Sustained low diffusing capacity in hepatopulmonary syndrome after liver transplantation

AIM： To study the presence of sustained low diffusing capacity （DLco） after liver transplantation （LT） in patients with hepatopulmonary syndrome （HpS）. METHODS： Six patients with mild-to-severe HpS and 24 without HpS who underwent LT were prospectively followed before and after LT at mid-term （median, 15 mo）. HpS patients were also assessed at Iong-tem （median, 86 mo）. RESULTS： Before LT, HpS patients showed lower paO2 （71 ± 8 mmHg）, higher AapO2 （43 ± 10 mmHg） and lower DLco （54% ± 9% predicted）, due to a combination of moderate-to-severe ventilation-perfusion （VA/Q） imbalance, mild shunt and diffusion limitation, than non- HpS patients （94 ± 4 mmHg and 19 ± 3 mmHg, and 85% ± 3% predicted, respectively）（p 〈 0.05 each）. Seven non-HpS patients had also reduced DLco （70% ± 4% predicted）. At mid- and long-term after LT, compared to pre- LT, HpS patients normalized paO2 （91 ± 3 mmHg and 87 ± 5 mmHg）, AapO2 （14 ± 3 mmHg and 23 ± 5 mmHg） and all VA/Q descriptors （p 〈 0.05 each） without changes in DLco （53% ± 8% and 56% ± 7% predicted, respectively）. post-LT DLco in non-HpS patients with pre- LT low DLco was unchanged （75% ± 6% predicted）. CONCLUSION： While complete VA/Q resolution in HpS indicates a reversible functional disturbance, sustained low DLco after LT also present in some non-HpS patients, points to persistence of sub-clinical liver-induced pulmonary vascular changes.

Background:platinum chemotherapy(CT)remains the backbone of systemic therapy for patients with smallcell lung cancer(SCLC).The nucleotide excision repair(NER)pathway plays a central role in the repair of the DNA damage exerted by platinum *** in this repair mechanism may affect patients’*** and Methods:We conducted a retrospective analysis of data from 38 patients with extensive disease(ED)-SCLC who underwent platinum-CT at the Clinical Oncology Unit,Careggi University Hospital,Florence(Italy),from 2015 to *** expression analysis and single nucleotide polymorphism(SNp)characterization of three NER pathway genes—namely ERCC1,ERCC2,and ERCC5—were performed on patient tumor ***:Overall,elevated expression of ERCC genes was observed in SCLC patients compared to healthy *** with low ERCC1 and ERCC5 expression levels exhibited a better median progression-free survival(mpFS=7.1 vs.4.9 months,p=0.39 for ERCC1 and mpFS=6.9 vs.4.8 months,p=0.093 for ERCC5)and overall survival(mOS=8.7 vs.6.0 months,p=0.4 for ERCC1 and mOS=7.2 vs.6.2 months,p=0.13 for ERCC5).genotyping analysis of five SNps of ERCC genes showed a longer survival in patients harboring the wild-type genotype or the heterozygous variant of the ERCC1 rs11615 SNp(p=0.24 for pFS and p=0.14 for OS)and of the rs13181 and rs1799793 ERCC2 SNps(p=0.43 and p=0.26 for pFS and p=0.21 and p=0.16 for OS,respectively)compared to patients with homozygous mutant ***:The comprehensive analysis of ERCC gene expression and SNp variants appears to identify patients who derive greater survival benefits from platinum-CT.

Spain has been one of the main global pandemic epicenters for coronavirus disease 2019(COVID-19).Here,we analyzed>41000 genomes(including>26000 high-quality(HQ)genomes)downloaded from the gISAID repository,including 1245(922 HQ)sampled in *** aim of this study was to investigate genome variation of novel severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)and reconstruct phylogeographic and transmission patterns in *** analysis suggested at least 34 independent introductions of SARS-CoV-2 to Spain at the beginning of the *** lineages spread very successfully in the country,probably favored by super-spreaders,namely,A2 a4(7.8%),A2 a5(38.4%),A2 a10(2.8%),B3 a(30.1%),and B9(8.7%),which accounted for 87.9% of all genomes in the Spanish *** distinct feature of the Spanish SARS-Cov-2 genomes was the higher frequency of B lineages(39.3%,mainly B3 a+B9)than found in any other European *** B3 a,B9,(and an important sub-lineage of A2 a5,namely,A2 a5 c)most likely originated in Spain,the other three haplogroups were imported from other European *** B3 a strain may have originated in the Basque Country from a B3 ancestor of uncertain geographic origin,whereas B9 likely emerged in *** time of the most recent common ancestor(TMRCA)of SARS-CoV-2 suggested that the first coronavirus entered the country around 11 February 2020,as estimated from the TMRCA of B3 a,the first lineage detected in the ***,earlier claims that the D614 g mutation is associated to higher transmissibility is not consistent with the very high prevalence of COVID-19 in Spain when compared to other countries with lower disease incidence but much higher frequency of this mutation(56.4% in Spain vs.82.4% in rest of Europe).Instead,the data support a major role of genetic drift in modeling the micro-geographic stratification of virus strains across the country as well as the role of SARS-CoV-2 super-spreaders.

Multicomponent models based on the Lattice Boltzmann Method(LBM)have clear advantages with respect to other approaches,such as good parallel performances and scalability and the automatic resolution of breakup and coalescence *** flow simulations are useful for a wide range of applications,yet many multicomponent models for LBMare limited in their numerical stability and therefore do not allow exploration of physically relevant low viscosity *** we performa quantitative study and validations,varying parameters such as viscosity,droplet radius,domain size and acceleration for stationary and translating droplet simulations for the color-gradientmethod with centralmoments(Cg-CM)formulation,as this method promises increased numerical stability with respect to the *** focus on numerical stability and on the effect of decreasing grid-spacing,*** resolution,in the extremely low viscosity regime for stationary droplet *** effects of small-and large-scale anisotropy,due to grid-spacing and domain-size,respectively,are investigated for a stationary *** effects on numerical stability of applying a uniform acceleration in one direction on the domain,*** both the droplet and the ambient,is explored into the low viscosity regime,to probe the numerical stability of the method under dynamical conditions.

Effect of the manipulation of the duodenal papilla during double balloon enteroscopy

AIM: To determine the hypothesis that inflating the balloons in the duodenal papilla determines changes in the biochemical markers of ***: Four groups of pigs were used: group papilla(gp), the overtube's balloon was inflated in the area of the papilla; gp + double balloon enteroscopy(gp + DBE), the overtube's balloon was kept inflated in the area of the papilla for 20 min before a DBE; group DBE(gDBE), DBE was carried out after insuring the balloon's inflation far from the pancreatic papilla; and group control(gC). Serum concentrations of amylase, lipase and C-reactive protein(CRp) were evaluated. pancreases were processed for histopathology ***: Main changes occurred 24 h after the procedure compared with baseline levels. Amylase levels increased significantly in gp(59.2% higher) and were moderately higher in groups gp + DBE and gDBE(22.7% and 20%, respectively). Lipase increased in gp and gp + DBE, whereas it hardly changed in gDBE and in gC. CRp increased significantly in gp, gp + DBE and gDBE, while no changes were reported for gC. No statistically significant difference between groups gp and gp + DBE was found for the histopathological findings, except for vacuolization and necrosis of the pancreatic parenchyma that was higher in gp than in gp + ***: The manipulation of the duodenal papilla by the inflated overtube's balloon during DBE causes pancreatic structural damage and increased biochemical markers associated with pancreatitis.

The performances of the Color-gradient(Cg)and the Shan-Chen(SC)multicomponent Lattice Boltzmann models are quantitatively compared side-by-side on multiple physical flow problems where breakup,coalescence and contraction of fluid ligaments are *** flow problems are relevant to microfluidic applications,jetting of microdroplets as seen in inkjet printing,as well as emulsion dynamics.A significantly wider range of parameters is shown to be accessible for Cg in terms of density-ratio,viscosity-ratio and surface tension *** stability for a high density ratio O(1000)is required for simulating the drop formation process during inkjet printing which we show here to be achievable using the Cg model but not using the SC *** results show that the Cg model is a suitable choice for challenging simulations of droplet formation,due to a combination of both numerical stability and physical *** also present a novel approach to incorporate repulsion forces between interfaces for Cg,with possible applications to the study of stabilized ***,we show that the Cg model can produce similar results to a known multirange potentials extension of the SC model for modelling a disjoining pressure,opening up its use for the study of dense stabilized emulsions.

Optimal flow conditions of a tracheobronchial model to reengineer lung structures

The high demand for lung transplants cannot be matched by an adequate number of lungs from donors. Since fully ex-novo lungs are far from being feasible, tissue engineering is actively considering implantation of engineered lungs where the devitalized structure of a donor is used as scaffold to be repopulated by stem cells of the receiving patient. A decellularized donated lung is treated inside a bioreactor where transport through the tracheobronchial tree (TBT) will allow for both deposition of stem cells and nourishment for their subsequent growth, thus developing new lung tissue. The key concern is to set optimally the boundary conditions to utilize in the bioreactor. We propose a predictive model of slow liquid ventilation, which combines a one-dimensional (1-D) mathematical model of the TBT and a solute deposition model strongly dependent on fluid velocity across the tree. With it, we were able to track and drive the concentration of a generic solute across the airways, looking for its optimal distribution. This was given by properly adjusting the pumps’ regime serving the bioreactor. A feedback system, created by coupling the two models, allowed us to derive the optimal pattern. The TBT model can be easily invertible, thus yielding a straightforward flow/pressure law at the inlet to optimize the efficiency of the bioreactor.

Biomechanical analysis of the "running" vs. "conventional" diagonal stride uphill techniques as performed by elite cross-country skiers

Water sorption thermodynamics in glassy polymers endowed with hydrogen bonding interactions

In this contribution, we review and critically compare the results of the analyses we have previously performed on water sorption thermodynamics in a series of polyimides. The experimental investigation was performed by combining gravimetric tests and in situ vibrational spectroscopy. A non-equilibrium theory, based on a compressible lattice framework accounting for the glassy state of the polymer and for the occurrence of hydrogen bonding interactions, has been used to interpret data. Information at a molecular level gained by vibrational spectroscopy has been used to tailor the model equations. The main features of water sorption thermodynamics are well captured, qualitatively and quantitatively, by the adopted model which displays a remarkable agreement with experimental results.
