Repeated anastomotic recurrence of colorectal tumors: Genetic analysis of two cases

AIM: To investigate genetics of two cases of colorectal tumor local recurrence and throw some light on the etiopathogenesis of anastomotic recurrence. METHODS: Two cases are presented: a 65-year-old female receiving two colonic resections for primary anastomotic recurrences within 21 mo, and a 57-year-old female undergoing two local excisions of recurrent anastomotic adenomas within 26 mo. A loss of heterozygosity (LOH) study of 25 microsatellite markers and a mutational analysis of genes BRAF , K-RAS and APC were performed in samples of neoplastic and normal colonic mucosa collected over the years. RESULTS: A diffuse genetic instability was present in all samples, including neoplastic and normal colonic mucosa. Two different patterns of genetic alterations (LOH at 5q21 and 18p11.23 in the first case, and LOH at 1p34 and 3p14 in the second) were found to be associated with carcinogenesis over the years. A role for the genes MYC-L (mapping at 1p34) and FIHT (mapping at 3p14.2) is suggested, whereas a role for APC (mapping at 5q21) is not shown. CONCLUSION: The study challenges the most credited intraluminal implantation and metachronous carcinogenesis theories, and suggests a persistent, patient-specific alteration as the trigger of colorectal cancer anastomotic recurrence.

Spatiotemporal variation in the adult sex ratio,male aggregation,and movement of two tropical cloud forest dung beetles

While theory suggests that at conception the sex ratio should be balanced(1:1),this can be variable across space and time in wild ***,studies of the environmental factors that regulate adult sex ratio(ASR)in species with different life history traits are *** capture–recapture over a year,we analyzed the influence of habitat type(forest and nonforest)and season(rainy and dry)on variation in ASR,male aggregation and the trajectory movement of 2 dung beetle species with different life history traits:Deltochilum mexicanum(a hornless roller species)and Dichotomius satanas(a tunneler species with horns on its head and thorax).We found opposite *** *** population tends to be female-biased,but the population of *** tends to be predominantly male,and observed values were not related to habitat type or ***,the 95%confidence intervals estimated were highly variable between seasons depending on *** examining the monthly variation in ASR for both habitats,we found that it depends on the *** addition,male aggregation differed between species depending on habitat type and season,and species movement patterns were closely related to their habitat *** on our results,we argue that comparative population studies of species with different life history traits are necessary to understand the variation in demographic parameters as well as its ecological and evolutionary implications in the face of spatial and climatic environmental variation.

Assessment of groundwater contamination risk in an agricultural area in north Italy

In the present study a specific approach is followed,considering the Pesticide DRASTIC and Susceptibility index(SI)methods and a GIS framework,to assess groundwater vulnerability in the agricultural area of Albenga,in north *** results indicate"high"to"very high"vulnerability to groundwater contamination along the coastline and the middle part of the Albenga plain,for almost 49%and 56%of the total study area for Pesticide DRASTIC and SI methods,*** sensitive regions depict characteristics such as shallow depth to groundwater,extensive deposits of alluvial silty clays,flat topography and intensive agricultural *** distribution of nitrates concentration in groundwater in the study area is slightly better correlated with the SI(0.728)compared to Pesticide DRASTIC(0.693),thus indicating that both methods are characterized by quite good *** analysis was also performed to acknowledge statistical uncertainty in the estimation of each parameter used,assess its impact and thus identify the most critical parameters that require further investigation in the *** to water is the parameter that exhibited the largest impact on the Pesticide DRASTIC vulnerability index followed by the impact of the vadose zone and *** the other hand,the SI method is more sensitive to the removal of the topography parameter followed by the aquifer media and the depth to water parameters.

Multicomponent models based on the Lattice Boltzmann Method(LBM)have clear advantages with respect to other approaches,such as good parallel performances and scalability and the automatic resolution of breakup and coalescence *** flow simulations are useful for a wide range of applications,yet many multicomponent models for LBMare limited in their numerical stability and therefore do not allow exploration of physically relevant low viscosity *** we performa quantitative study and validations,varying parameters such as viscosity,droplet radius,domain size and acceleration for stationary and translating droplet simulations for the color-gradientmethod with centralmoments(CG-CM)formulation,as this method promises increased numerical stability with respect to the *** focus on numerical stability and on the effect of decreasing grid-spacing,*** resolution,in the extremely low viscosity regime for stationary droplet *** effects of small-and large-scale anisotropy,due to grid-spacing and domain-size,respectively,are investigated for a stationary *** effects on numerical stability of applying a uniform acceleration in one direction on the domain,*** both the droplet and the ambient,is explored into the low viscosity regime,to probe the numerical stability of the method under dynamical conditions.

Antiplatelet therapy in very elderly and comorbid patients with acute coronary syndromes

With population ageing and rise of life expectancy,a progressively increasing proportion of patients presenting with an acute coronary syndrome(ACS)are older adults,including those at extreme chronological *** amounts of data,including randomized clinical trials,have shown that the benefits of an early revascularization are maintained also at very old age,resulting in improved outcome after an acute coronary *** the contrary,the optimal antiplatelet therapy(APT)remains unclear in these patients,because of both safety and efficacy ***,age-related multiple organ dysfunction and high prevalence of comorbidities may on the one hand reduce the therapeutic effects of administered drugs;on the other hand,it leads to increased vulnerability to drug toxicity and side ***,management of APT is particularly challenging in elderly patients because of higher risk of both ischemic and bleeding *** aim of the present paper is to review the current evidence,gaps in knowledge and ongoing research regarding APT in the setting of an ACS in elderly and very elderly patients,and in those with significant comorbidities including chronic kidney disease,diabetes mellitus and frailty.

Plants need to respond quickly and appropriately to various types of light signals from the environment to optimize growth and development. The immediate response to shading, reduced photon flux （low light）, and changes in spectral quality involves changes in gene regulation. In the case of more persistent shade, the plant shows a dramatic change in the organization of chromatin. Both plant responses are controlled via photoreceptor signaling proteins. Re- cently, several studies have revealed similar features of chromatin reorganization in response to various abiotic and biotic signals, while others have unveiled intricate molecular networks of light signaling towards gene regulation. This opinion paper briefly describes the chromatin （de）compaction response from a light-signaling perspective to provide a link be- tween chromatin and the molecular network of photoreceptors and E3 ubiquitin ligase complexes.

Electrical breakdown of an acrylic dielectric elastomer: effects of hemispherical probing electrode’s size and force

Dielectric elastomers are widely investigated as soft electromechanically active polymers(EAPs)for actuators,stretch/force sensors,and mechanical energy harvesters to generate *** the performance of such devices is limited by the dielectric strength of the constitutive material,the electrical breakdown of soft elastomers for electromechanical transduction is still scarcely ***,we describe a custom-made setup to measure electrical breakdown of soft EAPs,and we present data for a widely studied acrylic elastomer(VHB 4905 from 3M).The elastomer was electrically stimulated via a planar and a hemispherical metal *** breakdown was characterized under different conditions to investigate the effects of the radius of curvature and applied force of the hemispherical *** a given radius of curvature,the breakdown field increased by about 50% for a nearly 10-fold increase of the applied mechanical stress,while with a given mechanical stress the breakdown field increased by about 20% for an approximately two fold increase of the radius of *** results indicate that the breakdown field is highly dependent on the boundary conditions,suggesting the need for reporting breakdown data always in close association with the measurement *** findings might help future investigations in elucidating the ultimate breakdown mechanism/s of soft elastomers.

The presence of dome-and-keel provinces in Archean cratons has been connected with the initiation of plate tectonics on Earth as these features are most commonly observed in Archean *** Quadrilátero Ferrífero in Brazil has been identified as a Paleoproterozoic dome-and-keel province for more than three *** prevailing model suggests that it formed during the Rhyacian Transamazonian orogeny,making it unique among dome-and-keel ***,a lack of appropriate lithologies,datable minerals and the metamorphic overprint of later orogenesis has resulted in a cryptic metamorphic record for the formation of this dome-and-keel province.A clinopyroxene-bearing migmatite from the core of the Bacao dome has peak P-T conditions of 5-7 kbar and 700-750 ℃ and a published age of ca.2730 Ma based on U-Pb ages of zircon from leucosomes,suggesting that this age represents the migmatisation event.A fine-grained epidote-albite-titanite assemblage overprints the coarse-grained clinopyroxene and amphibole,giving P-T conditions of 8-9 kbar and 550℃ with an associated titanite age of ca.2050 Ma.A garnet-bearing amphibolite sample also from the core of the dome has peak P-T conditions of 7-8 kbar and 650-700℃,and texturally late titanite from this sample produces an age of ca.2060 *** additional samples were collected from the edges of the dome.A garnet-gedrite bearing felsic schist produces peak P-T conditions of 8-9 kbar and 650-700℃ on a clockwise P-T *** sample has a U-Pb zircon age of ca.2775 Ma,which could date metamorphism or be the age of its volcaniclastic *** unconstrained titanite from the sample gives an age of ca.2040 Ma.A garnet-bearing amphibolite that occurs as a boudin within the felsic schist gives both zircon and titanite ages of ca.2050 Ma and has peak P-T conditions of 5-6 kbar and 650-700 ℃ on a near isobaric P-T *** amphibolite dike,observed to cross-cut the felsic schist produces a zircon U-Pb age of ca.2760 Ma

Multiple giant diverticula of the foregut causing upper gastrointestinal obstruction

Small bowel diverticulosis represents an uncommon disorder (except for Meckel diverticulum) often misdiagnosed since it causes non-specific gastroin- testinal symptoms. Most of times the diagnosis is carried out in case of related complications, such as diverticulitis, hemorrhage, perforation or obstruction. Intestinal obstruction can be caused by inflammatory stenosis due to repeated episodes of diverticulitis, volvulus, intussusception or jejunal stones. Herein we report a case of multiple jejunal diverticula causing chronic gastrointestinal obstruction.
