Clinical Evaluation of a Vancomycin Dosage Strategy Based on a Serum Trough Concentration Model in Elderly Patients with Severe Pneumonia

Objective This study aimed to evaluate the clinical benefits of a vancomycin dosage strategy based on a serum trough concentration model in elderly *** This prospective single-center, open-label, randomized controlled trial categorized 66 elderly patients with severe pneumonia into study and control groups. The control group received vancomycin using a regimen decided by the attending physician. Meanwhile, the study group received individualized vancomycin therapy with a dosing strategy based on a serum trough concentration model. The primary endpoint was the proportion of patients with serum trough concentrations reaching the target *** secondary endpoints were clinical response, vancomycin treatment duration, and vancomycinassociated acute kidney injury(VA-AKI) *** All patients were at least 60 years old(median age = 81 years). The proportion of patients with target trough concentration achievement(≥ 15 mg/L) with the initial vancomycin regimen was significantly higher in the study group compared to the control group(75.8% vs. 42.4%, P = 0.006).Forty-five patients(68.2%) achieved clinical success, the median duration of vancomycin therapy was10.0 days, and VA-AKI occurred in eight patients(12.1%). However, there were no significant differences in these parameters between the two groups. The model for predicting vancomycin trough concentrations was upgraded to: serum trough concentration(mg/L) = 17.194-0.104 × creatinine clearance rate(mL/min) + 0.313 × vancomycin daily dose [mg/(kg·d)].Conclusion A vancomycin dosage strategy based on a serum trough concentration model can improve the proportion of patients achieving target trough concentrations in elderly patients with severe pneumonia.

Various aluminum-doped zinc oxide （AZO） films were prepared on Si substrate by atomic layer de- position （ALD） at 100 ℃. The effect of the composition of AZO films on their electrical, optical characteristics, structural property and surface topography was investi- gated. The appearance of electrical resistivity shows their semiconducing properties. In most of the visible light band, all the AZO films present transparency of more than 80 %. A1 doping suppresses the AZO film crystallization. When the A1 doping concentration increases up to 3.95 at%, the AZO film has some small multicrystal grains with random orientation. A1 doping improves the roughness of i-ZnO film. The root mean square （RMS） roughness of samples prepared by ALD is much smaller than that pre- pared by radio-frequency magnetron sputtering reported.

The sodium smelting of vanadium-titanium magnetite can achieve the comprehensive utilization of Fe,V,and ***,the generation of alkaline slag during this process may cause damage to refractory *** wettability and corrosion behavior of alkaline slag on three types of refractory(MgO-C,SiC,and high alumina refractory)substrates were investigated at temperatures up to 1200℃.The effects of duration on the wettability of molten slag on SiC substrates were also *** showed that the high alumina refractory exhibited better wettability with the molten slag than the others,and thus,it is easier to be *** results of scanning electron microscopy coupled with energy dispersive spectroscopy showed that MgO-C and high alumina refractory substrates were severely *** was a visible and regular interfacial reaction layer between the slag and SiC refractory substrate,which was produced by the redox reaction between the metal oxides in the slag and the SiC refractory *** the increase in holding time,the interface layer expands and silico-ferrite phases are generated at the *** redox reaction between Fe_(2)O_(3) and SiC substrate is the main reason for the *** comparing the differences in wettability and corrosion behavior between the alkaline slag from sodium smelting of vanadium-titanium magnetite and MgO-C,SiC and high alumina refractories,it is concluded that SiC refractory has good corrosion resistance to the *** oxides in the slag accelerate the oxidation rate of SiC refractory.

We report the transit observations of the ultra-hot Jupiter WASP-121b using the Goodman High Throughput Spectrograph at the 4 m ground-based Southern Astrophysical Research Telescope,covering the wavelength range502-900 *** dividing the target and reference star into 19 spectroscopic passbands and applying differential spectrophotometry,we derive spectroscopic transit light curves and fit them using a Gaussian process framework to determine transit depths for every *** obtained optical transmission spectrum shows a steep increased slope toward the blue wavelength,which seems to be too steep to be accounted for by Rayleigh scattering *** note that the transmission spectrum from this work and other works differ obviously from each other,which was pointed out previously by Wilson et *** evidence for temporal atmospheric *** perform a free chemistry retrieval analysis on the optical transmission spectra from this work and the literature HST/WFC3 NIR *** determine TiO,VO and H_(2)O with abundances of-5.95_(-0.42)^(+0.47)dex,-6.72_(-1.79)^(+0.51)dex and-4.13_(-0.46)^(+0.63)dex,*** compare the abundances of all three of these molecules derived from this work and previous works,and find that they are not consistent with each other,indicating the chemical compositions of the terminator region may change over long *** multi-epoch and high-precision transit observations are required to further confirm this *** note that when combining the transmission spectra in the optical and in NIR in retrieval analysis,the abundances of V and VO,the NIR-to-optical offset and the cloud deck pressure may be coupled with each other.

Effects of Gambogic Acid on Regulation of Steroid Receptor Coactivator-3 in Lung Adenocarcinoma A549 Cells

Objective： To investigate the effects of gambogic acid （GA） on the proliferation and apoptosis of Human lung adenocarcinoma A549 cells in vitro, as well as the regulation of steroid receptor coactivator-3 （SRC-3） to explore the relationship between them. Methods： The effect of GA on the growth of A549 cells was studied by MTT assay. Apoptosis was detected through Hoechst 33258 staining. RT-PCR and Western blot technologies were applied to assess the expression of SRC-3, whereas, the localization of SRC-3 was determined by using confocal microscopy method. Results： GA presented striking proliferation inhibition potency on A549 cells in vitro in a time- and dose-dependent manner, with the IC50 value for 24 h was 3.17±0.13 μmol/L. Hoechst 33258 staining showed that GA could induce apoptosis in A549 cells. Over-expression of SRC-3 was found in A549 cells, whereas the mRNA and protein expression levels of SRC-3 were significantly downregulated in A549 cells induced by GA in a dose-dependent manner. The disposition of SRC-3 was situated mainly at the nuclear. Conclusion： GA may exert its strong anti-leukemia effects through the regulation of the expression of SRC-3. It may be a new target for the therapy of lung cancer.

A palladium-catalyzed multi-acetoxylation of 1,3-disubstituted 1H-pyrazole-5-carboxylate derivatives containing multiple potential reactive sites is described. Therein, the sequence of this process has been appropriately investigated. The protocol mainly provides the di- and tri-acetoxylated products for 1,3- diarylpyrazoles. Besides, it is found that the acetoxylation of C（sp3）-H bond is prior to that of C（sp2）-H bond under structurally competitive conditions.
