥封闭的大气泡数量也较多,因此暂态饱和区的持续时间较长。对于1.3 g/cm^(3)的土柱,其湿润锋时程曲线的模拟值与实测值基本吻合。综合考虑黄土地层稳定的天然平均干密度、毛细水上升高度及其持水特性以及生态滤沟回填土在后期将经历反复的渗透固结与毛细作用,采用1.2 g/cm^(3)作为回填控制干密度,以确保市政道路生态滤沟中的植土在旱季仍能维持植被生长所需的土壤湿度。

Introduction: Cystic lymphangiomas are rare benign malformative tumors of the lymphatic system of obscure etiopathogenesis. The cervico-facial location remains the most common (75%). Although benign, these tumors remain potentially fatal, due to possible compression of the upper aero-digestive tract. The aim of this work is to study the epidemiological, diagnostic and therapeutic characteristics of cervico-mandibular congenital cystic lymphangiomas in the pediatric surgery department of the Donka National Hospital (HND) Conakry. Patients and methods: This is a retrospective and descriptive study of 13 files lasting 7 years from January 2015 to December 31, 2021. The files of children whose age is less than or equal to 15 years operated on cervical tumor with histological evidence of cystic lymphangioma were retained. The data were analyzed using SPSS statistical software 21 and anonymously. Results: The incidence of this study was 1.86 cases per year and a sex ratio of 0.62 in favor of girls. The average age was 8 months 19 days. In the antecedents, we only find poorly monitored pregnancies. The average size of the tumors was 11.85 cm. Cervical ultrasound and standard x-ray of the cervical mass were the only examinations performed. Total surgical excision of the cervical tumor was performed in all patients. The mass was polycystic on exploration. The histological examination of the surgical specimens was in favor of a cystic lymphangioma. The surgical consequences were simple in 11 patients (84.62%) and complicated by parietal suppuration in 2 cases (15.38%). There were no cases of recurrence after one year of follow-up. Conclusion: Cervico-mandibular cystic lymphangiomas are the most frequent locations of congenital lymphangiomas in children. Their severity is linked to the risk of compression of the aero-digestive tracts. Their diagnosis must be confirmed by the histology of the surgical specimen. Web services have gained popularity m recent years anu prowue a new moue, u~ w^u, w,,,~,, ,^- cilitates interaction of scientific and business applications through the Internet. More often, several services with similar functionality are available from a large and changing number of service provid- ers. Quality of Service （QoS） is the dominant factor in service selection and is of great importance to users. In this paper, we propose a model for QoS measurement and web services selection. The model consists of QoS model, QoS monitoring, QoS comparison and service selection with a QoS feedback mechanism. The most suitable service is to take into account the agreed QoS, monitoring is done during invocation phase and if any deviation is recorded, next suitable service is selected. Fi- nally the model is proved to be feasible and effective by simulation experiments.

An adaptive routing protocol based on relative positions in VANETs

After presenting weaknesses of several classic routing protocols applied in vehicular ad hoc networks （VANETs） by a qualitative comparison, this paper proposes a novel self-adaptive routing protocol, named ARPP. With respect to the specific characteristics of VANETs, the proposed routing protocol adopts a dynamic topology establishment and time-varying control message sending mechanism. A direction-based forwarding strategy and a specific warning solution enhance the routing performance in ARPP. Simulation results show that the ARPP protocol outperforms the classic AODV in urban vehicle environment.
