AIM To investigate the microvascular(skeletal muscle tissue oxygenation; SmO_2) response to transfusion in patients undergoing elective complex spine *** After IRB approval and written informed consent, 20 patients aged 18 to 85 years of age undergoing > 3level anterior and posterior spine fusion surgery were enrolled in the study. patients were followed throughout the operative procedure, and for 12 h postoperatively. In addition to standard American Society of Anesthesiologists monitors, invasive measurements including central venous pressure, continual analysis of stroke volume(SV), cardiac output(CO), cardiac index(CI), and stroke volume variability(SVV) was performed. To measure skeletal muscle oxygen saturation(SmO_2) during the study period, a non-invasive adhesive skin sensor based on Near Infrared Spectroscopy was placed over the deltoid muscle for continuous recording of optical spectra. All administration of fluids and blood products followed standard procedures at the Hospital for Special Surgery, without deviation from usual standards of care at the discretion of the Attending Anesthesiologist based on individual patient comorbidities, hemodynamic status, and laboratory data. Time stamps were collected for administration of colloids and blood products, to allow for analysis of SmO_2 immediately before, during, and after administration of these fluids, and to allow for analysis of hemodynamic data around the same time points. Hemodynamic and oxygenation variables were collected continuously throughout the surgery, including heart rate, blood pressure, mean arterial pressure, SV, CO, CI, SVV, and SmO_2. Bivariate analyses were conducted to examine the potential associations between the outcome of interest, SmO_2, and each hemodynamic parameter measured using pearson's correlation coeffi-cient, both for the overall cohort and within-patients individually. The rapid growth of spine degenerative surgery has led to unrelenting efforts to define and prevent possible complications, the incidence of which is probably higher than that reported and varies according to the region of the spine involved(cervical and thoracolumbar) and the severity of the surgery. Several issues are becoming progressively clearer, such as complication rates in primary versus revision spinal surgery, complications in the elderly, the contribution of minimally invasive surgery to the reduction of complication rate. In this paper the most common surgical complications in degenerative spinal surgery are outlined and discussed.

AIM: To determine the hypothesis that inflating the balloons in the duodenal papilla determines changes in the biochemical markers of ***: Four groups of pigs were used: Group papilla(Gp), the overtube's balloon was inflated in the area of the papilla; Gp + double balloon enteroscopy(Gp + DBE), the overtube's balloon was kept inflated in the area of the papilla for 20 min before a DBE; Group DBE(GDBE), DBE was carried out after insuring the balloon's inflation far from the pancreatic papilla; and Group control(GC). Serum concentrations of amylase, lipase and C-reactive protein(CRp) were evaluated. pancreases were processed for histopathology ***: Main changes occurred 24 h after the procedure compared with baseline levels. Amylase levels increased significantly in Gp(59.2% higher) and were moderately higher in groups Gp + DBE and GDBE(22.7% and 20%, respectively). Lipase increased in Gp and Gp + DBE, whereas it hardly changed in GDBE and in GC. CRp increased significantly in Gp, Gp + DBE and GDBE, while no changes were reported for GC. No statistically significant difference between groups Gp and Gp + DBE was found for the histopathological findings, except for vacuolization and necrosis of the pancreatic parenchyma that was higher in Gp than in Gp + ***: The manipulation of the duodenal papilla by the inflated overtube's balloon during DBE causes pancreatic structural damage and increased biochemical markers associated with pancreatitis.

The high demand for lung transplants cannot be matched by an adequate number of lungs from donors. Since fully ex-novo lungs are far from being feasible, tissue engineering is actively considering implantation of engineered lungs where the devitalized structure of a donor is used as scaffold to be repopulated by stem cells of the receiving patient. A decellularized donated lung is treated inside a bioreactor where transport through the tracheobronchial tree (TBT) will allow for both deposition of stem cells and nourishment for their subsequent growth, thus developing new lung tissue. The key concern is to set optimally the boundary conditions to utilize in the bioreactor. We propose a predictive model of slow liquid ventilation, which combines a one-dimensional (1-D) mathematical model of the TBT and a solute deposition model strongly dependent on fluid velocity across the tree. With it, we were able to track and drive the concentration of a generic solute across the airways, looking for its optimal distribution. This was given by properly adjusting the pumps’ regime serving the bioreactor. A feedback system, created by coupling the two models, allowed us to derive the optimal pattern. The TBT model can be easily invertible, thus yielding a straightforward flow/pressure law at the inlet to optimize the efficiency of the bioreactor.

The performances of the Color-Gradient(CG)and the Shan-Chen(SC)multicomponent Lattice Boltzmann models are quantitatively compared side-by-side on multiple physical flow problems where breakup,coalescence and contraction of fluid ligaments are *** flow problems are relevant to microfluidic applications,jetting of microdroplets as seen in inkjet printing,as well as emulsion dynamics.A significantly wider range of parameters is shown to be accessible for CG in terms of density-ratio,viscosity-ratio and surface tension *** stability for a high density ratio O(1000)is required for simulating the drop formation process during inkjet printing which we show here to be achievable using the CG model but not using the SC *** results show that the CG model is a suitable choice for challenging simulations of droplet formation,due to a combination of both numerical stability and physical *** also present a novel approach to incorporate repulsion forces between interfaces for CG,with possible applications to the study of stabilized ***,we show that the CG model can produce similar results to a known multirange potentials extension of the SC model for modelling a disjoining pressure,opening up its use for the study of dense stabilized emulsions.
