Assessment of multi-modality evaluations of obscure gastrointestinal bleeding

AIMto determine the frequency of bleeding source detection in patients with obscure gastrointestinal bleeding (OGIB) who underwent double balloon enteroscopy (DBE) after pre-procedure imaging [multiphase computed tomography enterography (MPCtE), video capsule endoscopy (VCE), or both] and assess the impact of imaging on DBE diagnostic *** cohort study using a prospectively maintained database of all adult patients presenting with OGIB who underwent DBE from September 1st, 2002 to June 30th, 2013 at a single tertiary *** hundred and ninety five patients (52% females; median age 68 years) underwent DBE for OGIB. AVCE and/or MPCtE performed within 1 year prior to DBE (in 441 patients) increased the diagnostic yield of DBE (67.1% with preceding imaging vs 59.5% without). Using DBE as the gold standard, VCE and MPCtE had a diagnostic yield of 72.7% and 32.5% respectively. there were no increased odds of finding a bleeding site at DBE compared to VCE (OR = 1.3, P = 0.150). there were increased odds of finding a bleeding site at DBE compared to MPCtE (OR = 5.9, P tients with overt OGIB, diagnostic yield of DBE was not affected by preceding *** is a safe and well-tolerated procedure for the diagnosis and treatment of OGIB, with a diagnostic yield that may be increased after obtaining a preceding VCE or MPCtE. However, inpatients with active ongoing bleeding may benefit from proceeding directly to antegrade DBE.

Narrow band imaging evaluation of duodenal villi in patients with and without celiac disease:A prospective study

BACKGROUND Duodenal biopsies are commonly obtained during esophagogastroduodenoscopy(EGD) but are very often histopathologically normal. therefore, a more strategic method for evaluating the duodenal mucosa and avoiding unnecessary biopsies is *** to examine the clinical utility of narrow band imaging(NBI) for evaluating duodenal villous *** We performed a prospective cohort study of adult patients at Mayo Clinic Rochester from 2013-2014 who were referred for EGD with duodenal biopsies. A staff endoscopist scored, in real-time, the NBI-based appearance of duodenal villi into one of three categories(normal, partial villous atrophy, or complete villous atrophy), captured ≥ 2 representative duodenal NBI images, and obtained mucosal biopsies therein. Images were then scored by an advanced endoscopist and gastroenterology fellow, and biopsies(gold standard) by a pathologist, in a masked fashion using the same three-category classification. Performing endoscopist, advanced endoscopist, and fellow NBI scores were compared to histopathology to calculate performance characteristics [sensitivity, specificity,positive and negative, negative predictive value(NPV), and accuracy]. Inter-rater agreement was assessed with Cohen's ***112 patients were included. the most common referring indications were dyspepsia(47%), nausea(23%), and suspected celiac disease(14%). Duodenal histopathology scores were: 84% normal, 11% partial atrophy, and 5% complete atrophy. Performing endoscopist NBI scores were 79% normal, 14% partial atrophy, and 6% complete atrophy compared to 91%, 5%, and 4% and 70%, 24%,and 6% for advanced endoscopist and fellow, respectively. NBI performed favorably for all raters, with a notably high(92%-100%) NPV. NBI score agreement was best between performing endoscopist and fellow(κ = 0.65).CONCLUSION NBI facilitates accurate, non-invasive evaluation of duodenal villi. 本刊准备连载Robinson与Silvia合著的《Digital Signal Proces-sing and time Series Analysis》的节译,目的正是作者们在原序中最后所说的“使实践工程师及科学家能在工作中赶上他本行中的许多新发展”。近十几年来,科学及工程中惊人的发展,有很大一部分是跟数字计算机及数字分析分不开的。在这个“爆炸式”的发展时期,由于人所共知的原因,我们大部分科技人员与知识的门户隔绝了;重要的新发展只有极少数人了解。为了想对社会主义祖国四个现代化尽一点绵力,本刊打算用部分篇幅向读者们提供一些重要的基础知识;许多题目是自成体系的,与系统工程有关的,而且是偏重于电子学领域的。连续登载本书的节译,就是我们系统介绍知识的一点努力。限于篇幅,本刊对许多重要问题只能作参考性的介绍。因此,本书的译稿,我们只能登载其中的叙述部分,而不得不放弃作者们认为最重要的习题及其答案。如果有多数读者认为需要,希望来信,将来也可能将漏载的部分作专刊出版。限于水平,译稿肯定有误谬之处,希望读者发现后赐函,以便订正。

Human activity affects the perception of risk by mule deer

Human activity has been shown to influence how animals assess the risk of predation, but we know little about the spatial scale of such impacts. We quantified how vigilance and flight behavior in mule deer Odocoileus hemionus varied with distance from an area of concentrated human activity--a subalpine field station. An observer walked trails at various distances away from the station looking for deer. Upon encounter, the observer walked toward the focal animal and noted the distance at which it alerted and directed its attention to the approaching human （Alert Distance; AD）, and the distance at which it fled （Flight Initiation Distance;. FID）. AD and FID both increased nonlinearly with distance from the center of the field station, reaching pla- teaus around 250 m and 750 m, respectively. Deer also tended to flee by stotting or running, rather than by walking, when far from the station but they walked away when near the station. these results indicate that deer perceive lower risk near a focused area of human activity, and that vigilance and flight behaviors respond on somewhat different spatial scales. the concept of a spatial ＂human footprint＂ on behavior may be useful for understanding how human activities affect wildlife

Cardiovascular Risk Assessment in the Nursing team of a Cardiology Hospital

Objective: Cardiovascular diseases are the most common causes of morbidity and the leading cause of mortality in the world scenario, accounting for about 20% of all deaths in individuals over 30 years. It has attributed this to the increase in the company’s exposure to risk factors. It identifies cardiovascular risk factors in the nursing team and compares the cardiovascular risk by Framingham score among professionals of middle and upper working in a referral hospital in cardiology. Method: Cross-sectional quantitative study was conducted in 2014, in a reference hospital in cardiology located in Recife/Pernambuco, Brazil. Data of cardiologic risk factors were collected from 82 nursing team members, comprised of technical, auxiliary nurses, and nurses between 30 and 74 years of age. the cardiovascular risk level was evaluated by the Framingham Score. Data were organized and analyzed by means of the SPSS, with descriptive statistics and Student-t test for the continuous variables. Results: Family history, stress and sedentary life style were the prevalent risk factors in more than half of the professionals. Only 5.23% of the technical and auxiliary nurses had a high risk score for cardiovascular event. Conclusion: the data indicates a low cardiovascular risk for these professionals and the presence of risk factors that can be modified. Health surveillance of these professionals is necessary in order to avoid a change to a risk of greater vulnerability.
