Liver Lead Levels in Snow Goose (Chen caerulescens) in a Wetland near the City of Durango, Mexico

The use of lead in ammunition for hunting exposes waterfowl to lead poisoning (plumbism) by accidental consumption of shotgun pellets. To test this hypothesis we sampled 18 liver tissue samples of Snow Goose (Chen caerulescens) collected during the 2012-2013 hunting season in a wetland near the city of Durango, Mexico. We quantified liver lead levels using an atomic absorption spectrophotometer and portions of liver were fixed and stained for their histological study. Average lead concentration (in dry weight) were under the normal range (mean = 0.73 ± 0.2, standard error) which do not represent any risk of poisoning. Liver tissue injuries were not observed in the histopathological analysis, suggesting no reaction to a xenobiotic agent such as lead. Gastrointestinal content analysis showed lead pellet in the gizzard of one individual, but we could not find a relationship between pellet ingestion and lead concentration in the liver. Although the results did not provide evidences of lethal or sublethal effects caused by lead poisoning, they show a possible risk due to the presence of lead pellets in the digestive tract.

AIM To investigate any changing trends in the etiologies of hepatocellular carcinoma(HCC) in Argentina during the last years. METHODS A longitudinal cohort study was conducted by 14 regional hospitals starting in 2009 through 2016. All adult patients with newly diagnosed HCC either with pathology or imaging criteria were included. Patients were classified as presenting non-alcoholic fatty liver disease(NAFLD) either by histology or clinically, provided that all other etiologies of liver disease were ruled out, fatty liver was present on abdominal ultrasound and alcohol consumption was excluded. Complete follow-up was assessed in all included subjects since the date of HCC diagnosis until death or last medical visit.RESULTS A total of 708 consecutive adults with HCC were included. Six out of 14 hospitals were liver transplant centers(n = 484). The prevalence of diabetes mellitus was 27.7%. Overall, HCV was the main cause of liver disease related with HCC(37%) including cirrhotic and non-cirrhotic patients, followed by alcoholic liver disease 20.8%, NAFLD 11.4%, cryptogenic 9.6%, HBV 5.4% infection, cholestatic disease and autoimmune hepatitis 2.2%, and other causes 9.9%. A 6-fold increase in the percentage corresponding to NAFLDHCC was detected when the starting year, i.e., 2009 was compared to the last one, i.e., 2015(4.3% vs 25.6%; P < 0.0001). Accordingly, a higher prevalence of diabetes mellitus was present in NAFLD-HCC group 61.7% when compared to other than NAFLD-HCC 23.3%(P < 0.0001). Lower median AFP values at HCC diagnosis were observed between NAFLD-HCC and non-NAFLD groups(6.6 ng/m L vs 26 ng/m L; P = 0.02). Neither NAFLD nor other HCC etiologies were associated with higher mortality.CONCLUSION The growing incidence of NAFLD-HCC documented in the United States and Europe is also observed in Argentina, a confirmation with important Public Health implications.

History of Infection Prior to Hematopoietic Stem Cell Transplant in the Recipient as a Risk Factor to Develop Acute Graft versus Host Disease

Background: Acute graft-versus-host disease (aGVHD) is the most frequent and severe complication after allogeneic hematopoietic stem cell transplantation (HSCT). Objective: To determine if a history of prior infection in the allogeneic HSTC recipient is a risk factor to develop aGVHD. Methods: A retrospective cohort study based on data collected from the Department of Hematopoietic Stem Cell Transplantation at the Instituto Nacional de Pediatría (INP) from January 1998 to December 2016 was performed to identify if prior infection was a predictive risk factor for aGVHD. Results: 27 patients developed aGVHD (36.4%). Median time to aGVHD presentation was 82 days (9 to 273 days). Most patients developed grade III aGVHD. Following the multivariate analysis peripheral blood > bone marrow (OR 12.3;p 8.3 × 106/kg (OR 7.1;p = 0.05), peripheral blood (OR: 11.4;p < 0.001), infection 3 months prior to allogeneic transplant (OR: 4.5;p < 0.03) and CMV infection in the recipient (OR: 4.68;p Conclusions: Either bacterial infection or CMV infection in the recipient was significant risk factor within the aGVHD recipients;it is important to consider these factors for patients that are going to receive an allogeneic HSCT.

Occult hepatitis B virus infection among Mexican human immunodeficiency virus-1-infected patients

AIM: To determine the frequency of occult hepatitis B infection(OHBI) in a group of human immunodeficiency virus(HIV)-1+/ hepatitis B surface antigen negative(HBsAg)- patients from Mexico. METHODS: We investigated the presence of OHBI in 49 HIV-1+/HBsAg- patients. Hepatitis B virus(HBV) DNA was analyzed using nested PCR to amplify the Core(C) region and by real-time PCR to amplify a region of the S and X genes. The possible associations between the variables and OHBI were investigated using Pearson's χ2 and/or Fisher's exact test.RESULTS: We found that the frequency of OHBI was 49% among the group of 49 HIV-1+/HBsAg- patients studied. The presence of OHBI was significantly associated with the HIV-1 RNA viral load [odds ratio(OR) = 8.75; P = 0.001; 95%CI: 2.26-33.79] and with HIVantiretroviral treatment with drugs that interfere with HBV replication(lamivudine, tenofovir or emtricitabine)(OR = 0.25; P = 0.05; 95%CI: 0.08-1.05). CONCLUSION: The OHBI frequency is high among 49 Mexican HIV-1+/HBsAg- patients and it was more frequent in patients with detectable HIV RNA, and less frequent in patients who are undergoing HIV-ARV treatment with drugs active against HBV.
